Patients presenting with dialysis-dependent renal failure have poor renal ou tcomes. There is limited data regarding the clinical presentation an d outcomes of antiGBM disease from India. Aim : To study the incidence, clinical, biochemical and pathological characteristics and ou tcome of patients with anti-glomerular basement membrane (anti-GBM) a ntibody disease. Methods and Material: This is a retrospective study conducted by screening renal biopsy reports of patients presente d with rapidly progressing glomerulonephritis (RPGN) over a period f 45 months(JAN 2015-OCT 2018) Those patients who had histopathological features suggestive of ANTI GBM d isease or those who had positive ANTI-GBM antibody titers were furt her analyzed. Their records were reviewed for the duration of sym ptoms before presentation, clinical features, and biochemical, p thology and serology reports. Follow up details were noted. Results: A total of 97 patients presented with RPGN during the period of 45 months (Jan 2015 to Oct 2018). Anti GBM disease (10/97) constituted 10.30% of rapidly progressive glomerular disease. Males and females w re equally affected (M: F 5:5), males mean age; 43.6+ 6.9 yrs and females mean age 28.8+ 10.1 yrs. The presenting symptoms were pedal edema (80%), oliguria (60%), hematuria (40%) and hemoptysis (20% ). Kidney biopsy was done after a mean period of 10.7+ 7.2 days after first presentation. Mean sr. creatinine was 7.7+ 3.3 mg/dl, Only 2 patients had a creatinine of <5 mg /dl. Eight patients screened for circulati ng anti-GBM antibodies were positive and one patient positive f or p-ANCA. Mean crescents per biopsy specimen were 71 %, 3 had 100% crescents (30%). Conclusions: Anti GBM disease constituted 10.30% of rapidly progressive GN. Males and younger females were pred ominantly affected .All patients had >50% crescents and sever e r nal failure at presentation. Unfortunately, they are more frequently seen in a rural area. PKP is indicated as gold standard surgery in full-t hickness corneal involvement as: nonhealing corneal ulcers, penetrat ing corneal trauma, dystrophies, degenerations and failed lamellar kera topl sties. This procedure is a well-established, effective and give s faster visual rehabilitation. Still, it has many postoperative co mplications like graft failure, graft rejection, cataract formation, glauc oma, and corneal vascularization. Purpose: To evaluate surgical outcome of penetrating keratop l sty done in adults having full thickness corneal lesion at a rural hospital in relation to 1) Clinical graft clarity 2) Best corrected visual acuity and 3) Occurrence of postoperative complications and indic ation for PKPs. Methods: In this hospital-based, a prospective study we stu died all cases that underwent PKP in adults at PRH Loni, by trained surgeons during a total 10 years period i.e. from January 20 08 to December 2017. Results: We studied 114 PKPs. There were 68 (59.64%) males. Clear graft in 59 (51.75%) in all PKPs and in optical PKP s graft clarity was 61.84%. Postoperative complications in 55 (48.24%). Best corrected V/A > 6/18 in 24 cases (21.05 %) more than 6/60 in 64 cases (56.14%) and >CF 3 Meter in 93 patients (81.58 %). Conclusions: Post PKP graft clarity and BCVA depends on indica tion for PKP. {"title":"MICROALBUMINURIA REDUCTION WITH EARLY PHARMACOTHERAPEUTIC INTERVENTION FOR BLOOD PRESSURE CONTROL IN CASES OF HYPERTENSIVE T2DM SUBJECTS","authors":"S. Sud","doi":"10.26838/MEDRECH.2019.6.3.493","DOIUrl":"https://doi.org/10.26838/MEDRECH.2019.6.3.493","url":null,"abstract":"Article History Received: April’ 2019 Accepted: May’ 2019 Objective: A prospective randomized observational study to assess the importance of early intervention with pharmacotherapy to control B.P. in T2DM having microalbuminuria, and its reduction in comparison to a delayed initiation for the same. Materials and methods: Patients with T2DM, HTN, and Microalbuminuria, on treatment for Diabetes but drug naïve for HTN were selected and divided into two groups. Group A was advised LSM for B.P. control but no pharmacotherapy. Group B gave the same advice plus Azilsartan 40 mg/day. UACR, B.P., and HbA1c were recorded on every visit. Second visit (3 months) Group A added on Azilsartan 40 mg/day to control the B.P. to target. Group B advised titration of Azilsartan dosage for the same. On the third visit (6 months) all the three parameters were recorded as before. Results: (n=66) completed the study. (A=36 and B=30). The data analysis showed that at the end of 6 months ~33% of the patients in Group A could become non-microalbuminuric in comparison to ~67% of the patients in Group B. Reduction in microalbuminuria could be achieved in ~72% of patients from Group A, whereas ~87% of patients could achieve the same in Group B.
