Lizbeth Ramos Gutiérrez","doi":"10.35366/110718","DOIUrl":"https://doi.org/10.35366/110718","url":null,"abstract":"Disruption of the pubis during labor is a rare lesion with a frequency of 0.1-0.8%, usually related to elderly primigravid patients, there is little evidence reported in young patients, but practically none in pediatric age. In our case, it is a 15-year-old female patient who is in full-term pregnancy with dystocic labor and with a skeletally immature pelvis, with disruption of the symphysis pubis that caused rotational instability of the pelvic ring, therefore which required surgical treatment. As there is little literature in this regard that unifies the management criteria for this type of pelvic injury that is a product of labor, it is impossible to standardize care protocols, therefore the best way to manage them is to adhere to what is established in patients with similar injuries. as a result of trauma of another nature. {"title":"Resultados clinicorradiológicos en pacientes operados con clavos de Kirschner en fracturas de antebrazo en el Instituto Nacional de Pediatría durante el periodo 2017-2019","authors":"Rubén Lechuga González","doi":"10.35366/107267","DOIUrl":"https://doi.org/10.35366/107267","url":null,"abstract":"Introduction: Trauma is the leading cause of visits to pediatric emergency rooms, with an estimated annual rate of fractures between 12- 36.1 per 1,000 children. Radius and ulna fractures are a frequent pathology treated in the emergency department of this unit. In our hospital, a total of 46 open or closed reductions with internal fixation were performed in 2017, in 2018, 42 cases and in 2019, 51 cases. Therefore, this work will be carried out in search of the results obtained in those patients who underwent surgery for diaphyseal forearm fractures. Evaluating the clinical radiological results of operated patients will allow to know if the results obtained in this unit are similar to those reported in the national and international literature. This will help us identify areas of opportunity to implement standardized management for forearm fracture patients. Objective: To determine the clinical-radiological results in forearm fractures operated with Kirschner nails at the Instituto Nacional de Pediatría during the period 2017-2019. Material and methods: Observational, retrospective and descriptive research, where the information unit will be the electronic medical record of patients who meet the following criteria: complete according to NOM 004, diagnosed with forearm fracture, operated on open reduction or closed and fixation with Kirschner nails at the Instituto Nacional de Pediatría during the period 2017-2019. Results: A total of 139 cases were identified that met the inclusion criteria, frequencies of the affected sex, side and fractured area were found similar to those reported in other series. Only one case presented complications, this being an exposed fracture with subsequent wound dehiscence. The healing time was increased in patients with exposed fractures and in those in whom open reduction was performed. Conclusions: I n the present study it was found that the clinical and radiological results of these patients are good in almost all cases, comparable to that reported in the national and international literature. As the number of younger and more active patients treated with TKA continues to increase, consideration of better fixation as a means of improving implant longevity is required. This technique, less used than cemented knee replacement in the last decades, sees a revival thanks an advance in prosthetic design, instrumentation and operative technique. The related literature in some cases shows conflicting data on survival and on the revision’s rate, but in most cases a success rate comparable to cemented TKA is reported. Noncemented components offer the potential advantage of a biologic interface between the bone and implants, which could demonstrate the greatest advantage in long-term durable fixation in the follow-up of young patients undergoing arthroplasty. The optimal fixation in TKA is a subject of debate with the majority of surgeons favouring cemented fixation.
