[Behavioral treatment of functional anejaculation and factors influencing the therapeutic effect]. [功能性不射精的行为治疗及影响疗效的因素]。

{"title":"[Behavioral treatment of functional anejaculation and factors influencing the therapeutic effect].","authors":"Yu-Ping Fan, Wen-Qiang Huang, Bin-Ya Liu, Meng-Meng Ma, Jin-Xia Zheng, Xiao-Ming Teng","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To study the effect of a modified behavioral treatment (MBT) on functional anejaculation and analyze the factors influencing the therapeutic efficacy.</p><p><strong>Methods: </strong>We enrolled in this study 59 men aged 24-45 years visiting the Andrology Clinic of Shanghai First Maternity and Infant Hospital from August 2019 to May 2021 and complaining of aejaculation in sexual intercourse but normally ejaculating during masturbation. Thirty-nine of the patients underwent conventional behavioral treatment (the CBT group) and the other 20 received MBT, namely, changing the masturbation method combined with audiovisual stimulation during sexual intercourse (the MBT group). We compared the therapeutic effects between the two groups of patients, and analyzed the correlation of the outcomes of MBT with age, abstinence duration, use of audiovisual stimulation, change of the sexual position, mean bilateral testis volume and sex hormone levels.</p><p><strong>Results: </strong>After treatment, 22 (37.29%) of the patients achieved successful ejaculation at least once in sexual intercourse, 11 (55.00%) in the MBT group, and the other 11 (28.21) in the CBT group, with a significantly higher effectiveness rate in the former than in the latter (P<0.05). The effectiveness rate was significantly correlated to the method of standing-position masturbation plus sexual intercourse and reduction in the frequency of masturbation among various strategies of behavioral treatment (P<0.05).</p><p><strong>Conclusion: </strong>MBT has a certain effect on functional anejaculation, and targeting the previous events of the patient is the key to the therapeutic efficacy. {"title":"[Causal association between depression and stress urinary incontinence: A two-sample bidirectional Mendelian randomization study].","authors":"Cheng-Xiao Jiang, Wei-Qi Yin, Jing-Jing Xu, Ying-Jiao Shi, Li Wang, Zhi-Bo Zheng, Rui Su, Qin-Bo Hu, Jun-Hai Qian, Shu-Ben Sun","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To investigate the causal correlation between depression and stress urinary incontinence (SUI) using Mendelian randomization (MR) analysis.</p><p><strong>Methods: </strong>We searched the FinnGen Consortium database for genome-wide association studies (GWAS) on depression and obtained 23 424 case samples and 192 220 control samples, with the GWAS data on SUI provided by the UK Biobank, including 4 340 case samples and 458 670 control samples. We investigated the correlation between depression and SUI based on the depression data collected from the Psychiatric Genomics Consortium (PGC). We employed inverse-variance weighting as the main method for the MR study, and performed sensitivity analysis to verify the accuracy and stability of the findings.</p><p><strong>Results: </strong>Analysis of the data from the UK Biobank and FinnGen Consortium showed that depression was significantly correlated with an increased risk of SUI (P=0.005), but not SUI with the risk of depression (P=0.927). {"title":"[Action mechanism of Huotu Jiji Pellets in the treatment of erectile dysfunction: An exploration based on network pharmacology and molecular docking].","authors":"Xue-Qin Chen, Xuan Zhou, Hong-Ping Shen, Jia-Yi Song, Yun-Jie Chen, Yuan-Bin Zhang, Yi-Li Cai, Yi Yu, Ya-Hua Liu","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To explore the potential action mechanism of Huotu Jiji Pellets (HJP) in the treatment of erectile dysfunction (ED) based on network pharmacology and molecular docking.</p><p><strong>Methods: </strong>We identified the main effective compounds and active molecular targets of HJP from the database of Traditional Chinese Medicine Systems Pharmacology (TCMSP) and Integrative Pharmacology-Based Research Platform of Traditional Chinese Medicine (TCMIP) and the therapeutic target genes of ED from the databases of Genecards. Then we obtained the common targets of HJP and ED using the Venny software, constructed a protein-protein interaction (PPI) network of HJP acting on ED, and screened out the core targets with the Cytoscape software. Lastly we performed GO functional enrichment and KEGG pathway enrichment analyses of the core targets followed by molecular docking of HJP and the core targets using Chem3D and AutoDock Tools and QuickVina-W software.</p><p><strong>Results: </strong>A total of 64 effective compounds, 822 drug-related targets, 1 783 disease-related targets and 320 common targets were obtained in this study. PPI network analysis showed that the core targets of HJP for ED included ESR1, HSP90AA1, SRC, and STAT3. GO functional enrichment analysis indicated the involvement of the core targets in such biological processes as response to xenobiotic stimulus, positive regulation of kinase activity, and positive regulation of MAPK cascade. KEGG pathway enrichment analysis suggested that PI3K-Akt, apoptosis, MAPK, HIF-1, VEGF, autophagy and other signaling pathways may be related to the mechanism of HJP acting on ED. {"title":"[Comprehensive therapy of traditional Chinese medicine for erectile dysfunction with damp-heat stasis: A clinical observation of 103 cases].","authors":"Jun-Chao Yao, Bao-Jun Ju, Xiao Li, Lu-Yu Li, Miao-Miao Ma, Yong-Tao Zhang","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To assess the clinical effect and safety of comprehensive therapy of traditional Chinese medicine (TCM) in the treatment of erectile dysfunction (ED) with damp-heat stasis.</p><p><strong>Methods: </strong>We selected 108 cases of ED with damp-heat stasis meeting the inclusion criteria and treated with tadalafil (the control group, n = 54) or tadalafil + comprehensive TCM therapy (the trial group, n = 54) in the First Affiliated Hospital of Henan University of Chinese Medicine in the same period. After 8 weeks of treatment, we recorded the patients' scores on IIEF-5, TCM syndrome, erectile quality (EQS), 9-Item Patient Health Questionnaire (PHQ-9) and Generalized Anxiety Scale 7 (GAD-7). At 16 weeks of our study, we collected the efficacy parameters, safety indicators and adverse reactions by telephone follow-up and compared the data obtained between the two groups of patients.</p><p><strong>Results: </strong>Totally, 103 of the patients completed the study, 51 in the control and 52 in the trial group. Compared with the baseline, the IIEF-5 and EQS scores were both markedly increased after 8 weeks of treatment in the trial group (12.35±3.00 vs 18.36±2.82, P< 0.05; 39.5 [30.25-43] vs 67.5 [54.5-76.75], P< 0.05) and the control (11.96±2.79 vs 15.88±3.86, P< 0.05; 38.0 [29-42] vs 56 [49-64], P< 0.05), even more significantly in the former than in the latter (P< 0.05); the TCM syndrome and GAD-7 scores were remarkably decreased in the trial (9.5 [8-12] vs 4.0 [2.25-5], P< 0.05; 5 [2.25-6.75] vs 2.5 [1-4.75], P< 0.05) and the control group (10.0 [8-12] vs 5.0 [3-6], P< 0.05; 5.0 [3-6] vs 4.0 [2-5], P< 0.05), even more significantly in the former than in the latter (P< 0.05), so were the PHQ-9 scores (P< 0.05), but with no statistically significant difference between the two groups (P > 0.05). The IIEF-5 scores of the two groups remained significantly higher than the baseline during the follow-up (P< 0.05), even higher in the trial than in the control group (17.04±2.60 vs 14.16±3.34, P< 0.05). {"title":"[Correlation of lipid metabolism-related single nucleotide polymorphisms with abnormal semen quality in men].","authors":"Xuan Liu, Yuan-Jing Li, Yue-Jia Li, Jia-Jie Bi, Jing Ma, Fang-Fang Dai, Shu-Song Wang","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To explore the potential impact of lipid metabolism-related single nucleotide polymorphisms (SNP) on semen quality in men.</p><p><strong>Methods: </strong>We selected 284 semen samples from Xingtai Infertility Hospital and Hebei Human Sperm Bank collected between February and October 2023, 33 from oligozoospermia (OS), 97 from asthenozoospermia (AS) and 54 from oligoasthenozoospermia (OAS) patients and the other 100 from normal men. We performed computer-assisted semen analysis (CASA) of the samples, extracted blood DNA and, using the MassARRAY System, genotyped the target genes, determined the genotypes of 13 SNPs and compared their distribution, their correlation with BMI and semen quality in different groups.</p><p><strong>Results: </strong>The mutant homozygous (TT) genotype of the FADS2 rs2727270 gene seemed to be a risk factor for AS (OR = 4.420, P= 0.047), while the APOA2 rs5082-A allele and MC4R rs17782313 heterozygous (TC) genotype important protective factors for OS (OR = 0.422 and 0.389; P= 0.045 and 0.043, respectively). A significantly higher sperm concentration was found associated with the MC4R rs17782313 heterozygous (TC) genotype than with the homozygous (CC) genotype. {"title":"[Magnetic stimulation combined with moxibustion improves mild to moderate overactive bladder and sexual function in women].","authors":"Dan Zhou, Jia-Wei Wang, Si-Jing Wang, Chao-Liang Shi, Guo-Wei Shi, Yang-Yun Wang, Ke Xu","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To investigate the clinical effect of magnetic stimulation combined with moxibustion on mild to moderate overactive bladder (OAB) and sexual function in women.</p><p><strong>Methods: </strong>We enrolled 80 female patients with mild to moderate OAB in this study and equally randomized them into a control and an experimental group, the former treated by magnetic stimulation and the latter by magnetic stimulation combined with moxibustion, both for 8 weeks. We obtained from the patients their OAB syndrome scores (OABSS), 72-hour urination diary (72-h UD) scores, International Consultation on Incontinence Questionnaire - Overactive Bladder (ICIQ-OAB) scores and female sexual function indexes (FSFI), and compared them between the two groups before and after intervention.</p><p><strong>Results: </strong>A total of 77 patients completed the study, 37 in the control and 40 in the experimental group. There were no statistically significant differences in the baseline data between the two groups (P > 0.05). We equally randomized the subjects into an experimental and a control group, the former receiving MSPI and the latter trained on the conventional teaching model without any psychological support intervention.</p><p><strong>Results: </strong>Compared with the baseline, the HAMD scores were significantly decreased in the experimental group after intervention (12.4±2.1 vs 8.9±2.4, P<0.01), but increased in the controls (13.1±1.8 vs 14.7±1.9, P<0.01); the skill scores dramatically increased in the experimental group (82.6±4.7 vs 91.2±2.4, P<0.01), but decreased in the control group after intervention (81.0±3.5 vs 80.4±2.7, P = 0.28).</p><p><strong>Conclusion: </strong>MSPI can significantly enhance the learning enthusiasm of nursing students in a short period, reduce their psychological stress and improve teaching outcomes. Most cases of PCa are treated by radical prostatectomy, but with the development of medical imaging and innovation in therapeutic theories and technology, focal therapy has shown better application prospects in the treatment of PCa. Compared with radical prostatectomy, focal therapy yields satisfactory results in terms of effectiveness and reduction of complications in addition to avoidance of overtreatment and treatment-related financial burden. {"title":"[Testis-sparing microsurgery for benign testis tumor: A report of 16 cases].","authors":"Lei Yu, Jing Zhao, Hong-Qiang Wang, Pei-Hong Zhou, Jian-Hua Men, Gang Wang, Qiang Li, Yu Pan, Wen-Xin Li, Lin Qian, Shen-Qian Li, Pei-Tao Wang, Tao Jing","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To investigate the safety and clinical effect of testis-sparing microsurgery (TSMS) in the treatment of benign testis tumor (BTT).</p><p><strong>Methods: </strong>We retrospectively analyzed the clinical data on 16 cases of BTT treated in the Department of Andrology of the Affiliated Hospital of Qingdao University from October 2020 to February 2023. The median age of the patients was 23 years. All the tumors were unilateral, 7 in the left and 9 in the right side, with a median diameter of 1.85 cm (1.0-3.5 cm). The patients all underwent color Doppler flow imaging (CDFI), MRI, semen analysis and examination of serum T, alpha-fetoprotein (AFP), human chorionic gonadotropin (HCG) and lactate dehydrogenase (LDH), followed by TSMS. The boundaries between the tumors and normal testis tissue were accurately identified under the microscope, and the tumors and the adjacent normal testis tissue 2 mm from their margins were excised completely. Bipolar coagulation forceps were used for wound hemostasis to maximally preserve the normal testis tissue. The resected specimens were subjected to fast frozen pathology intraoperatively, and the patients were followed up for 14-40 months by regular scrotal CDFI, MRI and examinations of serum T and semen parameters.</p><p><strong>Results: </strong>The levels of serum T, AFP, HCG and LDH and semen parameters were all within the normal range preoperatively. TSMS were successfully completed in all the cases, and all were pathologically confirmed as BTT according to the latest edition of WHO Classification of Tumors: Urinary and Male Genital Tumors. CDFI showed normal blood supply within the testis tissue at 1 month after surgery. No signs of intra-testicular tumor residue, recurrence or metastasis, nor significant changes in the levels of serum T, AFP, HCG or LDH or semen parameters were observed during the follow-up as compared with the baseline. Natural conception was achieved in 2 cases at 16 and 18 months respectively after surgery.</p><p><strong>Conclusion: </strong>BTT can be differentially diagnosed by CDFI and MRI before surgery and confirmed by histopathology.
