A four-dimensional excretory urography (4D-CTEU) abdominal CT scan revealed a hypoattenuating fluid-filled structure dorsally to the urinary bladder, extending cranially, continuing as the uterine horns, and ending blindly caudally. On post-contrast images, during all contrast phases, a continuity of the urinary bladder and cranial vaginal walls was underlined by a hyperattenuating mucosal surface with the creation of a hypoattenuating fistulous tract. Cornelis","doi":"10.21825/vdt.90044","DOIUrl":"https://doi.org/10.21825/vdt.90044","url":null,"abstract":"In this prospective, double-blinded, randomized, clinical trial, it was evaluated whether paracetamol, as an adjunct to NSAID and opioid analgesia, might limit the requirements for intraoperative fentanyl and postoperative methadone administration in dogs undergoing a singlesite thoracolumbar hemilaminectomy for surgical treatment of an intervertebral disc extrusion. Twelve client-owned dogs were randomly assigned to two multimodal analgesia groups: NSAID + paracetamol group (group NP) and NSAID + placebo group (group N). Intraoperative analgesic assessment was based on the clinical evaluation of a nociceptive response, whereas postoperative analgesic assessment was determined by using the short form of the Glasgow Composite Pain Scale. No statistically significant difference was found in both groups for the intraoperative need for fentanyl (P = 0.18). The probability of having to administer rescue analgesia postoperatively was significantly higher in group N than in group NP (P = 0.01). For both groups, there were no serious side effects reported, nor was any significant difference found between both groups regarding the occurrence of side effects (P = 0.55). {"title":"Noodzakelijke stappen naar een daling van het antibioticumgebruik op Vlaamse melkveebedrijven - Deel 1: preventie en selectief droogzetten","authors":"L. Creytens, S. Piepers, Z. De Vliegher","doi":"10.21825/vdt.90042","DOIUrl":"https://doi.org/10.21825/vdt.90042","url":null,"abstract":"De landbouwsector ligt -ook in België- reeds langere tijd onder vuur met betrekking tot overmatig en onvoldoende onderbouwd antibioticumgebruik en de daaraan gekoppelde link met de ontwikkeling van verworven antibioticumresistentie, ook bij humane pathogenen. Sinds 2012 wordt in verschillende sectoren actie ondernomen om het overmatige en/of het verkeerde gebruik van antibiotica aan banden te leggen via adviezen, sensibilisatie, bovenwettelijke maatregelen en wetgeving. Ook in de melkveesector wordt gezocht naar meer verantwoorde manieren om deze belangrijke geneesmiddelen op een correctere manier in te zetten, om tot een substantiële reductie te komen. Aangezien zestig tot zeventig procent van het antibioticumgebruik op een gemiddeld Vlaams melkveebedrijf ingezet wordt in het kader van uiergezondheid, zal een betere uiergezondheid dankzij een geoptimaliseerd management leiden tot een verminderd antibioticumgebruik. Vandaag bestaan zelfs tools om het mastitismanagement op een bedrijf objectief te scoren, zodoende een betere mastitispreventie en uiergezondheid te bekomen. Ook zijn het selectief droogzetten van koeien en het selectief behandelen van klinische mastitis essentiële elementen om tot een meer verantwoord en verminderd gebruik van antibiotica te komen. Binnen dit concept van selectief droogzetten wordt aan de hand van specifieke parameters (bacteriologisch onderzoek, historiek van klinische mastitis, celgetalgegevens) bepaald welke koeien (hoogstwaarschijnlijk) intramammair geïnfecteerd zijn op het moment van droogzetten. Enkel deze dieren krijgen langwerkende antibiotica toegediend. De niet-geïnfecteerde koeien worden drooggezet zonder antibioticum maar worden wel beschermd tegen nieuwe intramammaire infecties met speenafsluiters. Dit leidt tot een daling van het aantal dierdagdoseringen zonder of met erg beperkte negatieve effecten voor de uiergezondheid en melkproductie op langere termijn. De voorwaarde is wel dat het uiergezondheidsmanagement op het bedrijf op punt staat. Het selectief droogzetten is al enkele jaren de norm in de Scandinavische landen en Nederland en het heeft nu ook zijn intrede gemaakt in de Belgische melkveehouderij. Het werd door de nieuwe Europese Verordening (2019/6), die in voege is sinds 28 januari 2022, theoretisch gezien toch, zelfs de norm. The owner reported that the dog had been presented for routine spaying two weeks earlier in another clinic. During the operation, the veterinarian discovered a unilateral mass at the tip of the left uterine horn, which he could not remove and ovariohysterectomy was not performed. The patient was then referred to the Hospital of the Faculty of Veterinary Medicine, Trakia University, Stara Zagora, Bulgaria. Transabdominal ultrasonography revealed the presence of a hypoechoic mass in the region of the left ovary (50.0 × 71.8 mm). Complete blood cell counts and biochemical laboratory analysis showed no abnormalities. Caudal midline celiotomy was performed and a substantially altered left ovary was observed. The right ovary and uterus had a normal size and structure. Macroscopic changes in the other abdominal organs were not found. The histopathological finding of the neoplastic formation was characteristic of an ovarian cystadenoma. At necropsy, a pronounced global distention of the uterus was noted with a prominent dilation of the upper end of the right horn and two dilations of the external wall (myometrium) of the left horn. The uterine lumen was filled with a green to yellowish pasty material. A pure culture of Staphylococcus pseudintermedius was isolated. The diagnosis of pyometra caused by S. pseudintermedius was made. Pyometra in wild canids has been rarely reported. The cause of death is suggested to be similar to the cause described in dogs, namely septic shock. Er werd een enquête opgesteld waarin gepeild werd naar het gebruik van deze voorschriften. De enquête werd uitgevoerd tijdens de periode april 2021-maart 2022. Er werd onder meer nagegaan wat de beperkingen zijn van de beschikbaarheid van de huidige diergeneesmiddelen. Er namen 132 Vlaamse dierenartsen deel; 117 (88,6%) van de respondenten schrijft medicatie voor. Hiervan schrijft 94,9% humane geneesmiddelen voor en 55,5% magistrale bereidingen. Daarnaast werd nagegaan welke geneesmiddelenklassen dierenartsen het vaakst voorschrijven. Bij humane geneesmiddelen zijn dit respiratoire geneesmiddelen (72,1%), gastro-intestinale farmaca (69,4%) en oogmedicatie (64,9%). This condition is characterized by a chronic, diffuse, fibrocollageneous thickening of parietal and visceral peritoneum with secondary encapsulation of abdominal organs, mostly small intestines. Etiopathogenesis remains incompletely understood. SEP can be divided into primary, idiopathic forms and secondary forms. Secondary SEP can be caused by many different underlying disorders of which peritoneal dialysis is the most common one in humans. Diagnosis of SEP remains difficult due to its vague clinical symptoms; therefore, a combination of medical imaging, surgery and histopathology is warranted. Treatment is challenging and the prognosis is variable, depending on the severity of the disease. A combination of surgery, medicinal therapy, nutritional support and treatment of underlying disorders is used.
