The acupuncture group was treated with acupuncture at Sishencong (EX-HN 1) and bilateral Shenmen (HT 7), Sanyinjiao (SP 6) combined with compatibility of acupoints based on syndrome differentiation, once a day for 6 d and then rest for 1 d, for a total of 4 weeks. The medication group was treated with oral estazolam tablets before bedtime, 1 tablet each time, for a total of 4 weeks. Before and after treatment, the scores of Pittsburgh sleep quality index (PSQI), mini-mental state examination (MMSE), Montreal cognitive assessment (MoCA) and auditory verbal memory test (AVMT) of the two groups were compared, and the effects were evaluated.</p><p><strong>Results: </strong>After treatment, the PSQI sub-item scores and total scores of the two groups were lower than those before treatment ( <i>P</i><0.05 ), and above scores in the acupuncture group were lower than those in the medication group ( <i>P</i><0.05 ); the scores of MMSE, MoCA and AVMT in the two groups were higher than those before treatment ( <i>P</i><0.05 ), and the scores in the acupuncture group were higher than those in the medication group ( <i>P</i><0.05 ). Before menstrual period, on the first day of menstruation, and on the third day after menstruation, LSCI was used to observe the surface microcirculation at the abdominal acupoints of conception vessel, i. e. Yinjiao (CV 7), Qihai (CV 6), Shimen (CV 5), Guanyuan (CV 4), Zhongji (CV 3) and Qugou (CV 2), acupoints of thoroughfare vessel, i. e. Huangshu (KI 16), Zhongzhu (KI 15), Siman (KI 14), Qixue (KI 13), Dahe (KI 12), Henggu (KI 11) and acupoints of lumbosacral region of governor vessel, i. e. Xuanshu (GV 5), Mingmen (GV 4), Yaoyangguan (GV 3), Yaoshu (GV 2) as well as two non-acupoints.</p><p><strong>Results: </strong>Before menstrual period, there was no significant difference in the surface blood perfusion of the acupoints between the dysmenorrhea group and the normal group (<i>P</i>>0.05). On the first day of menstruation, the surface blood perfusion of Xuanshu (GV 5), Mingmen (GV 4), Yaoyangguan (GV 3) and right Huangshu (KI 16) in the dysmenorrhea group was higher than that in the normal group (<i>P</i><0.05, <i>P</i><0.01). On the third day after menstruation, the surface blood perfusion of the right Henggu (KI 11) in the dysmenorrhea group was lower than that in the normal group (<i>P</i><0.05).</p><p><strong>Conclusion: </strong>In patients with primary dysmenorrhea, on the first day of menstruation, the surface blood perfusion of Xuanshu (GV 5), Mingmen (GV 4), Yaoyangguan (GV 3) of governor vessel, and the right Huangshu (KI 16) of thoroughfare vessel is increased, while on the third day after menstruation, the surface blood perfusion of the right Henggu (KI 11) of thoroughfare vessel is decreased. In the model group and the electroacupuncture group, the model of acute blunt trauma of gastrocnemius muscle was established by self-made impactor. In the electroacupuncture group, electroacupuncture was applied at \"Chengshan\" (BL 57) and \"Yanglingquan\" (GB 34) on the right side, with disperse-dense wave, in frequency of 2 Hz/100 Hz, once a day, 30 min each time. Electroacupuncture intervention was performed for 3, 7 and 14 days according to the sampling time. On the 1st, 3rd, 7th and 14th days after modeling, the mechanical withdrawal pain threshold of hindfoot was detected by Von Frey method; the standing time and the maximum contact area of the right hindfoot were recorded by Cat Walk XT<sup>TM</sup> animal gait analysis instrument; the morphology of the right gastrocnemius muscle and the number of inflammatory cells were observed by HE staining; the positive expression of paired box gene 7 (Pax7) and myogenic differentiation (MyoD) of the right gastrocnemius muscle was detected by immunofluorescence.</p><p><strong>Results: </strong>After modeling, the muscle fiber rupture and massive infiltration of red blood cells and inflammatory cells were observed in the right gastrocnemius muscle; after electroacupuncture intervention, the morphology of muscle fiber was intact and the infiltration of inflammatory cells was improved. Compared with the blank group, in the model group, the differences of mechanical withdrawal pain threshold between the left and right foot were increased (<i>P</i><0.05), the standing time was shortened and the maximum contact area of the right hindfoot was decreased (<i>P</i><0.05), the number of inflammatory cells and the positive expression of Pax7 and MyoD of the right gastrocnemius muscle were increased (<i>P</i><0.05) on the 1st, 3rd, 7th and 14th days after modeling. The Hunyuan moxibustion group was treated with <i>Hunyuan</i> moxibustion at Guanyuan(CV 4),40 min each time, once a day; in the western medication group,loperamide hydrochloride capsules (2 mg each time, 3 times a day) and bacillus licheniformis live capsules (0.5 g each time, 3 times a day) were given orally.Both groups were treated for 20 days. The scores of irritable bowel syndrome(IBS)symptom severity scale(IBS-SSS), IBS quality of life scale (IBS-QOL) and TCM symptom grading quantitative were observed before and after treatment, and the clinical efficacy and safety were evaluated in the two groups.</p><p><strong>Results: </strong>After treatment,each item scores and total scores of IBS-SSS in the two groups were lower than those before treatment(<i>P</i><0.05), and the total scores of IBS-QOL were higher than those before treatment (<i>P</i><0.05);each item score and total score of IBS-SSS in the Hunyuan moxibustion group were lower than those in the western medication group (<i>P</i><0.05), and the total score of IBS-QOL in the Hunyuan moxibustion group was higher than that in the western medication group (<i>P</i><0.05).After treatment, each item score and total score of TCM symptom grading quantitative in the Hunyuan moxibustion group were lower than those before treatment (<i>P</i><0.05), the abdominal pain, diarrhea, lack of appetite scores and total score in the western medication group were lower than those before treatment (<i>P</i><0.05);and the abdominal pain, soreness and weakness of waist and knees, fear to cold and cold limbs scores and total score in the Hunyuan moxibustion group were lower than those in the western medication group (<i>P</i><0.05).The total effective rate was 90.0%(27/30)in the Hunyuan moxibustion group, which was higher than 73.3%(22/30)in the western medication group (<i>P</i><0.05). {"title":"[Analysis on status quo of outcomes and measurement instruments of randomized controlled trials of acupuncture for post-stroke dysphagia].","authors":"Wen-Cong Cao,&nbsp;Xing-Ying Qiu,&nbsp;Bing-Qing Liu,&nbsp;Geng Li,&nbsp;Ze-Huai Wen","doi":"10.13703/j.0255-2930.20220217-k0007","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20220217-k0007","url":null,"abstract":"<p><strong>Objective: </strong>To analyze the report status of outcomes and measurement instruments of randomized controlled trials (RCTs) of acupuncture for post-stroke dysphagia, so as to provide a basis for designing clinical trials and developing the core outcome set in acupuncture for post-stroke dysphagia.</p><p><strong>Methods: </strong>RCTs of acupuncture for post-stroke dysphagia were searched in databases i.e. CNKI, SinoMed, Wanfang, PubMed, EMbase, Web of Science and clinical trial registries i.e. ClinicalTrials.gov and Chinese Clinical Trial Registry (ChiCTR), from January 1st, 2012 to October 30th, 2021. By literature screening and data extraction, outcomes and measurement instruments were summarized and analyzed.</p><p><strong>Results: </strong>A total of 172 trials (including 165 RCTs and 7 ongoing trials registrations) were included, involving 91 outcomes. The outcomes could be classified into 7 domains according to functional attributes, namely clinical manifestation, physical and chemical examination, quality of life, TCM symptoms/syndromes, long-term prognosis, safety assessment and economic evaluation. It was found that there were various measurements instruments with large differences, inconsistent measurement time point and without discriminatively reporting primary or secondary outcomes.</p><p><strong>Conclusion: </strong>The status quo of outcomes and measurement instruments of RCTs of acupuncture for post-stroke dysphagia is not conducive to the summary and comparison of each trial's results. Forty-eight rats with successful model establishment were further randomly divided into a model group and three moxibustion groups (mild moxibustion group, rotating moxibustion group and sparrow-pecking moxibustion group), with 12 rats in each group. The acupoints \"Quchi\" (LI 11), \"Dazhui\" (GV 14) and <i>ashi</i> point were used in all moxibustion groups, with mild moxibustion, rotating moxibustion, and sparrow-pecking moxibustion intervention given respectively, each acupoint was treated with moxibustion for 10 min a day, and 6 days were considered one course of treatment, with a total of three courses. After the intervention, the arthritis index (AI), the Evans blue (EB) extravasated volume in the soft tissue of the right hind paw, and the levels of tumor necrosis factor (TNF)-α and interleukin (IL)-10 in the serum were measured by ELISA in each group. The volume of the bilateral hind paw was measured; the infrared thermal imaging was collected to analyze the temperature of the plantar area of the bilateral foot pads, and the reaction time of plantar heat pain was calculated before and after modeling, as well as after the 1st, 2nd and 3rd courses of interrention. The ankle dorsiflexion angle of the right hind foot was also measured before and after modeling, as well as after the intervention.</p><p><strong>Results: </strong>After modeling, compared with the normal group, the rats in the model group had more high-temperature areas in the bilateral hind limbs, abnormal AI score, abnormal bilateral hind paw volume, abnormal temperature of the plantar area of the bilateral foot pads, abnormal foot pain response time, abnormal right hind ankle dorsiflexion angle, abnormal right hind paw soft tissue EB extravasation, and abnormal serum TNF-α and IL-10 levels (<i>P</i><0.01, <i>P</i><0.05). After the intervention, compared with the model group, the rats in each moxibustion group had decreased or disappeared high-temperature areas in the bilateral hind limbs, EB extravasated volume in the soft tissue of the right hind paw was reduced (<i>P</i><0.05), and the right ankle dorsiflexion angle was increased (<i>P</i><0.05), serum level of TNF-α was reduced, and level of IL-10 increased (<i>P</i><0.05); the AI scores in the mild moxibustion group and the sparrow-pecking moxibustion group was decreased (<i>P</i><0.01, <i>P</i><0.05). Based on <i>jingjin</i> (muscle region of meridian, sinew/fascia) theory and taking the visceral membrane as the principal, acupuncture is delivered at sinew/fascia to promote <i>qi</i> circulation, resolve stasis and open the orifice. Guided by CT, the needle is inserted at Zhongji (CV 3), the front-<i>mu</i> point of the urinary bladder, and then goes to the prostatic capsule, meaning \"the disease of <i>zang</i> organ is treated by needling the front-<i>mu</i> point\". The concrete operation procedure of \"bone-approaching\" method is recorded originally in <i>Huangdi Neijing</i> (Inner Canon of Yellow Emperor) as short needling and <i>Shu</i> needling (referring to the category of the five needling technique). The periosteum is the most effective stimulation target of \"bone-approaching\" acupuncture for analgesia, regaining consciousness and regulating spirit. The \"bone-approaching\" acupuncture is not only prominently effective on bone <i>bi</i> syndrome, but also has the unique effect on painful, encephalogenic and emotional diseases. The paper summarizes and improves \"bone-approaching\" acupuncture, i.e. \"touching bone surface\" with needle tip by slow insertion, \"touching bone surface\" without pain by swift insertion and \"touching bone\" with needle body by oblique insertion. After the control of confounding factors with propensity score matching, 218 cases were included in either acupuncture group or non-acupuncture group. In the acupuncture group, acupuncture was applied to Dazhui (GV 14), Baihui (GV 20), <i>ashi</i> points, bilateral neck-Jiaji (EX-B 2), Fengchi (GB 20), Houxi (SI 3), Shenmai (BL 62), etc. The treatment was given once daily, one course of intervention was composed of 5 treatments and 3 courses were included. In the non-acupuncture group, the oral administration of imrecoxib tablets and cobalt tablets was prescribed for 2 weeks. Before and after treatment, the scores of Northwick Park questionnaire (NPQ) and the simplified McGill pain questionnaire (SF-MPQ) were observed, and the safety was assessed in patients of the two groups.</p><p><strong>Results: </strong>After treatment completion, the scores of NPQ and SF-MPQ were all reduced when compared with those before treatment in each group (<i>P</i><0.001), and the scores of NPQ and SF-MPQ in the acupuncture group were lower than those of the non-acupuncture group (<i>P</i><0.001). This software is designed to provide auxiliary teaching of structured organization and visualization of law knowledge of compatibility data of acupuncture and moxibustion prescriptions from ancient texts, textbooks, and clinical case records. The software is installed as a plugin in the Microsoft Office Excel, allowing the generation of visually appealing graphs and associated rules that align with the cognitive patterns of teachers and students majoring in acupuncture and moxibustion.
