The key rationale for intraoperative computed tomography (iCT) is that it facilitates the most comprehensive assessment of fracture reduction. Real-time visual feedback is now considered an essential component in hand, spine and general orthopaedic surgery. Frequently proposed as the new gold standard of care, iCT has been widely adopted in maxillofacial surgery, especially internationally. In contrast, our Australian experience is that access to iCT and therefore uptake is relatively lacking. The craniofacial skeleton is a complex structure owing to its various convexities and concavities, sinonasal elements, biomechanical buttresses, visceral and occlusal components and neurovascular structures. Classic approaches to open reduction internal fixation aim to reduce incisions to prevent complications such as ectropion, salivary leak or nerve damage. However, intraoperative assessment of fractures in minimally-invasive techniques are hindered by the limited extent of exposure, especially in the setting of complex fracture patterns or those that involve dynamic or functional endpoints. With iCT, surgeons may visualise, adjust and revise fixation without wide exposure while the patient is on the operating table. This obviates an additional operation that may be required due to an unsatisfactory fixation. iCT is cost-effective and reduces radiation exposure. Challenges in integrating iCT include competing for availability with other specialties, training staff and lack of awareness about the benefits this technology offers. Despite demonstrated efficacy in most specialties, there is scarce validation of current systems in plastic and reconstructive surgery, let alone a system unique to the specialty. This review aims to establish the efficacy of current systems in plastic and reconstructive surgery and formulate a classification unique to our specialty. Methods: A PUBMED literature search was performed in January 2023 encompassing all available literature. Search terms included ‘classification’, ‘grading’, ‘postoperative complications’ and ‘adverse events’. Further additions through bibliographic linkage generated a total of 363 articles. Results: Twenty-four papers were related to PRS. Sixteen used current systems to describe postoperative complications, whereas eight papers used a system designed uniquely for PRS. The downfall of conventional systems is the simplicity of return to the operating theatre, constituting a single grade in the Clavien-Dindo Classification. Return to theatre has a broad scope for heterogeneity in plastic and reconstructive surgery, particularly in reconstructive patients. A modification of the Clavien-Dindo Classification is proposed. Conclusion: There is limited efficacy of current classification systems for postoperative complications in plastic and reconstructive surgery nor a well-validated system unique to the specialty. A modified system with a detailed emphasis on return to the theatre will be of value to plastic and reconstructive surgery and ideally correlate with important outcomes. {"title":"How to do the boutonniere Elastoplast splint technique (BEST) for the management of mallet deformities","authors":"F. I have used the boutonniere (button-hole) Elastoplast (adhesive dressing) splint technique (BEST) throughout my hand surgical career for simple and complicated injuries. With patients with fractures over 30 per cent of the articular surface of the distal interphalangeal joint (DIPJ) this technique avoids open intervention and open reduction internal fixation (ORIF). Unsuccessful conservative management where the treatment sometimes extends over eight months also can be successfully handled by this technique.\u0000The accompanying audiovisual presentation is the basis of this teaching tool. Slides are supplemented by voiceover descriptions explaining the intricacies of this three-layered Elastoplast technique creating a boutonniere reduction position which needs to be reapplied with fresh Elastoplast weekly over a three week period. This produces an excellent outcome. The use of an Apple iPhone goniometer application (smartphone app) may provide a practical alternative for clinicians. This study sought to assess accuracy in range-of-motion measurements obtained by a smartphone app compared to a traditional universal goniometer (UG) in healthy and diseased hands.\u0000MethodsPatients with various hand disease processes were recruited through a tertiary hospital hand clinic, as well as healthy volunteers. Middle and little fingers were assessed at the metacarpophalangeal (MCP), proximal interphalangeal (PIP) and distal interphalangeal (DIP) joints for flexion and extension. Measurements were taken using a UG and a smartphone app by two independent evaluators using a standardised proforma.\u0000ResultsTwenty patients were recruited (50% male, 50% female; 13 healthy volunteers and seven with hand pathology). There were no significant differences in measurements using the smartphone app. The only significant difference found using the UG was for little finger PIP joint flexion; however, the difference was only 2.5°. Comparison of the average measurements using a UG and smartphone app found significant differences for six joints, with none of these > 5° (largest 4.6°).\u0000ConclusionThe smartphone app is as reliable as traditional UG and would be a practical tool for everyday use in hand clinics.
