It is expressed in both a sporadic form and genetic form and typically presents with cutaneous manifestations described as skin blisters in sun exposed areas. Case A 54-year-old male presented complaining of bullous itchy lesions on his hands and upper extremities that were at different stages of healing. Lab results were consistent with porphyria including elevated serum total porphyrins. He was scheduled for phlebotomy every other week for six weeks, hydroxychloroquine, minimize any sun exposure and to completely stop smoking. Conclusion Widespread skin lesions associated with underlying liver disease is a characteristic presentation for PCT. Hepatitis C is an antecedent risk factor for PCT, but can now be treated with antiviral therapy with the expectation of attainment of a sustained virologic response. Improvements in arresting progressive liver disease in Hepatitis C patients may improve PCT symptoms, as well. This study looks at the Prevalence and Management Outcomes of malaria cases that presented by different age categories at a Private Secondary Hospital by the residents of Agbani Road, Enugu state. Methodology This is hospital based cross-sectional retrospective study of patients who presented with fever from Month of January 2017 to Month of December, 2020. Malaria Diagnosis was made by WHO recommended RDT test kits and Microscopy, Structured questionnaire was used to collect information on Long Lasting Insecticidal Nets (LLINs) use from all the patients who patronize the hospital. All malaria positive cases were treated according to Malaria Treatment Guidelines. Analysis of data was done by SPSS.Version22. Results Out of 678 febrile Patients, 65% (443/678) stronglytested positive to malaria: P-value = 0.00135. 50% (221/443) malaria cases, 68% (70/103) admitted cases, 78% (31/40) complicated cases and 91% (10/11) death respectively occurred moreamong < 10years and pregnant mothers. 89% (396/443) successful treatment rate was achieved (P=0.000) and 83% (39/47) of resistantmalaria responded to antibiotics, P-value0.0001).9% (40/443) of the treated cases and Only 1 % (7/73) of cases who use LLINs returnedwith malaria within 28days. Conclusion Malaria Prevalence and outcomesis worseamong < 10 years and pregnant mothers and some anti-malaria resistant cases responded to antibiotics.LLINs use reducesprevalence of malaria re-infection in this setting. CT scan showed a 5mm left ureter stone, mesenteric root mass as well as a left lung pleuralbased lesion. Metastatic Neuroendocrine Tumor (NET) was suspected, hence a Gallium-68 Dotatate PET was performed. It showed intense uptake in the mesenteric mass, as well as in the pleural nodule, and no intense uptake in the liver. The liver is the most common site for small bowel NET metastasis. It is unusual to have pleural metastasis without liver metastasis. A review of this patient’s past medical history revealed a motor vehicle accident 20 years ago which required an exploratory laparotomy and splenectomy. Gallium 68 Dotatate PET scan can detect splenosis because it has somatostatin receptors. In patients with an existing neoplasm, metastases should be ruled out before confirming the diagnosis of splenosis. Although usually well tolerated, an increasing number of serious complications related to rituximab have been noted with its wide usage. We report a 67-year-old man who developed biopsy-proven Allergic Interstitial Nephritis (AIN) after treatment with rituximab for his Primary Central Nervous System Lymphoma (PCNSL). Rituximab-induced AIN was confirmed by kidney biopsy, and his kidney function improved to his baseline with supportive care and four weeks of steroid treatment. While rare, AIN could be a possible adverse effect of rituximab. To our knowledge, this is the first case report of a biopsy-proven AIN from rituximab. The association of AIN and rituximab in our case necessitates a high index of suspicion to facilitate early detection of AIN and timely discontinuation of the offending medication. MRSA Infections could quickly escalate into severe sepsis resulting in death, if not recognized and treated abruptly. This pathogen uncommonly causes Community-Acquired Pneumonia (CAP), which can lead to under treatment due to delayed coverage with anti-MRSA antibiotics resulting in poor clinical outcome. We herein describe an unusual case of MRSA CAP during COVID-19 pandemic in an 80-year-old male who was unresponsive and found to be in septic shock, intubated outside the hospital setting, and then brought to intensive care unit for further management. Laboratory and radiographic studies revealed MRSA in sputum culture and extensive bilateral consolidation with bilateral ground glass opacities and pleural effusions on imaging. Our patient was successfully treated with linezolid and extubated within 48 hours with a favorable outcome. High index of suspicion and a timely coverage with anti-MRSA antibiotics would reduce mortality and lead to a better outcome in otherwise fatal infection. Risk is very high in certain occupations in endemic regions and the diagnosis may be challenging in absence of high clinical suspicions. Here, we report a case of coccidiomycosis meningitis in an immunocompetent individual, who went to Arizona for a week for his painting job. The aim of this study was to extrapolate, through scientific evidence and previous work on the effects of microgravity, on the role of neuroscience, physical exercise and psychomotor skills. Through verbal and non-verbal language, tonic-emotional communication, one is able to help the person in his uniqueness, exploiting the communicative, emotional and motor potential of the latter which, remained latent, led him to isolation; therefore trying to develop a harmonious relationship with himself and with the world. I hope that this work can provide food for thought with respect to the essential need of each individual to be satisfied. They must take into account and take a “look” into the needs, desires and potential of the individual. Within this thesis, the theory and practice of this science has been briefly described, its application, in the specific case, in the isolation of the special forces, in the cognitive and postural adaptation capacity of the military air force. described the techniques, such as Psychocontact and the motor-muscular relaxation methods that I have mostly used in this path, still in progress. I conclude my thesis with personal considerations on this experience that involved me personally, no longer looking only with the eyes of a rehabilitation therapist, but with those of a professional, of a health scientist whose training is based on a egodynamic conception, centered on the subject in its entirety and in its complexity.) Through this new science, I was able to appreciate my inner change, which opened up new perspectives to help the person; no longer patient, but as a person understood in his uniqueness and identity. There is, of course, still a lot of work to be done and this little dissertation only wants to enrich or eventually fill where classical medicine fails to reach; always to meet and help the person. She arrived to emergency for onset orthopnea and dyspnoea on mild exertion. During the hospitalization is made a diagnosis of metastatic pancreatic adenocarcinoma. 10 days after admission suddenly the women reported severe pain in lower limbs and she was unable to maintain an upright position. Femoral, popliteal and posterior tibial pulses were not palpable bilaterally. At the Echo-doppler of the lower limbs limb pulses were barely appreciable with a tardus-parvus waveform. angiography showed critical thrombus formation in the sub-renal descending segment of the aorta and iliac arteries (which are CT commonly found in Leriche Syndrome); there was no perfusion of the inferior mesenteric artery; the internal and external iliac arteries as well as thigh and leg arteries showed slow and filiform flow with multiple sub-stenotic segments. Despite the immediate medical and surgical treatment woman died. Patients without known atherosclerosis risk factors should be investigated for hypercoagulable diseases or for co-morbidities which carry an increased risk of thrombo-embolism such as cancer.
