The medical record folder at the Glugur Darat Health Center in Medan is currently in the form of a landscape and uses pink cardboard which is easy to tear and does not last long. The purpose of this study was to analyze and design a medical record folder in terms of three aspects, namely the physical aspect of the form, the anatomical aspect of the form, and the aspect of form content at the Glugur Darat Health Center in Medan. This research method uses descriptive, qualitative methods with saturated sampling technique, which was carried out at the Glugur Darat Health Center Medan in July-September 2022. The subjects in this study were Medical Record Officers, Filing Officers and Registration Officers at the Glugur Darat Health Center Medan. The result of this study is the design of a medical record folder at the Glugur Darat Health Center in Medan which is viewed from three aspects, namely the physical aspect of the form, a medical record folder made of thin cardboard is easy to tear and not durable. In the anatomical aspect, the form was found to only contain the name of the Family Folder, the name of the Puskesmas and the logo of the Puskesmas in the Heading section. Long-term use of paper-based or conventional medical records might result in issues including misfiles, disorganized shelves, and limited storage space. As a result, it's important to implement a procedure for the retention, elimination, and destruction of medical record files in order to eliminate useless files, decrease the growth in the number of files, and maintain the standard of medical record services. This study aims to evaluate the 5 M (Man, Money, Method, Material, and Machine) components of the process of eliminating and destroying medical record files at RSUD Muntilan. The object of research is medical record files, storage facilities, storage, implementation, and destruction of medical files. The method used in this study is a qualitative method through data collection by observation and interviews with medical record officers. The research was carried out at the Muntilan Hospital from June to July 2022. A good tracer should be made of strong and colorful materials. The function of the tracer is to show where medical record documents are when they are not in storage. Tracers can also improve efficiency and accuracy by showing where medical records are stored when they return. The purpose of this study was to redesign the tracer at Imelda Workers' Hospital in Indonesia by replacing materials, shapes, and designs to make it easier for officers to carry out their duties, namely inserting tracers into the shelves between the densely packed medical record files. The research method used is descriptive qualitative research. And the research instruments used are interview guidelines and observation check lists. The tracer (outguide) at Imelda Workers' Hospital in Indonesia has not been used for about a year because all tracers (outguides) have been damaged. The tracer (outguide) was damaged because the material was too stiff and broke easily when the officer inserted the tracer into the storage rack between the dense medical record files. RSU Imelda Workers Indonesia requires a tracer (outguide) made of strong, durable, not easily damaged, torn, broken and researchers have redesigned the tracer (outguide) as the results in the design above, namely by using flexible mica plastic material with a thickness of 0.70 mm , and the color chosen by the researcher in this design is black. The aim of this research is to determine the overview of the storage system with the length of time for retrieval of medical record files at RSIA Artha Mahinrus Medan. This research is descriptive qualitative to see by describing the results as they are and comparing them with the theory which is then drawn conclusions. The time of the study was carried out in May at the Artha Mahinrus Mother and Child Hospital, Medan. The population and sample used are medical record employees in the registration and storage division (filling). The instruments used in data collection were observation and interviews. Based on the results of this study, it shows that the storage system at the Artha Mahinrus Mother and Child Hospital Medan is a centralized storage system, by combining medical record files in one room. The time of taking medical record files is very influential on education, age, years of service and storage shelves. The education of medical record file storage officers is not all D3 medical record graduates. Hasil studi pendahuluan pada ruang filing, pintu penyimpanan tidak dikunci karena merupakan pintu akses menuju ruang rekam medis, ditemukan rekam medis disimpan pada kardus, penyimpanan berkas tersimpan dengan kurang rapi membuat rekam medis rusak dan terlipat, dan tidak tersedia tracer. Tujuan dari penelitian ini untuk mengetahui penerapan aspek keamanan dan kerahasiaan rekam medis sesuai standar MIRM 11 dan 14. Jenis penelitian ini adalah kualitatif dengan pendekatan studi kasus. Subjek penelitian berjumlah 4 informan dengan metode pengumpulan data menggunakan wawancara mendalam, observasi, dan studi dokumentasi. Analisis data menggunakan analisis tematik. Hasil penelitian tersedia SPO dan SK untuk pencegahan penggunaan rekam medis tanpa izin. Perlindungan dari kehilangan melakukan pencatatan pada buku register. Perlindungan dari kerusakan adalah mengganti cover, fasilitas yang cukup, serta terdapat petugas K3. Perlindungan dari gangguan akses adalah dengan petugas selalu berjaga pada ruang penyimpanan. Perlindungan pada ruang penyimpanan terhadap akses yang tidak berhak adalah pintu selalu di kunci. Tersedia SPO mengenai kerahasiaan dan privasi informasi. Regulasi dilaksanakan pada saat pelepasan informasi dan pada saat mengakses berkas. If the implementation of depreciation is delayed, there will be a buildup of medical record documents that fill the medical record document storage rack. The accumulation of medical record documents makes storage shelves untidy and prone to misplacement of medical record documents (missfile). In Permenkes No. 269/MENKES/PERIII/2008 concerning medical records, medical records are files containing notes and documents regarding patient identity, examination, treatment, actions and other services provided to patients. The purpose of this study was to determine the implementation of shrinking medical record files at Madani Hospital Medan. This type of research is descriptive qualitative. This method is used to describe the cause of the non-implementation of shrinkage of medical record files at RSU Madani Medan. Collecting data in this study conducted interviews with medical record officers. Based on the results of research at Madani General Hospital in Medan, there were 6 medical record officers and only 4 people with RMIK D-III educational background. The implementation of sorting medical record files at RSU Madani Medan is not in accordance with standard operating procedures because the sorting process is more than 5 years old. Hospitals should provide training for filing officers. For non-medical record officers, medical record education and health information to better understand the implementation of depreciation. Tenaga kesehatan khususnya PMIK harus memperhatikan prosedur penanganan Covid-19 dalam memberikan pelayanan kepada pasien. Prosedur pentatalaksanaan rekam medis perlu diajarkan kepada mahasiswa agar memperoleh pengetahuan dan pemahaman tentang pentatalaksanaan rekam medis di masa pandemi Covid-19. Diperlukan visualisasi pentatalaksanaan rekam medis di masa pandemi Covid-19 bagi mahasiswa dalam media video tutorial. Tujuan penelitian ini yaitu mengembangkan visualisasi dalam media video tutorial tentang pentatalaksanaan rekam medis di masa pandemi Covid-19 pada pembelajaran praktikum laboratorium. Penelitian menggunakan metode research and development yang dilaksanakan di laboratorium rekam medis manual Poltekkes Kemenkes Semarang pada bulan Agustus hingga Desember 2021. Tahapan dalam penelitian ini diawali dengan analisis kebutuhan melalui FGD, dilanjutkan dengan tahapan pembuatan video tutorial, uji validitas, dan diakhiri pada tahapan uji coba. Hasil penelitian diperoleh visualisasi dalam bentuk video tutorial yang telah dinyatakan sangat valid dalam uji materi sebesar 85% dan uji media sebesar 81% oleh pakar. Uji praktikalitas terhadap video yang dilakukan kepada mahasiswa diperoleh hasil praktis dengan nilai 78% sehingga mampu memberikan visualisasi kepada mahasiswa pentatalaksanaan rekam medis di masa pandemi Covid-19. Berdasarkan PERMENKES Nomor 31 Tahun (2019) tentang Sistem Informasi, Puskesmas harus menerapkan Sistem Informasi untuk membantu proses pengambilan keputusan dalam melaksanakan manajemen Puskesmas untuk mencapai sasaran kegiatannya.Untuk kegiatan registrasi & pelaporan di Puskesmas tumbuan sejauh ini masih dilakukan secara manual menggunakan buku register.Hal ini menyebabkan data yg dikumpulkan tidak akurat, rentan terjadi kehilangan data, duplikasi data serta dalam pelaporannya membutuhkan waktu yang lama. Untuk mengatasi masalah tersebut, akan dikembangkan sebuah sistem informasi elektronik supaya beban pekerjaan petugas akan berkurang dan tata cara pendataan pasien menjadi lebih baik, tidak memakan waktu untuk pencarian dan perekapan data, serta pelaporan bisa lebih cepat untuk dikerjakan dan disampaikan kepada pihak yang terkait. Adapun metode yang digunakan untuk membuat sistem ini menggunakan visual basic dan database MySql serta menggunakan metode pengembangan DFD (data flow diagram). Dalam penelitian ini menghasilkan prototype sistem informasi puskesmas yang meliputi menu login, menu input data petugas, registrasi pasien dan menu laporan. If the employee feel satisfied with their work, the employee will produce good performance, and vice versa. Based on the result of the preliminary study with the head of the medical record unit, it is known that they never get a reward if they work well, In addition there are still many officer who come late and performance of officers is still not paid attention. This condition resulted in work performed by medical recorders at Nur Hidayah Bantul hospital is not optimal, so it will affect work motivation. \u0000Objective: To determine the relationship between work motivation and the performance of medical recorder in the Nur Hidayah Bantul Hospital \u0000Method: This is a quantitative research with cross sectional design. The data collection technique used total sampling with a total respondent of 16 medical recorder. \u0000Results: From the results of the study, the results of work motivation showed that most medical recorder had sufficient work motivation (63%) and the performance of medical recorder mostly had sufficient performance (81%).
