Hoarseness or dysphasia is the commonest presenting symptom and usually it presents as a submucosal mass on laryngoscopy. A total of 90% of these tumors occur in the supraglottic larynx and the rest in the glottis and the subglottic region. Functional activity is seen in a few (2.9%), none are associated with paraneoplastic syndromes. Setting: Department of Head and Neck Oncosurgery, Kidwai Memorial Institute of Oncology, Bengaluru. Case report: A 35-year-old male presented to us with hoarseness of voice since 4 months duration. Contrast arteriography demonstrated that the left superior thyroid artery supplied greater than 80% of the blood supply to the laryngeal mass. Superselective embolization was done from the right femoral under local anesthesia and sedation which was uneventful. Intervention: The tumor was excised from lateral pharyngotomy approach with an elective tracheostomy. Microscopy suggested it to be paraganglioma and immunohistochemistry confirmed it. Conclusion: Complete surgical resection or partial laryngectomy with meticulous dissection of surrounding tissues and preservation of neurovascular structures give an excellent prognosis as far as oncological clearance is concerned. Malignant paragangliomas of the larynx are rare and a major meta-analysis is necessary to provide a true biological behavior of this tumor.","PeriodicalId":258448,"journal":{"name":"International journal of phonosurgery and laryngology","volume":"7 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2012-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126272572","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is the second most common primary hematologic tumor of the larynx after plasmacytoma. Although squamous cell carcinoma amounts to 90% of the laryngeal tumors, diagnosis of laryngeal lymphoma is very important as nonsurgical management is indicated in all stages of this disease. Case report: A case of 10-year-old boy with hoarseness of voice with direct laryngoscopic biopsy reported as diffuse B-cell lymphoma. The sections were positive for epithelial membrane antigen (EMA), LCA, CD138, CD4 and Mum1 and ALK. He was managed under MCP 842 protocol. He had complete response after the treatment and was symptom free after 13 months of therapy. Conclusion: Primary laryngeal lymphoma is a rare entity and should be managed with current chemotherapeutic protocol. Differentiating it between squamous cell carcinoma is important as management protocol in lymphoma is nonsurgical. Aid of immunohistochemistry helps in early diagnosis and management of these cases.","PeriodicalId":258448,"journal":{"name":"International journal of phonosurgery and laryngology","volume":"27 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2012-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"131862032","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Burning introduced injection technique for unilateral vocal fold paralysis in 1911 using intracordal paraffin by means of a novel injecting device that he designed. It was not until the 1950s to 1960s, however, when Arnold championed injection medialization that it become the mainstay of phonosurgical treatment for paralytic dysphonia. Medialization of the vocal fold edge by means of neural reinnervation (phrenic or descending hypoglossal to recurrent laryngeal nerve) was introduced by Balance in 1924 and advanced in recent years by Tucker and Crumley. Clinical presentations of laryngopharyngeal reflux disorder may vary from very subtle to mammoth proportions, ranging across a panorama of features like chronic laryngitis, indolent laryngeal ulcers or granulomas, onto life-threatening glottic/sub-glottic stenosis and laryngeal malignancies very rarely. With the advent of videostroboscopy, laryngologists have now acquired a ‘cutting-edge’ sensitive tool to look into the intricacies of the larynx and thereby study the pathological effects of laryngopharyngeal reflux in great detail. Objective: To assess the efficacy of videostroboscopy with voice analysis, as a diagnostic and prognostic tool in a cohort of patients with laryngopharyngeal reflux disorder. Study method: This prospective study was conducted over a two-year period, among 50 patients with hoarseness of voice and features of reflux laryngitis, who underwent videostroboscopy with voice analysis at initial diagnosis and at routine follow-up, to objectively study the response and outcome of anti-reflux therapy on the larynx. Observation and result: Videostroboscopy provided a vivid picture of the changes in larynx due to chronic acid reflux and proved efficacious in precisely judging the treatment responses. Conclusion: Our study reflects the recent concept highlighted in world literature that videostroboscopy is an ideal, sensitive tool for early diagnosis and in prognosis of acid induced laryngeal disorder.","PeriodicalId":258448,"journal":{"name":"International journal of phonosurgery and laryngology","volume":"25 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126833794","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Their evaluation necessitates the use of questionnaires to know both extent of symptoms and effect on quality of life. These should be designed to get maximum information with minimum trouble to the patient. Parkinson’s disease (PD) involves swallowing disorder as a symptom, often unreported. Hence, this needs to be assessed separately. Aim: To develop a questionnaire, in the vernacular, to check for the presence of swallowing problems in PD patients and to study its reliability and validity. Materials and methods: We developed an 11-item questionnaire in English, modeled on the Tel Aviv swallowing disturbance questionnaire (SDQ), chosen for its brevity. This was translated into the vernacular (Malayalam, native tongue of the state of Kerala in India) and administered to patients (n = 106) attending the weekly PD clinic of a tertiary care center, in consultation with the attending n in charge of movement disorder patients. The same questionnaire was also administered to age-matched controls (patients without any swallowing disorders) (n = 63) attending the ear, nose, and throat (ENT); neurology; and geriatric clinics. The scores were documented and statistical analysis done, to check for internal reliability and validity. Results: Comparison of mean total scores between cases (2.89) and controls (1.51) showed a statistically significant difference, indicating statistical validity (p = 0.009). Guttman’s split-half correlation showed fairly good reliability for both cases (0.812) and controls (0.851). Conclusion: The present questionnaire to assess dysphagia in PD patients has been found to be reliable and valid for use. We recommend it as a screening tool in the neurology outpatient department (OPD).
