Participant: 50-year-old male with severe keratoconus. Methods and Procedure: The patient underwent the BLAST procedure described in this article to treat his condition, with a follow-up at 1 week, 9 months, and 1-year post-op. Results: Measurements include Keratometry Maximal (K Max), Best Corrected Visual Acuity (BCVA), as well as Uncorrected Visual Acuity (UCVA) and pachymetry using the Pentacam at each follow-up. The BCVA showed drastic improvements, from 20/200 with pinhole pre-op to 20/50 post-op with a scleral contact lens. The thinnest pachymetry values also demonstrated a significant increase, going from 410 mm pre-op to 633 mm post-op. Discussion and Conclusion: The BLAST intervention provides a new treatment option that shows marked improvements in all parameters measured in follow-up, with a significant increase in the patient's visual acuity. Surgical manipulation can increase the risk of inflammation, scarring, and disease progression. The objective of this study was to understand the characteristics of OCP patients who received entropion repairs and determine the difference in entropion recurrence or revision in patients with OCP or other cicatricial entropion repair. Methods: A retrospective chart review was performed from January 2010 to June 2020. OCP patients from an autoimmune blistering disease clinic with entropion repair were compared to patients with repair of cicatricial entropion of other etiology. A generalized estimating equation assessed for evidence of recurrence of entropion or required revision. Results: 19 OCP patients with cicatricial entropion and 60 patients with cicatricial entropion secondary to other etiologies received 38 and 75 entropion repairs, respectively. No difference in prevalence of revision or recurrence was found between groups. Magnetic resonance imaging of the orbits demonstrated a well-circumscribed, ring enhancing mass of the right anterior supero-medial orbit. Medial orbitotomy and removal of the orbital mass was performed, and histopathology revealed lymphoid proliferation surrounding a microbial infiltrate staining for gram-positive filamentous bacteria with sulfur granules. Overall, the findings were consistent with chronic canaliculitis secondary to Actinomyces species complicated by formation of an orbital pyogenic granuloma. She has had no recurrence of disease at five months of follow-up.
