Demian","doi":"10.2478/jim-2022-0011","DOIUrl":"https://doi.org/10.2478/jim-2022-0011","url":null,"abstract":"Abstract Introduction: Schnitzler syndrome (SchS), first described in 1972, is a rare autoinflammatory condition characterized by chronic urticaria and monoclonal gammopathy of IgM or, exceptionally, IgG profile. Additional features include recurrent fever, evidence of abnormal bone remodeling, a neutrophilic dermal infiltrate on skin biopsy, leukocytosis or elevated C-reactive protein, according to the Strasbourg criteria. Case Presentation: We describe the case of a 56-year-old Caucasian male patient, who suffered from chronic urticaria, moderate-grade fever, severe generalized fatigue and arthralgias. After five years of chronic disease evolution, he was referred to the hematology department where he was found to have IgM kappa light chain monoclonal gammopathy. The constellation of symptoms, a negative rheumatologic workup, and the finding of IgM monoclonal gammopathy determined the diagnosis of Schnitzler syndrome. Bone marrow biopsy proved the association of Waldenström macroglobulinemia. One such comorbidity is represented by psychological disorders, which negatively influence the clinical course of the disease. The purpose of our study is to offer a first glimpse into the stigmatization of Romanian psoriasis patients, as well as their potential anxious or depressive manifestations. Materials and methods: The present study is based on an adapted questionnaire previously used to estimate stigmatizing attitudes in the general population, mirrored to reflect the patients’ experience in social situations such as shaking hands, maintaining friendship, kissing, or intercourse. GAD7 and PHQ8 scales were used to screen for anxiety or depression traits in the same patients. Results: Our study group consisted of 12 psoriasis patients, with a mean age of 46.75 years. While few patients reported feeling discriminated against in social contexts, 16.66% presented with mild depressive symptoms recorded by PHQ8 and 24.99% presented mild anxiety manifestations according to their GAD7 scores. This treatment has a lot of benefits, but unfortunately it also presents a number of complications and risks, among which favoring the accumulation of bacterial plaque is the most concerning. Aim of the study: In this study we aimed to evaluate the effectiveness of using the dark-field microscope as a method of motivating adult patients with orthodontic treatment in maintaining a proper oral hygiene. Material and methods: Thirty-six patients were selected based on the inclusion and exclusion criteria. At time T0, patients were divided into two groups: Group 1–microscope and Group 2 – control, in such a way that the groups were as homogeneous as possible. Dental plaque samples were collected and analyzed under a dark field microscope in the presence of the patient for subjects from Group 1 but not Group 2. All patients were then instructed on oral hygiene techniques. The following periodontal indices were recorded of each patient: plaque index (PI), index of bleeding on probing (BOP), probing depth (PD) – at T0 and T1 (after one month). Results: Comparing the PI and BOP values before (T0) and after treatment (T1), statistically significant results were detected (PI: p = 0.0020, BOP: p = 0.0297), which denotes that patients who observed the bacterial load of their dental plaque in real time using the dark-field microscope had a better oral hygiene and lower values of periodontal health indices. Regarding the PD index, the difference between time T0 and T1 was not statistically significant (p = 1.4762). Conclusions: During the orthodontic treatment, adult patients often present gingival inflammation caused by improper hygiene. Coroș","doi":"10.2478/jim-2022-0007","DOIUrl":"https://doi.org/10.2478/jim-2022-0007","url":null,"abstract":"Abstract Introduction: The creation of an abdominal stoma is a common procedure performed by surgeons as a part of the treatment for benign and malignant conditions in general surgery. Stoma formation is simple, but sometimes the associated postoperative complications have an impact on the patients’ physical and psychological state. The majority of complications do not require reoperation, but when it is indicated, we have to assess the most appropriate option for the patient. Material and Methods: We conducted a retrospective study in a single surgical center, the Department of Surgery, Mureș County Hospital, Târgu Mureș, Romania, using data from patients who have been admitted under elective conditions for stoma-related complications between 2005 and 2019. Results: A total number of 877 ostomies (653 colostomies and 224 ileostomies) were performed, and 157 patients (17.9%) developed some type of stoma complication and required surgical intervention. The mean age was 64.5 ± 2.1 years, with a male-female ratio of 1.3 to 1. The leading comorbidities included cardiovascular disease (52.2% of cases), obesity (22.2%), and diabetes (18.4%). Parastomal hernia was the most frequent complication (47.5% of cases), followed by stoma prolapse (23.4%), parastomal stenosis (20.3%), and parastomal infection (8.2%). There was an association between age and the type of complication: parastomal hernia, stoma prolapse, and stenosis were more frequent in the elderly; parastomal infection was more prevalent in young patients. A longer hospital stay was observed in case of parastomal hernia. Conclusions: Stoma formation is associated with significant morbidity. Typically, the complications appear in the elderly. Conservative treatment is essential, but some of the late complications, such as parastomal hernia, stoma stenosis, stoma prolapse, and parastomal infection, require a surgical solution. The present study aimed to investigate mechanical properties of teeth restored with universal and bulk-fill composites, by using different dentin adhesives and techniques, under in vitro conditions. Material and methods: The study was carried out on freshly extracted premolar and molar teeth. After cavity preparation, the teeth were restored with conventional and bulk-fill resin composites, using different adhesive techniques. To assess the effect of the cavity preparation, the direct restoration, and the adhesive protocol on the tooth structure, a transillumination method and Vickers’ microhardness measurements were carried out. Results: The universal composite showed an average hardness of 55.35 HV at the occlusal level of the restoration, while the bulk-fill composite showed an average of 79.93 HV at the same level. A statistically significant difference was found between the hardness values of the two composites (p = 0.02). The transillumination test revealed micro-fissures in the tooth structure in the first phase after cavity preparation and also after polymerization. Conclusions: The bulk-fill composite showed higher hardness values than the universal composite. The tested dentin adhesives did not significantly affect the hardness of the dentin at the level of the adhesive interface. Sharda","doi":"10.2478/jim-2022-0008","DOIUrl":"https://doi.org/10.2478/jim-2022-0008","url":null,"abstract":"Abstract Mucormycosis is a potentially fatal disease caused by a fungus of the order Mucorales, most commonly involving the nasal sinuses, orbits, brain, lungs, and skin. The disease affects mostly immunosuppressed individuals and patients with chronic diseases such as diabetes. The prevalence of mucormycosis is 80 times higher (0.14 per 1000) in India compared to developed countries. Since the outbreak of the COVID-19 pandemic, there has been a sudden surge in the number of mucormycosis cases, especially on the Indian subcontinent. This can be attributed to what we consider to be the perfect iatrogenic recipe: a combination between the immunosuppression caused by COVID-19, the large prevalence of uncontrolled diabetes and the simultaneous use of corticosteroids. Other factors include the excessive use of antibiotics, antifungal drugs and zinc supplements, invasive ventilation, poor hygiene and sanitization as well as the use of industrial oxygen in hospitals. As a result, an overwhelmingly large number of COVID-19 patients have developed mucormycosis during the pandemic. A review of the literature suggests that all efforts should be made to keep tight control of glycemia in COVID-19 patients along with judicious use of corticosteroids.
