Critically ill patients who is receiving mechanical ventilation may have an increased risk of VAP due to multiple factors as sputum retentions and atelectasis. Thus, different combinations of chest physiotherapy assist in the re-expansion of the atelectatic lung, confer short-term improvement in total lung-thorax compliance and expiratory flow rates, and reduce the incidence of ventilator-associated pneumonia. Objective: To identify the effect of multimodality chest physiotherapy interventions on prevention of ventilator associated pneumonia among mechanically ventilated patients. Settings: The study was carried out at the general ICUs at Damanhur Medical National institute which is classified to general ICU I (15bed) and the General ICU II (13bed). Subjects: A convenient sample of 60 adult mechanically ventilated patients from the starting day of invasive mechanical ventilation were included in the study. Tools: Two tools were used to collect the data of this study. The first tool was outcomes of multimodality chest physiotherapy assessment that was used to assess the effect of multimodality chest physiotherapy interventions. The second tool was VAP bundle observation checklist that was used to assess nurses' compliance with ventilator bundle practices. Results: The findings of the current study revealed that there was a statistically significant difference between the intervention and control groups regarding the occurrence of VAP (P= 0.001). Patients who were subjected to multimodality chest physiotherapy interventions had lower VAP rate. Conclusion: The present study revealed that patients who had a twice-daily multimodality chest physiotherapy interventions in the form of manual hyperinflation, endotracheal suctioning, patient positioning plus chest percussion and mechanical chest vibration device had a better effect on decreasing VAP occurrence as well as enhance patient`s clinical outcome than those who do not had. Tracheal intubation with a cuffed tube is considered definitive airway management in adults. The tube cuff is designed to provide a seal against aspiration and to prevent leaks during positive pressure ventilation. A cuff pressure between 20 and 30 cmH 2 O has been proposed guideline published by the Royal College of Anaesthetists to prevent the complications of over/under inflation of the cuff. Objective: To compare the endotracheal tube cuff pressure measurements before and after nursing interventions among mechanically ventilated patients. Settings: The study was carried out in the general ICU at Damanhour Medical National Institute that consist of three main halls with fifteen beds; hall I (4 beds), hall II (4 beds), and hall III (8 beds). . Subjects: A convenient sample of 70 adult patients who were mechanically-ventilated via an endotracheal tube with high-volume low-pressure cuff and a diameter ranging from 7 – 8.5 mm were included in the study. Tools: one tool was developed by the researcher used for data collection namely “Endotracheal tube cuff pressure measurement record”. It was used to record the endotracheal tube cuff pressure measurements before and after the selected nursing interventions. It consists of three parts. Cuff pressure was assessed before nursing interventions and after nursing interventions. Results: The majority of the studied sample was male with age 51-60 years. Results of the study showed that there was a significant decrease in the mean values of endotracheal cuff pressure starting before till 30 minutes after the complete bed bath, oral hygiene, and right and left lateral positioning procedures. Conversely in the endotracheal suctioning, the mean value of ETT cuff pressure before showed an increase immediately after, followed by a decrease after 15 minutes, then an increase after 30 minutes. Conclusion: The study concluded that the cuff pressure measurements were significantly changed after performing the selected nursing interventions at different timing. One possible contributor to these difficulties may be deficit in emotion recognition with schizophrenic patients which are essential components of interpersonal communications competence. Emotion recognition is receptive emotional skills include the ability to accurately perceive emotion. Consequently, patients who have emotion recognition deficit often experience difficulty in interpersonal communications competence. Objective: Assess the level of interpersonal communication competence and the ability of emotional recognition among patients with schizophrenia. Settings: The study was conducted at EL-Maamoura Hospital for Psychiatric Medicine, in Alexandria, Egypt. Subjects: the data was collected from 270 outpatients with schizophrenia. Tools: three tools were used to collect the date; socio-demographic and clinical structured interview schedule, Interpersonal communication competence Scale (ICCS) and Bell-Lysaker Emotion Recognition Task (BLERT). Results: Findings of the present study revealed that 99.3% of the studied patients have low level of interpersonal communication competence and most of the studied patients have emotional recognition deficit in all emotion except happy emotion. Moreover, statistically positive significant correlations were found between overall interpersonal communication competence and overall emotional recognition among the studied subjects. (r=0.659, P<0.001). Conclusion : The present study concluded that, the studied subjects demonstrated low level of interpersonal communication competence, emotional recognition deficit and there was positive correlation between interpersonal communication competence and emotional recognition. Burger Allen Exercise and reflexology are considered the keystones for improving the lower extremity perfusion, and decreasing pain among diabetic patients. Objective: To determine the effect of Buerger Allen exercise versus Reflexology on lower extremity perfusion and pain among patients with type 2 Diabetes Mellitus. Settings: The study was carried out at Inpatient Medical Department of the National Medical Institute in Damanhur, El-Beheria governorate, Egypt. Subjects: A convenient sample of 60 adult diabetic patients. Tools: Four tools were used for data collection. Tool 1. Socio-Demographic and Clinical Data Sheet. Tool II. Ankle Brachial Index Scale (ABI) to assess the lower extremity perfusion. T ool III. Selected Clinical Features of Peripheral Vascular Disease Observational Checklist to assess the level of peripheral vascular disease (PVD). T ool IV . Pain Assessment Visual Analog Scale (VAS) to assess pain intensity. Results: The majority of the studied patients of the Buerger Allen exercise achieved \"normal\" level of Ankle Brechial Index (ABI) scale, “normal” level of PVD symptoms, and “mild” level of pain post intervention (73.3%, 90%, 66.7% respectively). 63.3% of the reflexology group patients achieved “normal” Ankle Brechial Index (ABI) scale, 100 % of them achieved “normal” level of PVD symptoms and 76.7 % achieved “no pain” post interventions. Statistical significant difference was found between the overall mean percent improvement of exercise and reflexology groups regarding level of pain (p<0.001*). Conclusion: Both Buerger Allen Exercise, and reflexology are effective in improving lower extremity perfusion among type 2 diabetes mellitus patients. Diabetic patients who performed reflexology intervention experience less pain more than those who performed Buerger Allen exercise. Objective: This study aimed to evaluate the effect of an educational intervention on self-care practices among patients with diabetic retinopathy. Setting: The study was conducted at Vitreoretinal Outpatient Clinic at Alexandria Main University Hospital, Alexandria, Egypt. Subjects: A convenience sample of 60 adult patients with DR. Tools: Three tools were used for data collection: Tool I: Patients' Bio-socio demographic Data Structured Interview Schedule. Tool II: Diabetic Retinopathy Patient's Knowledge Assessment Questionnaire to assess patients' knowledge related to DR. Tool III: Self-Care Practices Structured Interview Schedule to assess self-care practices undertaken by the patients. Results: The study group showed marked improvement post-self-care practices educational intervention application where all of the study group had good overall self-care practices than control group. Conclusion: The studied patients who received self-care practices educational intervention had significant improvement of knowledge and self-care practices than those who didn't receive it. One of the greatest challenges faced in the nursing profession is developing future nurse leader. Nurses must have leadership to move forward in harmony with changes in society and in health care. The leadership development program is a useful way for “unfreezing” views, ideas and opinions about leadership competence and, more importantly, for showing how the bedside nurses saw themselves as leaders; be empowered, have confidence in his/her abilities as a future leader and employ leadership practices. Objective: To determine the impact of implementing a leadership development training program for staff nurses on structural empowerment, leadership self-efficacy, and clinical leadership practices. Settings: This study was conducted in Inpatient Surgical Care Units at Alexandria Main University Hospital (N=15) Subjects: Nurses who are working in surgical units at morning and afternoon shift was selected randomly by simple random sample (N=40) were included in the study. Tools: Three tools were used in this study. The first tool was Conditions of Work Effectiveness Questionnaire-II (CWEQ-II) to measure structural empowerment. The second tool was Clinical Nurse Leadership Self-Efficacy Scale (CNLSE)) to assess the nurse’s confidence in performance of each of the 9 practice competencies of the clinical nurse leader. The third tool was Clinical Leadership Observation Scale to assess clinical leadership of staff nurses aligning with Kouzes and Posner (2007) model. In addition, demographic data sheet was developed by the researcher; and included questions related to gender, age, educational qualification, working unit, years of units and nursing experience Results: findings of the present study revealed that there was a highly significant improvement of overall structural empowerment and its dimensions, leadership self-efficacy and its dimensions as well as clinical leadership practices and its dimensions in the three phases of evaluation (before, immediately after and after 2 month of program implementation). The program also had moderate effect on structural empowerment and leadership self-efficacy and large effect on both clinical leadership practices and leadership knowledge. Conclusion: The study concluded that there was a highly positive significant correlation between leadership self-efficacy and structural empowerment and between clinical leadership practices and structural empowerment and leadership self-efficacy immediately after program implementation. Pregnant women’s relationship with maternity care providers and the maternity care system during pregnancy is vitally important. Care during this period needs to encompass basic human rights. Objective: To compare between nurses’ compliance with pregnant women’s bill of rights in an urban and rural areas in Alexandria. Settings: The study was carried out at El Shatby Maternity University Hospital and four family medicine centers in Abies, Alexandria. Subjects: A convenient sample of 120 nurses working for at least 2 years in antenatal units of the abovely mentioned settings. Tools: Three tools were used for data collection. The first tool was basic data Questionnaire to identify nurses' socio demographic, academic and professional data. The second tool was nurses’ knowledge about pregnant women’s bill of rights questionnaire. The third tool was nurses' compliance with pregnant women’s bill of rights, observational checklist. Results: Findings of the present study revealed that a statistically significant difference is observed among the two groups regarding the relationship between their total score of knowledge and level of compliance. Although, good knowledge among as much as 51.3% and 44.0% of urban and rural nurses, respectively, was associated with poor compliance level. Yet no good compliance was observed among the good knowledge rural ones unlike the urban ones. Where all the good compliant ones were good knowledge (p=0.041). Conclusion: The study concluded that nurse’s compliance with pregnant women’s bill of rights was unfortunately poor in both urban and rural settings. However, it has many draw backs. Objective: To identify factors hindering the effect of nebulization therapy among critically ill mechanically ventilated patients. Settings: four intensive care units at Alexandria Main University. Subjects: A convenience sample of 60 adult critically patients on invasive MV and, all critical care nurses about 50 nurses who were involved in providing direct patient care and working in the three different shifts. Tool: “Factors hindering the effect of nebulization therapy among mechanically ventilated patients’ assessment” was used to collect the needed data. Results: Findings of the current study revealed that there was a significant improvement in almost cardiovascular and respiratory parameters (p=<0.001* for all) after the nebulization therapy session with 4 hours, while Majority of the nurses had unsatisfactory level of performance regarding to nebulization therapy especially in the night shift with significant value for total score (p=0.006* for all). Conclusion: slower inspiratory flow, longer duty cycle, and large diameter tracheal tubes gives betters and effective nebulization. Disturbances in the perception of self are thought to be central to the development of psychosis. A growing body of research reflects that the incoherence or disorganization in sense of self in schizophrenia may mask individuals from perceiving reality accurately and perceive themselves as if they lost contact to themselves and spent a lot of time wondering about the kind of person they are. Objective: So the aim of this study was to assess the self- concept clarity, as well as positive and negative symptoms among patients with schizophrenia. And, to identify the relationship between self- concept clarity and both positive and negative symptoms among patients with schizophrenia. Settings: The study was conducted at EL-Maamoura Hospital for Psychiatric Medicine, in Alexandria, Egypt. Subjects: the data was collected from 200 in patients with schizophrenia . Tools: three tools were used to collect the date; socio-demographic and clinical structured interview schedule , The Brief Psychiatric Rating Scale (BPRS-version 4.0) and the Self-Concept Clarity Scale . Results: Findings of the present study revealed that around half of the studied sample demonstrated low self-concept clarity (50.5%). Whereas, 38.5% of the studied subjects had moderate self-concept clarity. Moreover, a statistically significant correlations were found between self-concept clarity in relation to positive and negative symptoms among the studied subjects. (r s = 0.242, P<0.001, and r s = 0.225, P=0.001, respectively). The overall brief psychiatric rating scale (BPRS) was proved to be the independent predictor of the self-concept clarity Conclusion : The present study concluded that the studied subjects demonstrated low self-concept, and positive and negative symptoms was positively correlated with self-concept clarity . Most of the studies evaluated the use of IPASS to improve handover of critically ill patients from one hospital area to another, but little was found to evaluate handover during nursing shift change in the general intensive care units (ICUs). Objective: Develop and evaluate the standardized IPASS inter-shift handover tool in the general ICUs. Settings: Five general ICUs at the Alexandria Main University Hospital. Subjects: Two samples were enrolled in this study. The first sample was 77 experts who participated in the development of IPASS handover information. The second sample was 100 bedside critical care nurses who used and clinically evaluated the IPASS handover. Tools: Two tools were used to collect the data of the study. Tool I was the IPASS Handover Information Questionnaire. Tool II was the Evaluation of the Current Handover Questionnaire. Results: Consensus on important and relevant IPASS handover information was achieved after the second e-Delphi round (all p < 0.001). 37% of the critical care nurses agreed that IPASS handover was clinically usable, and 53% strongly agreed on its clinical usability. Conclusion: The output of this study is an expert consensus and clinically usable IPASS handover information that is specific for nurses' inter-shift handover in the general ICUs.
