Hydatid cyst is one of these lesions, which mimics many other lung diseases. One of the other, which causes diagnostic confusion, is tuberculomas. Without histological verification, it is difficult to differentiate these lesions from lung cancer, especially in high-risk patients. Therefore, surgical excision would be needed in most cases. Elucidation of proper family history and screening of family members can help in clinching the diagnosis. An 8-year-old boy presented with episodic painless hematuria for 3 years, with a history of small renal stones in parents, which was managed conservatively. Ultrasound showed concretions in the renal pelvis, and urinalysis revealed hypercalciuria in the absence of renal tubular acidosis in index case as well as parents. {"title":"An Unusual Case of an Extensive Proximal Tibia Bone Defect Secondary to Complicated Cellulitis Managed by Unifocal Bone Transport Using an Ilizarov Fixator","authors":"V. Extensive bone defects are usually seen in the former. It is unusual to have large defects in isolated osteomyelitis without prior history of any trauma. We are reporting a case with a bone defect of around 17 cm due to infectious aetiology and its management by unifocal bone transport using the Ilizarov ring fixator. Case Report The present report describes a 45-year-old gentleman with newly diagnosed diabetes mellitus who was initially admitted under general surgery for management of cellulitis and was later admitted under orthopedics for suspected osteomyelitis of left proximal tibia and septic arthritis of left knee. Serial debridement, prolonged antibiotic therapy and skeletal stabilization by a temporary joint spanning external fixator was applied to control the infection. The infection finally resolved after 6 weeks with a bone defect of approximately 17 cm. Options for reconstruction were discussed with the patient along with their pros and cons. Unifocal bone transport by ilizarov ring fixator was finalized with the patient’s consent with the aim of achieving a functional limb. Bone healing index was 1.17 months/cm. ASAMI scoring showed excellent bone result and good functional result. LEFS was 55/80. The lower scores were because of the fused knee. Degrees of electrical injuries range from minor burns to cardiac arrest. Electrocution is cardiac arrest resulting from an electric shock. In Ghana, many electrocution cases are declared dead with little or no resuscitative measures. With the establishment of the emergency department (ED) at Komfo Anokye Teaching Hospital (KATH), such cases within the catchment area are being managed. We sought to describe the management of three (3) cases of electrocution admitted which were resuscitated at the KATH ED. Case Series We present three retrospective cases of electrocution involving two adults and one child presenting to the ED of KATH. None of them had any form of cardiopulmonary resuscitation (CPR) at the scene, or en route to the hospital, however, all cases received resuscitative measures of CPR, defibrillation, intubation and other supportive management, and were successfully discharged home in a few days with no major complications. Discussion Electrical injuries do occur; however, continuous education and caution should be taken especially whilst using electricity and electrical appliances. Workers with high exposure to electricity should emphasize maximum safety precautions and use of appropriate protective equipment. Home appliances should be well hidden and insulated to protect children.
