{"title":"Some of the Effects of Aqueous Leaf Extract of Cnidoscolous aconitifolius (Euphorbiaceae) on the Morphology and Histology of the Kidney and Liver of Sprague Dawley Rat.","authors":"D. It has continued to be used as food, medicine and ornamental plant till date.Awide variety of claims have been made for its medicinal efficacy as a treatment for numerous ailments ranging from its ability to strengthen fingernails and darken gray hair to curing alcoholism, insomnia, gout, and scorpion stings. This study was to elucidate the effect of aqueous leaf extract of Cnidoscolus aconitifolius commonly used as food supplement on the kidney and liver of Sprague Dawley rats. Thirty-two Sprague Dawley rats of both sexes (with an average weight of 235g) were randomly assigned into one control group (n=8) designated as A, while the extract treated groups consists of 3 subgroups and were designated as B, C, and D. the animals in the extract treated groups B, C and D were administered 600mg/kg, 400mg/kg and 200mg/kg bwt of the extract respectively, while the control group received equal volume of phosphate buffered saline (PBS). The administration was done orally using an orogastric tube for seven days (7d). Twenty-four hours after the last administration, all the animals were sacrificed by cervical dislocation. Laparatomy was performed and the liver and kidney excised, trimmed free of fat, rinsed in cold phosphate buffered saline solution. The liver was quickly fixed in 10% formolsaline, while the kidney was fixed in Bouin's fluid for histological processing. The histological findings after Hematoxylin and Eosin (H&E) staining showed that the extract treated sections the Kidney and Liver has no compromising effect on the cellular integrity of the studied organs. There were no degenerative changes and no intercellular vacuolations in the renal cortex and liver parenchyma. These findings indicate that the administration of aqueous leaf extract of Cnidoscolus aconitifolius has no adverse effects on hepatic and renal morphology of the treated Sprague Dawley rats. This poses some challenges to the surgeon especially where facilities are lacking. There is scanty report of encounters in the management of this condition from our subregion. Hence, we retrospectively reviewed the presentation and outcome of management of hypospadias at a tertiary health facility in Nigeria between Jan 2002 and Oct. 2009.There were 61 children with hypospadias during the period of which 47 (77.1%) had urethroplasty and this comprised 11 proximal, 4 midshaft and 32 distal types. Their ages ranged between 2 months and 14 years, median= 18 months. Ten patients declined surgery and 4 are awaiting intervention because of age and size of penis. Co-morbidities were ambiguous genitalia 3, Down's syndrome 1, undescended testis 3 and 2 haemoglobin SS patients. Five patients presented with circumcised hypospadias. Various techniques of urethroplasty were done with improvised equipments due to resource limitation. There was no mortality but complication rate was 36.7% which included 9 post- urethroplasty fistulas and 2 with complete dehiscence that needed a secondary repair. There were 3 post operative morbidity out of 5 circumcised hypospadias (p=0.127). Length of hospital stay was between 1 and 18 days (median 7.8 days). In conclusion, there is need for public education about hypospadias presentations to prevent delay referral and undue circumcision. Adekunle","doi":"10.4314/TJHC.V18I1.64483","DOIUrl":"https://doi.org/10.4314/TJHC.V18I1.64483","url":null,"abstract":"This was a cross-sectional study carried out on 372 clients receiving antenatal care at a rural, suburban and urban centres, in order to assess the acceptance of caesarean delivery amongst them and the factors influencing their attitude. Caesarean section was acceptable to 65.7%. Many respondents will refuse the surgery, because they do not think abdominal delivery is natural or necessary. However, if indicated by their health caregiver to be necessary to save life, 78.3% would comply. Others would leave for a religious healing home, another hospital, a traditional birth attendant or be delivered at home. The cultural reasons why some women decline include feeling that caesarean delivery is due to “spiritual attacks”, retribution for maternal infidelity and failure of a woman to fulfil her reproductive function. Respondents were more likely to accept the surgery if they had heard of it before or had a previous caesarean delivery. They were less likely to accept it if they were Christians or attended the rural health centre. Age, parity and educational level had no influence. Only 4% of the respondents would request a caesarean section for non-medical reasons. Health education needs to be carried out in the community, as well as during antenatal classes, to address the cultural beliefs. Medical records of all the children who underwent surgery for undescended testis at University of Ilorin teaching Hospital from January 1998 to December 2007 were retrospectively reviewed for demographic information; level of descent; age at surgery; intra operative findings; type of procedure; associated anomalies and postoperative complications. Forty six children had forty-nine undescended testes. Median age at surgery was 44 months, range, 1 month to 15years. A total of 47 orchidopexies and 2 orchidectomies were done. Right testis was involved in 24 (52.2%), left 19 (41.3%) and bilateral 3 (6.5%). Eleven (23.9%) patients had surgery before 2years, 25 (54.3%) before 5years and 21 (45.7%) had surgery at 5 years and above. Thirty-four (69.4%) testes were palpable. Of thirty- seven patients with records of level of descent, seventeen (45.9%) testes were in external ring and 16 (43.2%) inguinal canal. Twenty two (44.9%) testes were macroscopically atrophic , 19(86.4%) of the 22 were in patients older than 2 years(P=0.035). Post operatively, the testes retracted in two patients and there were scrotal hematoma in two that resolved with non operative management. This study showed that there is delayed presentation and therefore surgery in this setting. In this regard, the determination of copper/ceruloplasmin ratio has been proposed as one of the better indicators of copper status in pregnancy. Since copper/ceruloplasmin ratio varies from region to region, it has been advocated that each region determines its own cut-off. We therefore determined copper/ceruloplasmin ratio during normal pregnancy in Osogbo, SouthWestern Nigeria. 150 consenting pregnant women attending the antenatal Clinic of Ladoke Akintola University of Technology Teaching Hospital in different stages of gestation were recruited. Another 35 non pregnant age matched mothers were recruited as controls. 5 mls of venous blood was obtained from subjects and controls for the determination of serum copper with the aid of atomic absorption spectrophotometer and serum ceruloplasmin determined with a method based on its p- Phenylene Diamine Oxidase Activity according to the colourimetric method of Sunderman and Nomoto Copper in the first, second and third trimesters of pregnancy increases steadily and significantly compared with non pregnant subjects (36.72±3.75,46.88±0.02 and 58.13±2.50vs14.43±4.50imol/l, p significantly elevated above non pregnant controls, 47.21±8.88imol/l vs. 14.43±4.50imol/l p third trimesters increased significantly from each trimester and were significantly elevated above that of c o n t r o l s ( 4 9 . 5 6 ± 5 . 1 4 , 7 1 . 7 7 ± 7 . 0 7 a n d 90.70±2.77mg/100ml vs. 35.62±2.55mg/100ml, p trimester is significantly higher than in second and third trimesters while copper/ceruloplasmin ratios during second and third trimesters were similar. However, copper/ceruloplasmin ratio during first, second and third trimesters are significantly higher than in controls, 0.074 imol/g, 0.065 imol/g and 0.064 imol/g vs. 0.041 imol/g. Average copper/ceruloplasmin ratio in the total pregnant population was significantly higher than in controls 0.067 imol/g vs. 0.041 imol/g. Copper/ceruloplasmin ratio is highest during the first trimester and gradually decreases as pregnancy advances, this decrease may imply a decrease in available free copper to the fetus. This study was designed to investigate the effect of Vitamin C on Dexamethasone induced low birth weight an experimental model using theAlbino rats. Pregnant Sprague Dawley rats were divided into four groups of six rats each; group A- vehicle only, group B- Vitamin C treated (100mg/kg maternal body weight), group C-Dexamethasone treated (bolus dose of 100 micro grams of Dexamethasone/kg maternal body weight on day 13 followed by a daily dose of 200 micro grams/kg maternal body weight from day 14 till delivery) and group D -Dexamethasone and Vitamin C treated (100mg/kg maternal body weight of Vitamin C from day 7 of pregnancy to term; and on day 13 of pregnancy, were injected with a bolus dose of 100 micro grams/kg maternal body weight of Dexamethasone and subsequently a daily dose of 200 micro grams/kg maternal body weight until term). The weight and size of the litters along with the gestation period and weight gain during the period of study were recorded. T h e r e s u l t s s h owe d t h a t t h o u g h Dexamethasone caused low birth weight, Vitamin C had no significant (p>0.05) effect on dexamethasone induced low birth weight, litter size or gestation period.
