This study aimed to investigate whether the site of VES origin and other VES characteristics are associated with a decline in LVEF.\u0000 Methods: The rhythm Holter records and follow-up files of 400 patients who presented to the outpatient clinic with complaints such as palpitations, presyncope, syncope, and dyspnea between January 2017 and March 2023 and who were prescribed a rhythm Holter as part of their evaluation were retrospectively reviewed. The relationship between reduced LVEF, defined as LVEF <50% on echocardiography, and VES characteristics was examined in patients with normal LVEF and a VES burden above 10%, utilizing 24- and 48-h electrophysiology studies.\u0000 Results: The study comprised 34 patients with a mean age of 59.8±17.0 (range: 21–87) years. Among them, 55.9% (19 patients) were female, and the mean ejection fraction (EF) % was 49.5±11.3 (range: 25–67). Patients with EF % <50 (n=18) exhibited significantly higher diastolic diameter (5.3±0.5 vs. 4.7±0.5 cm, respectively; p=0.004), VES burden (32.3 vs. 16.7, respectively; p=0.0001), longer coupling interval (CI) measurements (p=0.018), and QRS duration (p=0.0001) compared to patients with EF % >50 (n=16). A positive correlation was observed between VES load and QRS duration in the group with EF <50% (r=0.664; p=0.002). Ertürk","doi":"10.51645/khj.2022.m269","DOIUrl":"https://doi.org/10.51645/khj.2022.m269","url":null,"abstract":"Introduction: In this study, we aimed to investigate the relationship between the preprocedural logistic clinical SYNTAX score (Log CSS) and major cardiovascular events (MACE) in patients with true bifurcation lesions who underwent revascularization with the double stent strategy.\u0000 Patients and Methods: A total of 233 non-ST elevation myocardial infarction (NSTEMI) patients who underwent revascularization with a double stent strategy were retrospectively included in the study. Anatomical SYNTAX score (SS) and log CSS were calculated. The patients were divided into two groups according to the cut-off value determined by ROC analysis. The prediction of two-year MACE with Log CSS was evaluated.\u0000 Results: MACE developed in 33.9% of patients with high Log CSS and 8.5% of patients with low Log CSS (p< 0.001). In the Kaplan-Meier survival analysis, the long-term survival of the high Log CSS patients was significantly lower than the low Log CSS group (Log-rank p< 0.001). Sacubitril/valsartan, a first-generation drug, was approved to use heart failure treatment recently. There are limited studies on the relationship between sacubitril/valsartan and left ventricular contraction. In our study, we aimed to evaluate the changes in left ventricular ejection fraction (LVEF) after sacubitril/valsartan treatment.\u0000 Patients and Methods: Fifty-two patients with heart failure and reduced ejection fraction (HFrEF) were enrolled in this study. The baseline demographic, clinical, and echocardiographic characteristics of 52 patients were compared using the Bayesian method.\u0000 Results: Fifty-two patients with heart failure and reduced ejection fraction (HFrEF) were included in the final analysis (66.2 ± 9.3 years, 69.2% male). Sacubitril/valsartan initial dose was low in 44.2% of patients, and intermediate in 55.8%. In the low initial dose population, the increase in absolute LVEF was 3.87 (95% HDI 1.53-6.20) and in the intermediate initial dose population, the increase in absolute LVEF was 5.89 (95% HDI 4.18-7.61). The C-reactive protein (CRP)-to-serum albumin ratio (CAR) has been shown to predict peripheral artery disease severity in patients with carotid artery disease (CAD). However, the relationship between CAR and long-term mortality in CAD patients following carotid artery stenting (CAS) remains uncertain. The goal of this study was to investigate the ability of CAR to predict long-term mortality in patients with CAS.\u0000 Patients and Methods: A total of 998 CAD patients who had percutaneous carotid artery intervention were reviewed retrospectively, and 482 of them without follow-up data were excluded from this study. The study included patients who had percutaneous carotid artery intervention between 2012 and 2018. Before the procedure, serum CRP, complete blood counts, and albumin levels were measured. The relationships between patients’ characteristics, procedural details, and CAR with all-cause mortality were evaluated in the long-term follow-up after CAS.\u0000 Results: Older age (HR= 1.044; 95% CI= 1.001-1.089; p= 0.046), smoking (HR= 2.636; 95% CI= 1.213- 5.728; p= 0.014), blood urea (HR= 1.017; 95% CI= 1.002-1.033; p= 0.025), albumin (HR= 0.252; 95% CI= 0.132-0.480; p< 0.001), CRP levels (HR= 1.336; 95% CI= 1.055-1.691; p= 0.016) and CAR (HR= 1.012; 95% CI= 1.003-1.022; p= 0.008) were statistically associated with all-cause mortality at 55.2 ± 12.4 months follow-up after CAS in multivariate COX regression analysis.\u0000 Conclusion: In patients undergoing CAS, CAR was independently related to long-term mortality. Bayraktar, Betul Nur Keser, Ebuzer Aydın","doi":"10.51645/khj.2022.m321","DOIUrl":"https://doi.org/10.51645/khj.2022.m321","url":null,"abstract":"Introduction: The aim of this study is to investigate the impact of obesity on patient compliance with elastic compression stockings and to reveal patient behavior-related and compression stockings-related factors for noncompliance.\u0000 Patients and Methods: Between July and August 2022, a total of 246 patients with chronic venous insufficiency were included in the study. The patients were divided into two groups, obese and non-obese patients (88 vs. 158 patients), and questioned for proper use of compression stockings and the patient-reported reasons for the non-compliance.\u0000 Results: The mean age was 49.0 ± 13.1, and the mean BMI was 28.6 ± 5.2. Only 51 patients (20.7%) out of 246 complied with stocking use. The mean number of days per week in which the compression stockings were properly used was 2.9 ± 1.7 in the study population. Patient compliance was higher in the non-obese group (12.5% vs. 25.3%, p= 0.017), and the number of days per week in which the compression stockings were worn properly was higher in the non-obese group (2.6 ± 1.3 vs. 3.1 ± 1.9, p= 0.032).\u0000 Conclusion: The compliance of our patients with elastic compression stockings was found to be as low as 20.7%. Compliance was lower in the group of obese patients. Obesity and diabetes mellitus were the only factors associated with noncompliance in univariate logistic regression analysis. The Neutrophil/Lymphocyte Ratio (NLR) is used to predict mortality and morbidity after congenital heart surgery. In this study, the effect of postoperative NLR changes on prolonged intensive care unit stay in newborns with TAPVD was searched.\u0000 Patients and Methods: The newborns with TAPVD who were followed up and operated on in the pediatric cardiac intensive care unit between May 1, 2020, and May 1, 2022, were included in the study. Hematological parameters and NLR changes were recorded before and after the operation (1st-2nd-3rd days). Long ICU duration (PCILOS, being in the longest 25 percent as duration) was accepted as morbidity. The effects of NLO changes on PCILOS were examined.\u0000 Results: Twenty-four patients were included in the study. The median age was 18 days (IQR 12 days-24 days) and the median weight was 3 kg (IQR= 2.8-3.2). Fifteen patients were male (63%). Median mechanical ventilator duration, intensive care unit stay, and hospital stay were 40 hours (IQR= 30-48 hours), seven days (IQR= 5-9), and 15 days (IQR= 12-18), respectively. PCILOS duration was determined as >9 days. Three patients (12.5%) died in the postoperative first 30 days. Self-care in heart failure is one of the fundamental elements of quality of life and disability-free living. The aim of this study was to assess the validity and reliability of the Turkish version of the Self-Care of Heart Failure Index v.2 (SCHFI).\u0000 Patients and Methods: The study sample included 233 patients who presented with a diagnosis of heart failure and volunteered to participate in the research at a training and research hospital. The data were collected using a personal information form and the SCHFI. The SCHFI was translated into Turkish, and the internal consistency coefficient and the item-total points reliability coefficient were analyzed for the reliability study. Yet, there is a paucity of data regarding long-term adverse events in patients with critical limb ischemia (CLI) undergoing endovascular revascularization for BTK lesions. Recently introduced systemic immune-inflammation index (SII) is a reliable indicator of poor outcomes in various cardiovascular conditions. Herein, we aimed to investigate the prognostic role of the SII on mortality in patients with CLI undergoing endovascular revascularization for BTK lesions.\u0000 Patients and Methods: The records of 112 patients with symptomatic CLI undergoing endovascular revascularization for BTK lesions between January 2015 and December 2019 were analyzed. Patients were divided into groups with low and high SII values based on an SII cut-off value derived from a ROC analysis. For each group, procedural details and follow-up outcomes were analyzed.\u0000 Results: The mean follow-up time was 40.3 ± 19.9 months. According to our data, patients with high SII values had higher rates of mortality compared to patients with low SII values (65.2% vs 30.3%, p< 0.001). To determine the SII cut-off value for predicting mortality, the ROC curve was drawn, and the best cut-off value was determined as 966 by using the Youden index, (AUC= 0.658, 95% CI= 0.556-0.760, p= 0.004). Only ten patients developed skin closure defects at the sternal incision line during the first postoperative week, and one of them required revision of the skin incision site. As far as we know, studies examining the relationship between SYNTAX Score and Cystatin C (Cys-C) are limited in the literature. In this study, we aimed to investigate the relationship between Cys-C level and SS in patients presenting with ST-elevation myocardial infarction (STEMI).\u0000 Patients and Methods: One hundred ninety-two patients who underwent coronary angiography for STEMI between June 2021 and December 2021 in our center were included in the study. The patients were divided into two groups according to their SS values: Group 1 included patients with SS<22, and Group 2 included patients with SS≥ 22. The two groups were compared in terms of baseline characteristics and serum Cys-C levels.\u0000 Results: Patients in Group 2 had significantly higher Cys-C levels than those in Group 1 (1.7 ± 0.53 vs 1.3 ± 0.29; p= 0.006). In the correlation analysis, SS showed a positive correlation with the Cys-C level (r= 0.189, p= 0.015). Multivariate logistic regression analysis showed that Cys-C level was an independent predictor of high SS (β=0.306, p= 0.006). When receiver operating characteristic (ROC) curve analysis was performed, the optimal cut-off value of Cys-C to predict severe coronary artery disease (indicating high SS) was Cys-C≥ 1.56, which was predictive for severe coronary artery disease with 74% sensitivity and 72% specificity [area under the curve (AUC)= 0.745, 95% confidence interval (CI)= 0.624-0.835, p<0.001).\u0000 Conclusion: Cystatin C levels are independently associated with high SS in STEMI patients.
