Lean management is known to eliminate waste and increase productivity and patient satisfaction. When the hospital faces financial difficulties the matter becomes more urgent. Fixing broken processes would enhance patient and staff satisfaction Methodology: This intervention research was done in a tertiary care operating room hospital in Lebanon. A multidisciplinary medical, nursing and quality representatives studied the root causes of problems and took appropriate actions. The advancement was measured by performance indicators over the period of 6 months. Results: The process re-design resulted in a major improvement of all indicators. Patients were better satisfied, and the flow of work become smoother. Conclusion: Lean Management increased OR efficiency, workflow, financial performance and patient satisfaction. Process re-design to reduce waste and ease work has resulted in a win-win situation for all stakeholders. More time is needed to achieve high level of performance. These relations include the treatment plans, interactions with physicians, nurses, and other staff in hospitals. Patient experience is a vital constituent of health care quality and Total Quality Management (TQM), it embraces several facets of health care delivery such as getting timely appointments, easy access to ease and access to information, and communication with health care providers. Understanding patient experience is a key step in moving toward patient-centred care. The following intervention looks at the integration and implementation of total quality management (TQM) in order to improve patient experience within the health sector. Methodology : This intervention research uses data and intervention from Lebanese hospitals where one hospital was used an experimentation site and the other as control. Patient satisfaction score was calculated using a 42 item questionnaire at the beginning of the intervention. Then, TQM was implemented in one hospital while in the second hospital nothing was done. A multidisciplinary team followed up corrective action in the first hospital. The advancement was measured by calculating the patient satisfaction score after 6 months. Results : The results showed an improvement in patient satisfaction in the intervention hospital using TQM while patient satisfaction remained unchanged in the non-intervention hospital. The overall patient satisfaction score increased from 60% to 78% in Hospital 1 whereas the scores were 62% then 63% in Hospital 2. It is worth noting the aspects related to the physical environment, admission and discharge processes showed major improvement. Also, an average value of 31.49% was indicated under the study period. Okonji (2019) examined the relationship between macroeconomic performance and economic wellbeing in Nigeria by using economic growth, debts, fiscal policy stance, monetary policy stance and efficiency of governance as measures of macroeconomic performance. The K-class model was used to estimate the data and the result showed that economic growth, through allocative efficiency engenders wellbeing while contractionary monetary policy inhibits wellbeing. Also, excessive borrowing undermines the wellbeing of the Nigerian citizens. Therefore, the study recommended that the monetary authority should maintain a relatively low interest rate.Ovat (2020) investigated the effect of macroeconomic policies on citizen’s misery in Nigeria by adapting the Hanke’s model. The study employed the Auto-regressive Distributive Lag (ARDL) model to analyse its data and found that real GDP (a proxy for economic growth) and fiscal policy failed to reduce the misery of Nigerians citizens. But monetary policy and trade policies significantly alleviate the misery of Nigerian citizens. ADF test was used to test the stationarity of the data, whereas Johansen co-integration test was employed to establish the cointegrating relation among the variables. Consequently, the findings reveal a long run equilibrium relation of income inequality and the independent variables. Moreover, vector error correction model (VECM) is estimated to find the long run and short run dynamics of the study. The results show that foreign capital inflows containing FDI, aid, debt and inflation have positive and significant effect on income inequality in the long run. While, remittances, GDP and trade openness have a negative and statistically significant impact on income inequality in the long run. {"title":"Opportunities and Constraints of Scaling Up Electricity Access in the Gambia","authors":"M. In the Gambia less than half of the population (42%) has access to electricity; over 90% are still dependent on solid biomass for cooking and heating. This has significantly hampered development and intensified poverty. Thus, a top priority for the government today is to scale up electricity access in the country especially in the rural areas. To fulfill this objective, the government has taken a number of policies and strategies all gear towards increasing national electricity access in the country. Implementation of these initiatives in a form of projects through Least Cost Power Development Plan (LCPDP) strategy have yield significant opportunities in the attainment of national energy policy target of 2030 by increasing the number of household and businesses with access to electricity. Notwithstanding, this quest is equally face with constraints in the process.These substantial risk constraints include technical, institutional, policy and financial challenges. Therefore, this article conclude that in order to achieve a reliable and effective supply of the total electricity need within the prevailing condition, the Government should encourage Public Private Arrangement of power supply with indigenize energy entities who can met the generation capacity of 50-100MW.The results further suggest that NAWEC and the Government should sign a performance contractin addition to the already signed service contract.
