American society and government are increasingly asking that health professionals address the needs of patients and are placing more emphasis on the management of pain. However, they are also challenging us to document the effectiveness of our efforts and to provide care in a cost-effective manner. In 1979 the American Society of Anesthesiology developed a classification system of pain centers: the major comprehensive pain center, comprehensive pain center, syndrome-oriented pain center, and the modality-oriented pain center. Each type of pain center has different resources available to meet patient needs. Pain centers have been documented in some studies to provide effective management of chronic pain problems. This study used a multi-site audit of randomly selected charts of patients advanced from oral analgesics to either patient controlled analgesia (PCA) or continuous intravenous (IV) infusion of opioids. A chart audit tool was developed and tested to establish reliability and validity. One hundred and thirty-six (136) chart audits were completed across nine acute care settings in three states. Data is also provided from a questionnaire mailed to health care professionals regarding the use of pain technology (N = 442). Study findings illustrate inconsistent indications for “high tech” strategies and important clinical implications for interventions such as PCA. Previous literature related to nurses' attitudes regarding euthanasia and assisted suicide is reviewed in this discussion of the issues central to nursing. These issues include ethical dimensions of nursing care, nursing attitudes regarding suicide, and nurses' unique roles with patients and family caregivers. This article presents the argument employed in a constitutional challenge to Washington's statute. By prohibiting mentally competent, terminally ill patients from hastening death by self-administering drugs, the state intrudes into and controls a profoundly and uniquely personal decision, one that is properly reserved to the individual, to be made in consultation with his or her doctor. This argument rests on Casey and Cruzan concerning constitutional liberty interests, and addresses the issue of equal protection. The development of pharmaceutical services to support the care of patients with terminal disease is discussed. The evolution of the modem hospice movement is described as is the documentation of hospice pharmaceutical services. The interdisciplinary team that normally provides such care is described. Responsibilities of pharmacists in providing care to terminally ill patients are discussed and hospice pharmaceutical care is defined including a position description for a hospice pharmacist. Principles of drug therapy in the management or pain are provided and other symptoms commonly seen in terminal care are listed. Case studies in pharmaceutical care of pain in terminally ill patients are presented.
