Das","doi":"10.3329/BJPSY.V29I2.37848","DOIUrl":"https://doi.org/10.3329/BJPSY.V29I2.37848","url":null,"abstract":"Hysterectomy is one of the most commonly performed gynecological surgical procedures on the uterus which are thought to be an insult to the women’s emotional equilibrium. It leads to the feeling of reduced femininity which may develop psychiatric disorders. The objective of the study was to evaluate the proportion and pattern of psychiatric disorders among the hysterectomized women. This was a cross sectional and comparative study, conducted in the department of Psychiatry in collaboration with the outpatient department of Gynecology and Obstetrics, Sylhet MAG Osmani Medical College Hospital, Sylhet, Bangladesh during the period from January 2012 to December 2012. For this purpose 365 married women of reproductive age group, after 3 to 6 months of planned hysterectomy fulfilling the inclusion and exclusion criteria were selected consecutively as a case group and age matched 365 healthy women were selected as control group. The result showed that, the mean age of the respondents was 41.48 (SD±3.79) years which was almost identical to control group (p=0.953). Most (49.5%) of the women were below the age of 40 years. Psychiatric disorders were most frequent in hysterectomized group (38.3%) as compared to control group (28.1%) (p<0.001). His urine test for cannabinoid was positive. He was diagnosed as substance use disorder and was treated with olanzapine along with psychotherapy. After few days, he developed fever followed by erythematous rash with pruritus and swelling of the face, eyelids, trunk and extremities with raised serum bilirubin, ALT and alkaline phosphatase. He was diagnosed as DRESS (Drug Reaction with Eosinophilia and Systemic Symptoms), a rare complication of olanzapine. This is a dangerous and life threatening adverse effect. The objective of the study was to investigate sociodemographic and other variables attributed to the disease. The first outbreak of this illness took place in Adiabad Islamia High School and College, Raipura, Narsinghdi. We rationally decided to conduct our research in this school in case-control design. Within 12 months’ study period, 125 students of class VI to X (45 cases and 80 controls) were interviewed face-to-face by structured questionnaire containing separate questionnaire for students and guardians. The result of the study showed that, all the cases came from lower socioeconomic class, majority (71.1%) from 14 to 16 years age group and most (77.8%) of the cases were female. Outbreak rapidly spread among the cases mostly by smelling foul odor (40%) and seeing index cases (33.3%). Majority (51.2%) of the cases and controls thought that the illness was due to both physical and psychological causes though most of them (93.6%) did not hear about the disease previously. It was evident that the role of socio-demographic variables for attributing the symptoms pattern of the disease was ambiguous.
