{"title":"Genetic mutations and prognostic indicators in differentiated thyroid cancer: a molecular perspective.","authors":"Halim Özçevik, Müge Öner Tamam, Gündüzalp Buğrahan Babacan, Selma Şengiz Erhan, Merve Nur Acar Tayyar, Biray Ertürk","doi":"10.55730/1300-0144.5944","DOIUrl":"10.55730/1300-0144.5944","url":null,"abstract":"<p><strong>Background/aim: </strong>The aim of this study was to investigate the relationship between the presence of the BRAF, HRAS, NRAS, and KRAS gene mutations and the development of dedifferentiation (iodine-refractory disease) and extrathyroidal disease in patients with differentiated thyroid carcinoma (DTC).</p><p><strong>Materials and methods: </strong>The patient group included 77 adults classified as intermediate or high-risk according to the American Thyroid Association's 2015 guidelines who underwent total thyroidectomy followed by radioiodine I-131 (RAI) therapy between June 2014 and December 2022. Clinical data were collected via the hospital information system, including the number of surgeries and RAI treatments and the levels of thyroglobulin (Tg), anti-thyroglobulin, and thyroid-stimulating hormone. The histopathological subtypes of DTC were reevaluated, and mutation analyses of the BRAF, KRAS, NRAS, and HRAS genes were performed using real-time polymerase chain reaction (PCR). Statistical analyses were conducted using Medcalc software, with p < 0.05 considered significant.</p><p><strong>Results: </strong>Of the 77 patients, most had classical papillary thyroid carcinoma, while others represented various subtypes. No mutations were found in BRAF K601E/V600_K601, KRAS G12x-G13D, or NRAS G12-G13; however, NRAS Q61x was found in one patient, HRAS Q61x in 12, and BRAFV600E/Ec in 36. A significant relationship was observed between HRAS Q61x mutation and disease response, alongside a significant association between gene mutations and iodine-refractory disease development (p = 0.0004). A ROC curve analysis indicated a 49.2 ng/mL threshold for Tg with 75% sensitivity and 94.1% specificity.</p><p><strong>Conclusion: </strong>The findings suggest that the HRAS Q61x gene mutation is significantly associated with iodine-resistant disease. The study sought to highlight differences in surgical approaches and their impact on patient outcomes.</p><p><strong>Materials and methods: </strong>A total of 107 patients were included, with 82 undergoing sternotomy and 25 undergoing thoracotomy. Preoperative right ventricular function parameters (mean pulmonary artery pressure, tissue Doppler imaging, and tricuspid annular plane systolic excursion), valve sizes, and other clinical data were recorded. Statistical analyses were performed to assess differences in outcomes between the two surgical groups.</p><p><strong>Results: </strong>Reoperation rates were significantly higher in the sternotomy group than in the thoracotomy group. Additionally, there were notable differences in valve sizes between the groups, with larger valves being used in sternotomy cases. However, mortality rates did not differ significantly between the groups.</p><p><strong>Conclusion: </strong>This study suggests that while sternotomy may lead to higher reoperation rates, the two surgical approaches yield comparable mortality outcomes. The primary aim of our study was to identify the most accurate method for calculating adenoma volume. Our secondary aim was to determine the relationship between adenoma volume and preoperative biochemical parameters (parathyroid hormone [PTH] and calcium levels).</p><p><strong>Material and methods: </strong>The medical histories of 75 patients who underwent surgery for a single parathyroid adenoma were prospectively collected. After excision, the adenomas were placed in water-filled syringes, and their actual volume was determined according to the amount of water displaced. The radiological volume of the adenomas was calculated using the ellipsoid body formula according to the ultrasonographic dimensions. The PTH and calcium levels of the patients were retrieved from the patients' hospital records.</p><p><strong>Results: </strong>The calculated radiological volumes correlated with the actual adenoma volumes, but there were significant differences between the two measurements. A new formula was developed based on actual volumes and radiological volumes. A positive correlation was found between the adenoma volumes and the preoperative PTH and calcium levels.</p><p><strong>Conclusion: </strong>Adenoma volume calculated using a mathematical formula based on ultrasound measurements does not accurately reflect the actual volume. They can contaminate solutions used for patient care and cause hospital outbreaks due to contaminated solutions. The aim of this study was to investigate the source and clonal relationship of <i>Ralstonia insidiosa</i> bacteremia detected in 28 patients between August and December 2021, as part of an unusual outbreak.</p><p><strong>Materials and methods: </strong>Active prospective surveillance studies were conducted, and environmental samples, including saline, antiseptic, and antibiotic solutions, injectors, arterial blood gas syringes, tap water, and hand soap, were collected from wards to determine the source of the outbreak. An arbitrary-primed polymerase chain reaction (AP-PCR) genotyping method was used to determine the clonal relationship between the isolates.</p><p><strong>Results: </strong>All the samples were cultured, and <i>R. insidiosa</i> was isolated from arterial blood gas syringes with the same location and time-based identifier (LOT number). All the arterial blood gas syringes were recalled from the hospital departments and sent back to the manufacturer. The outbreak was reported to the national health authorities. Clonal analysis between isolates from the patients and the blood gas syringes was performed using AP-PCR. Following surgery, she exhibited high levels of parathyroid hormone along with normalized calcium levels and persistent fatigue. However, recent advancements have come into use, so studies have been done to compare these new techniques to the gold standard TURP technique. The aim of this study was to compare the results of TURP, holmium laser enucleation of the prostate (HoLEP), and radiofrequency ablation (RFA) in men.</p><p><strong>Materials and methods: </strong>The patients who had the TURP procedure were defined as group 1 (G1), those who had HoLEP were in group 2 (G2), and those who had RFA were in group 3 (G3). Preoperative and postoperative results were compared, with postoperative checks done at 1, 12, 24, and 36 months.</p><p><strong>Results: </strong>There were 41, 40, and 40 patients in G1, G2, and G3, respectively. The mean ages for G1, G2, and G3 were 68.21 ± 8.19, 65.44 ± 10.48, and 77.32 ± 10.58, respectively. The decrease in international prostate symptom score (IPSS) was similar in G1 and G2 for all follow-up periods, but the decrease for G3 was smaller. The quality of life improvement can be summarized as G1 > G2 > G3 at the 36-month follow-up. Scores on the overactive bladder questionnaire (OAB-V8) decreased at the 1-month follow-up for G1 and G2, but then started to increase again over time. Scores on the international index of erectile function (IIEF-5) improved in a continuous fashion up to the 36<sup>th</sup> postoperative month for G1. The incontinence rate was highest in G2 for all time periods despite a decrease after the first postoperative month. The incontinence rate was the lowest for G3 across all time periods.</p><p><strong>Conclusions: </strong>The TURP and HoLEP procedures yielded equal improvement in IPSS, however HoLEP had a higher incontinence rate. RFA did not yield much improvement in IPSS, however it seems suitable for older patients with the lowest incontinence rates. Current treatment strategies are inadequate for addressing the complex nature of these injuries. Peripheral nerve healing and functional recovery are crucial components of both pathophysiology and therapeutic approaches. High fructose corn syrup (HFCS) is a sweetener frequently used in several beverages and foods. It is associated with several metabolic disturbances including insulin resistance and may impair nerve healing. This study investigated the therapeutic role of ivermectin on nerve regeneration following sciatic nerve injury and evaluated motor and sensorial functions together with histopathological evaluation. Additionally, we aimed to compare nerve healing between animals that consume HFCS and those that do not.</p><p><strong>Materials and methods: </strong>Forty-eight male Swiss albino mice were randomly divided into six groups, with three consuming HFCS-42 (11% v/v) and the other three regular tap water for 8 weeks. On day 28, sciatic nerve injury (SNI) was caused in all groups. Ivermectin (1 mg/kg) or gabapentin (30 mg/kg) treatments were administered to selected groups. Body weight, blood glucose, motor function (rotarod, open field test), and thermal-mechanical sensorial functions were assessed weekly. Finally, insulin levels were measured and histopathological samples were taken.</p><p><strong>Results: </strong>Eight weeks of HFCS consumption impaired mechanical and thermal sensory functions and resulted in histopathologically poor nerve repair. Ivermectin resulted in improved sensorial and faster motor function recovery in the HFCS groups. Elevated plasma insulin levels/HOMA-IR values were diminished by ivermectin in the HFCS groups. In the ivermectin non-HFCS group, histopathology revealed accelerated healing and higher scores in total. Carotid bodies (CB), located at the bifurcation of the carotid artery, regulate blood pH. This study aimed to assess the changes in CB-related pH levels following degeneration induced by ligation of both common carotid arteries (CCA).</p><p><strong>Materials and methods: </strong>We included 23 adult male New Zealand rabbits in the study. The animals were divided into three groups: a control group (n = 5), a sham group (saline injection, n = 5), and a study group subjected to bilateral CCA ligation (n = 13). pH levels and cardiac rhythms were monitored before, during, and after the procedure at two-day intervals over three weeks. Additionally, we quantified both healthy and degenerated neuron densities in the CB.</p><p><strong>Results: </strong>Initial pH levels were as follows: control group (7.42 ± 0.039), sham (7.39 ± 0.059), and study group (7.35 ± 0 .12). Prior to euthanasia, pH levels were the following: control (7.40 ± 0.029), sham (7.42 ± 0.041), and study (7.39 ± 0.12). The density of degenerated neurons in the CB was as follows: control (11 ± 4/mm<sup>3</sup>), sham (394 ± 74/mm<sup>3</sup>), and study (1363 ± 347/mm<sup>3</sup>). Statistical analysis showed the following significant differences: control vs. sham (p < 0.001), sham vs. study (p < 0.0005), and control vs. study (p < 0.000001).</p><p><strong>Conclusions: </strong>Increased degenerated neuron density in CB leads to reduced blood pH following CCA ligation due to acute ischemia of CB. The genus <i>Cinnamomum</i> belongs to the family Lauraceae, which comprises approximately 250 different species worldwide. The most common species on the market are <i>Cinnamomum verum</i> J. S. Presl, <i>C. cassia</i> (L.) J. Presl, <i>C. burmannii</i> (Nees & T. Nees) Blume, and <i>C. loureiroi</i> Nees. Cinnamon and its byproducts have been used for many years due to their antidiabetic effect. In the current study, the major chemical content and in vitro antidiabetic activities of different commercially available cinnamon samples were evaluated to determine whether they are safe to use for health purposes.</p><p><strong>Materials and methods: </strong>Macroscopic analyses, chromatographic analyses, and enzyme inhibition assays on diabetes-related enzymes (α-amylase, α-glucosidase, and aldose reductase) were performed on seven different samples (cinnamon sticks, tea bags, and capsules).</p><p><strong>Results: </strong>The cinnamon samples inhibited diabetes-related enzymes. The aqueous and ethanolic extracts of different cinnamon species demonstrated 7.73-333.69 mg/g of trans-cinnamaldehyde, and up to 43.73 mg/g of coumarin. Decoction and ethanolic extracts of <i>C. cassia</i>, <i>C. burmannii</i>, and <i>C. loureiroi</i> cinnamon sticks were detected to contain high levels of coumarin, which could pose a health risk, according to European Food Safety Authority data. Although antidiabetic activity was observed in the ready-made samples purchased from a herbalist, trans-cinnamaldehyde or coumarin compounds were not detected in the high-performance liquid chromatography analysis.</p><p><strong>Conclusion: </strong>The source of the cinnamon spice is crucial for the utilization of cinnamon both in food and therapeutic purposes. This study compared the recurrence rates of patients operated on for differentiated thyroid carcinoma with concomitant parathyroid adenoma and those operated on for thyroid carcinoma without detected parathyroid adenoma.</p><p><strong>Materials and methods: </strong>This retrospective study examined 340 patients who underwent total thyroidectomy for thyroid carcinoma at our institution. We compared patients with concurrent parathyroid adenoma to those without, assessing clinical, laboratory, surgical, and histopathological findings and thyroid carcinoma recurrence over a 6-year postoperative period.</p><p><strong>Results: </strong>Among all patients, 53/340 (16%) were identified with parathyroid adenoma and were predominantly over the age of 50 years (p < 0.00). There was a significant difference between patients with and without parathyroid adenoma regarding the preoperative serum Ca level (11.21 ± 0.41 mg/dL versus 9.5 ± 0.36 mg/dL, respectively), preoperative serum P level (2.3 ± 0.3 mg/dL versus 3.7 ± 1.4 mg/dL, respectively), and preoperative serum PTH level (142 ± 89.3 pg/mL versus 53.7 ± 11.5 pg/mL, respectively) (p < 0.001 overall). Residual thyroid tissue was detected in 215/340 (63%) patients, and a significant difference existed between patients with parathyroid adenoma (20/53, 38%) and without it (195/287, 68%) (p < 0.00). No significant differences were observed in surgical and/or tumor characteristics between patients with and without parathyroid adenoma.</p><p><strong>Conclusion: </strong>Our findings suggest that parathyroid adenoma has no direct impact on the recurrence of differentiated thyroid carcinomas in the short term.
