Ryu, Doo Hyung Yoon, Sam-Guk Park","doi":"10.4055/JKOA.2018.53.3.277","DOIUrl":"https://doi.org/10.4055/JKOA.2018.53.3.277","url":null,"abstract":"골다공증은 고령 인구에서 매우 흔한 근골격계 질환으로 골밀도 가 감소되고 이로 인해 골절도 발생할 수 있는 것이 특징이다. 골 다공증 치료제로 여러 가지 약제들이 사용되고 있고, 그 중에서 비스포스포네이트(bisphosphonate)는 파골세포의 활성 감소 또는 세포자멸사를 초래하여 골 흡수를 억제시키는 약제로 세계적으 로 가장 많이 사용되는 치료제이다. 비스포스포네이트의 안정성 에 대한 연구들이 많지만 장기간 사용으로 인해 비전형적 골절이 발생할 수 있다는 보고들도 많았으며 이는 주로 대퇴골의 전자하 부위에 나타났다. 그러나 척골에 발생한 비전형적 골절은 국내 에서는 물론 해외에서도 드물게 보고되었고, 국내 학술지에 보고 된 경우는 없었다. 지금까지 보고된 증례에서는 모두 7년 이상의 비스포스포네이 트 복용력을 보이는 70세 이상의 여자 환자들에서 특별한 외상력 없이 척골에 비전형적인 골절이 나타났다. 1예를 제외하고 모든 증례에서 척골에 비전형적인 골절이 진단되기 이전에 대퇴골 또 는 척추에 골절 병력이 있었다. 저자들은 3년간 비스포스포네이 트(alendronate, 70 mg/wk)를 복용 후, 다른 부위에 골절 병력이 없 이 발생한 좌측 척골의 비전형적인 골절 1예를 경험하여 문헌고 찰과 함께 보고하고자 한다.","PeriodicalId":258160,"journal":{"name":"The Journal of The Korean Orthopaedic Association","volume":"12 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2018-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121098036","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
