Hegazy","doi":"10.5742/MEIM.2015.92644","DOIUrl":"https://doi.org/10.5742/MEIM.2015.92644","url":null,"abstract":"Objective: This study was designed to detect 25-hydroxy vitamin D serum levels and bone mineral density (BMD) status in breast cancer patients, and to determine their relation to treatment and disease stages. Patients and methods: The study included 74 female patients with breast cancer and 52 healthy volunteers as the control group. Serum levels of 25-hydroxy vitamin D, calcium, phosphorus, and alkaline phosphatase were measured using ELISA kits, while dual energy x-ray absorptiometry (DXA) was performed to assess the BMD. Twelve patients received chemotherapy only; 12 received chemotherapy and hormonal therapy, 22 received chemotherapy and radiotherapy while 28 received chemotherapy, hormonal therapy and radiotherapy. Results: Serum levels of phosphorous and 25-hydroxy vitamin D were significantly lower (p =0.0001), and alkaline phosphatase was significantly increased (p =0.0001) in patients compared to the control. Hip, spine, and forearm DXA were significantly lower in patients than in controls (p =0.0001). The worst bone status was in those receiving both chemotherapy and hormonal therapy. The grade of tumor significantly correlated with the serum phosphorus level (p =0.048) and negatively with the serum 25hydroxyl vitamin D level (p =0.03) as well as with the DXA of hip (p =0.01) and spine (p =0.0001). Conclusion: Our study supports findings of increased incidence of hypovitaminosis D, osteoporosis and osteopenia in breast cancer patients. Hence, we throw light on the importance of offering calcium and vitamin D supplements to breast cancer patients. {"title":"Intra Cervical Foley Catheter as a Method for Induction of Labour : King Hussein Medical Centre Experience","authors":"Fatima Edwan","doi":"10.5742/MEIM.2015.92603","DOIUrl":"https://doi.org/10.5742/MEIM.2015.92603","url":null,"abstract":"Aim of study: To assess the efficacy of using intra cervical Foley catheter induction of labour among pregnant women with and without previous caesarean section and their neonatal out come at King Hussein Medical Centre, Amman, Jordan. Material and methods: The study was conducted in the labor department of King Hussein Medical Center (KHMC) Amman, Jordan during the period from August 2013 to January 2014 using a data sheet including patient questionnaire to review demographic characteristics of patients, indication for induction of labor and results of inductions regarding outcome, success rate and neonatal outcome. During the study period (August 2013 to January 2014) a total of 200 patients were admitted for induction of labor and their data were reviewed. Results: The most prevalent age group of induced patients was (25 34 years) which accounts for (54.5%) followed by (16-24) which accounts for (30.5%); 15% only were between the age of 35 and 45. Booking status : A little more than half of the patients were booked (only 58.5%); more than half of our patients were either para one (39.5% of the cases) or primigravida (24.5%). Only 35 of the cases had previous one caesarian rate (17.5) and (82.5%) had previous normal deliveries. Most of the studied cases were below 41 weeks of gestational age where (53.0%) of the cases were between gestational age 40-41 week while post term were (10%) of cases and only (1.5%) were less than 34 weeks. Intra cervical Foley’s catheter was the most common used method for induction of labour, where more than half of our patients used this method (54.5%) followed by prostaglandin(23.0%). Artificial rupture of membranes followed by oxytocin or seeping of membranes was the least used method with each method accounting for around 10 %. The most common indication for induction of labour was post date (47.5%) followed by others (40.5%) which include (non reactive NST, isolated decreased fetal movement, oligohydromimious, unstable lie, Intra uterine fetal death IUFD, congenital abnormality of the fetus and bad obstetric history; maternal diseases represent (8.5%) of the cases. Pre eclampsia (PET) cases represented the majority of cases (70.5%) followed by Diabetes Mellitus (17.5%); only 2 cases were essential hypertension (12.0%). More than half of the cases of induction ended with vaginal deliveries (58%); instrumental deliveries were used in 5.5% of cases and in 72 cases the induction ended by caesarean section which represents (36%) of cases. Mode of delivery in patients with previous CS: 65.5% of those cases ended up with a second CS. Vaginal delivery was achieved in more than 34% of the cases : instrumental deliveries in (6%) and normal vaginal deliveries in (28.5%). Failure to progress represented the most common indication for C/S in pregnant women with previous C/S who underwent induction of labour (60%) followed by fetal distress (37%), while obstructed labour was reported in only one case. Methods: We studied 100 NHL patients (divided into 2 groups depending on the presence of the double contour (DC) sign detected by PDUS) and 100 controls in a cross sectional study. Demographic, clinical and serological data were evaluated. PDUS was done to all patients and controls. Results: There was a statistically significant difference between the two groups regarding the presence of subclinical hyperuricemia in group (1)(p=0.008) who had higher s. creatinine and gouty nephropathy (p=0.002 and p=0.001 respectively). We tried to understand whether or not there is an association between severity of pain and complication rate in hospitalized children and adolescents with SCD in the present study. Methods: All hospitalized SCD patients with painful crisis between September 2012 and September 2013 were included into the study. The intensity of pain was assessed at the first visit. Pain scores were obtained using the Faces Pain scale and Verbal Descriptor Scale. Severity of pain was divided into three groups as mild, moderate, and severe according to the scales. Results: Seventy-nine patients under the age of 18 years-old with SCD and 146 episodes of painful crisis were evaluated. Forty-five (57%) patients were women and mean age was 11.5 years. The white blood cell counts, aspartate aminotransferase and C-reactive protein (CRP) were significantly higher while erythrocytes, hemoglobin, hematocrit and albumin levels were significantly lower in the severe pain episodes group (p<0.05 for all). The number of patients transfused was significantly high in the severe pain episodes group than the other two groups (p=0.006, p=0.001). Most of severe pain episodes group had complicated vaso-occlusive crisis (acute chest syndrome 41.6 %, Hepatic sequestration crisis 6.7%), (p<0.05). Conclusion: There may be an direct relationship between prevalence of complicated vaso-occlusive crisis and pain intensity of SCD. It has been described as an important etiological agent in uterus and during the intrapartum period in pregnant women. Objectives: Estimate the prevalence of HSV-2 antibodies among pregnant women in Kirkuk city. Patients and Methods: A cross sectional study (M.Sc. Thesis) was conducted in Kirkuk city and included 176 pregnant women, and 134 non-pregnant married women (control group) who attended at Azadi Teaching Hospital and Al Ta’akhi Health Care Center from the 20th of November 2012 to the 23rd of April 2013. Results: The study revealed that the 62.48 % of pregnant women were infected with HSV-2. The highest rate of IgM antibodies was found in 50% of pregnant women aged 18-23; this was also true for both IgM and IgG antibodies together that were found in 41.17% of women. The relation of seropositive HSV-2 antibodies with the total white blood cells (W.B.Cs) count showed a non-significant result with the probability (P) value >0.05. This was also true for the relation with absolute lymphocyte count (ALC), while its relation with absolute eosinophil count (AEC) showed a significant result, P <0.05. In regards to the relation of HSV-2 antibodies with serum interleukin-2 (IL-2), the result was non-significant. The relation with abortion number was significant. There was significant relation of abortion with gestational time of pregnancy in seropositive pregnant women. Conclusion: The seroprevalence of HSV-2 was relatively high in pregnant women in Kirkuk city. Primary and re-infection of latency occurred at the highest rate in age group 18-23 years old. Primary HSV-2 infection increases the AEC and IL-2 during pregnancy.
