Khattar","doi":"10.5005/JP-JOURNALS-10003-1099","DOIUrl":"https://doi.org/10.5005/JP-JOURNALS-10003-1099","url":null,"abstract":"Being centered around the sphenopalatine foramen, juvenile nasopharyngeal angiofibromas are notorious for growing toward the infratemporal fossa via the pterygomaxillary fissure. Since, this tumor grows from 'within' the sinonasal space, it would be best to surgically follow it from the same path that it took to grow. With the development of endoscopic techniques and instrumentation, endoscopic approaches to the infratemporal fossa have been universally accepted. Nevertheless, since the tumor does not follow a definitive pattern, each surgery needs to be tailored. The biological characteristics of fibroosseous lesions range from indolent to aggressive and from inflammatory to neoplastic. Cemento-ossifying fibroma is a rare, benign fibro-osseous lesion composed of lamellar bone, fibrous tissue and cementum. It is usually found in the jaw bone and presents in several variant pathological subtypes. The overlapping clinical, radiologic and histomorphologic features of these subtypes have led to diagnostic dilemma for the clinician, radiologist and the histopathologist. Clinically, the lesions are usually asymptomatic, slow-growing and wellcircumscribed. However, in very few cases, these tumors have demonstrated an aggressive course of development. Lesions that involve the midface and paranasal sinuses and presenting in earlier age tend to demonstrate aggressive behavior and rapid growth. In addition to dizziness it is very often accompanied by nausea and vomiting. Pharmacotherapy plays an important role in the management of vertigo. Vestibular suppressants and drugs to control nausea and vomiting constitute the mainstay of the pharmacotherapy of vertigo. Specific drug therapy can be given in patients where the underlying disease process causing the vertigo has been identified. Despite the availability of many classes of drugs, there are no definitive, universally accepted guidelines for the treatment of vertigo which makes it even more difficult to follow first-, second- and third-line therapies when treating patients. This 'head impulse test' (HIT) as is has been known since, has stood the test of being a reliable indicator of vestibular deficit, especially for the horizontal semicircular canal. The test has been described using various techniques, such as the scleral search coil, the video HIT, etc. but even a simple bedside clinical evaluation with a little amount of experience usually sufficed to detect overt vestibular deficits. Numerous studies have compared the efficacy of the HIT as compared with other tests of vestibular deficit, the caloric test (CT) being the most commonly compared one. A look at the physiology behind the two tests reveals the differing pathways tested by the two tests in mention, and hence it would be wise to say that the two are complimentary, rather than exclusive of the other. The only two 'potentially yielding' parts of the otherwise solid bony labyrinth are the oval and round windows, which by their relative movements, pressure differentials and resilience are responsible for all the functions attributed to the inner ear. In pathologies, such as trauma, infection or occasionally congenital dehiscence, there may develop a ‘third window’ that may serve as an abnormal communication for the inner ear fluids and manifest with audiovestibular symptoms. Three such distinct entities have been identified, namely ‘superior semicircular canal dehiscence syndrome, perilymphatic fistulae and labyrinthine fistulae’.
