At present there is no cure for RA and the current treatment modalities utilized have limitations and side effects. Over the last decades, mesenchymal stem/stromal cells (MSCs), both autologous and allogenic, have emerged as potential safe and effective alternatives. In this review, we highlighted the safety and efficacy of both autologous as well as allogenic MSCs to treat RA based on recently published clinical studies. Pathophysiology involves micro tears in the tendon leading to haemorrhage, rough granulation tissue formation and later repair. Even though the different treatment modalities have been claimed to be effective in treating this condition due to its chronic nature and tendency to recur with resumption of activity, no single modality has been considered. Platelet-rich plasma (PRP) injection as a treatment that has been used in recent times for various enthesopathies. It repairs incompletely healed injuries thereby reducing pain and increasing function. Patient was not involved in any sport activity. There was no local foot deformity and limb malalignment. The patient was treated conservatively. There was complete clinical and radiographic healing of the medial malleolus eight months after the first presentation. Osteochondrosis of the accessory ossification center of medial malleolus is generally unilateral and bilateral involvement is very rare. There are various differential diagnosis for this pathological condition with similar clinical features and presentation in this age group. We ruled out the uncommon pathological conditions causing chronic pain in the medial malleolus during this period of skeletal growth. We consider this painful condition to be classified as a bilateral osteochondroses of the accessory ossification center of the medial malleolus. Osteochondrosis of the accessory ossification center of the bilateral medial malleolus is a rare entity. Due to rarity of disease and common presentation similar to other disease this is commonly misdiagnosed or missed in clinical practice. Osteochondrosis is characterized by a disturbance of endochondral ossification in skeletally immature patients and should be investigated in children having a history of persistent foot and ankle pain. The management of the chronic diseases took a relative back-seat for an active treatment protocol strategy. The clinicians emphasised on the conservative measures to manage arthritis and osteoporosis. The delayed assessment had a profound negative impact with significant progression of arthritis and osteoporosis amongst the elderly. The crippling effect of COVID-19 pandemic in the elderly presented furthermore with the advanced stages of arthritis and worsened osteoporosis for the management. The delay in timely offer of surgery for the arthritic joint compromised the functional outcomes. There has been a silent worsening with increased morbidity and mortality.The literature review and guidelines formulated emphasized on the timely remedial measures either surgical or conservative for the management of arthritis and osteoporosis in an elderly to halt the silent progression, allow a healthy lifestyle with an improved quality of life. Fibroblast growth factor receptor 3 mutation at p16.3 locus of chromosome 4 leads to inhibition of sub-chondral growth. Middle aged achondroplastic patient with left fracture neck of femur following a fall of heavy object. It was a challenging case because of coxa-vara and meticulous templating is needed. 38 year old male sustained fracture neck of left femur in severe coxa-vara. Closed reduction with 16 mm partially threaded cannulated cancellous screw perpendicular to native neck- shaft angle. At regular follow up, mild lysis was seen around the screw and implant removal was done. Now with neck of femur non-union, walking full weight bearing with walker. The future course included excision arthroplasty or custom made prosthesis. Achrondroplasia characterised by defects in en-chondral bone formation leading to retarded cartilage growth. Literature search on peri-articular fracture management showed peadiatric disatal femur fracture fixed with percutaneous screws. In adult, neck of femur fracture is managed by percutaneous screw, DHS or prosthesis. Due to altered geometry of proximal femur DHS and Prosthesis was deferred.Early failure of CC screws in our case is due to perpendicular placement of screws which increased strain due to altered bio-mechanics. Achondroplastic dwarfs live a normal life. They are prone to peri-articular fractures because of anatomical variations. The worldwide prevalence of rheumatoid arthritis is estimated to vary between 0.5 to 1.5% with geographic variation. The rationale behind relating vitamin D and RA is based on two facts. The first one is that there is evidence indicating that patients with RA have low levels of vitamin D. The second one is that the presence of vitamin D and VDR in macrophages, chondrocytes and synovial cells in the joints of these patients has also been demonstrated. The present study was designed to study the vitamin D status/ level in rheumatoid arthritis in males and females. The study was carried on 100 subjects chosen randomly with 75 females and 25 males in age group of 16 – 65 years. Their demographic data was collected. Vitamin D level, rheumatoid factor assay and ESR were analyzed. Higher level of disease activity as measured by DAS 28 score was associated with vitamin D deficiency. Females reported higher values for ESR, DAS 28 score and lower levels of vitamin D level as compared to males.Vitamin D deficiency was positively associated with increase in DAS 28 score and rheumatoid arthritis (RA). Female sex had more preponderance for RA development. It results from sustained stress of weight bearing - hopping, jumping, running -which results in micro trauma to plantar fascia which further leads to plantar fasciitis. It constitutes 11% to 15% of all foot symptoms. Its prevalence is 8% to 10% in general population. It commonly affects at the age of 40 to 60 years. Recently platelet rich plasma was used in treating in degeneration, muscle and tendon injuries. Hence, the present study aimed to assess the treatment outcome of autologous platelet rich plasma injection in treatment of plantar fasciitis. In this prospective study, we enrolled 35 patients with plantar fasciitis coming to OPD or casualty. Patients satisfying inclusion criteria were selected based on consecutive sampling. 11 patients responded well to conservative management and 3 patients had loss of follow up. The different scoring systems were adopted such as VAS and AOFAS for pain assessment. The thickness of plantar fascia was determined by ultrasound technique. Autologous platelet rich plasma was prepared and the same was injected. The outcome analysis was done at 2 weeks, 3 months, and 6 months; and compared with pre injection values. From pre-injection to up to post-6 months period, the VAS reduction was statistically significant (P<0.0001). All the time interval, the AOFAS was increased which was statistically significant (P<0.001). The injection was effective in reducing the thickness of plantar fascia, which was found to be statistically significant at all the time intervals (P<0.001).
