{"title":"[Ovarian cancer -- can peritoneal therapy be regarded as new standard?].","authors":"A du Bois, B Schmalfeldt, W Meier, J Sehouli, J Pfisterer","doi":"10.1055/s-2006-933460","DOIUrl":"https://doi.org/10.1055/s-2006-933460","url":null,"abstract":"<p><p>Recently, the publication of the GOG 172 trial led to intraperitoneal (i. p.) chemotherapy in patients with ovarian cancer being regarded as potential new standard. The AGO Kommission Ovar, AGO-OVAR and NOGGO disagree with this view. In the GOG 172 study, several variables were mixed, and i. p.-therapy was not compared to the current standard paclitaxel plus carboplatin. The analysis was not based on an intention-to-treat (ITT) population, while even the slightest changes of significant results could lead to the elimination of their significance. Furthermore, the GOG 172 trial did not provide any details on second-line treatment which could have an impact on overall survival. High toxicity and the low number of cycles in i. p.-therapy might call the significant effect into question. The low median time of survival of the GOG 172 trial in the control arm (49.7 months) diverges from comparable collectives of three AGO-OVAR trials and the GOG 158 study (56.5-59.5 months). If the result of the GOG 172 trial had been similar, there would not have been any significance in comparison to i. p.-therapy. In middle income countries, and in many low income countries, breast cancer has become the most frequent cancer in women, supplanting cancer of the cervix. In such countries, breast cancer is usually diagnosed at an advanced stage, the majority in stage III and IV, indicating substantial delay in diagnosis. Further, because of the age distribution of the population pyramid, the majority of breast cancers are diagnosed in women under the age of 50. However, that age distribution does not mean that breast cancer is a different disease than in the West. {"title":"[Criteria of life quality after reconstructive breast cancer surgery].","authors":"H J Strittmatter, M Neises, S R Blecken","doi":"10.1055/s-2005-836887","DOIUrl":"https://doi.org/10.1055/s-2005-836887","url":null,"abstract":"<p><strong>Purpose: </strong>The aim of this study was to evaluate the quality of life of women after a breast cancer surgery. The question was if women which had reconstructive breast cancer surgery experience a higher quality of life than women who, for various distinct reasons, had not undergone reconstruction.</p><p><strong>Patients and methods: </strong>The participants of this study were women who had either received a mastectomy or those who at the same time underwent a breast reconstruction using implants during the time period from 1/1/2000 until 31/10/2003 at the University Hospital for Women of Heidelberg and Mannheim. With the help of three standardised questionnaires, the women could describe their post-surgical physical and psychological condition as well as the perceived quality of life. The study included 33 patients who had received breast implants and 31 patients without reconstruction.</p><p><strong>Results: </strong>Women who had breast cancer surgery with reconstruction through implants had less problems and restrictions concerning their physical condition as well as their functional status. Moreover, compared to those participants with no reconstruction, their cognitive and emotional burdens were not as pronounced and they also they were able to better cope with the disease. Thus, their overall quality of life was superior than that of the other women.</p><p><strong>Conclusion: </strong>Breast reconstruction after primary and secondary mastectomy is an important contribution in order to improve the self-esteem and quality of a patient's life.
