A right coronary artery (RCA) originating from a left anterior descending artery (LAD) is a rare subtype of SCA. Only a few cases are described in published literature.\u0000Case presentation: We described this anomaly in a 55-year-old male who presented with angina pectoris. The anomalous RCA was suspected by conventional coronary angiogram and was confirmed by computed tomography (CT) coronary angiography. Using CT, we demonstrated the course of the abnormal vessel and its relation to the main vessel. We also detected the presence of plaque, which caused luminal stenosis of the proximal LAD, which may cause global ischaemia.\u0000Conclusion: We concluded that although conventional coronary angiography is an important diagnostic method, new non-invasive methods such as CT coronary angiography can be a better screening tool to detect and characterise coronary anomalies.\u0000 \u0000Electronic Supplementary Materials\u0000Supplementary Material 1: Coronary angiogram demonstrates anomalous RCA originating from LAD.\u0000Supplementary Material 2: CTA coronary shows anomalous RCA originates from the mid-portion of LAD. About 25–30% of patients are found to have metastases at diagnosis commonly to lung, liver and bones. The incidence of renal cell carcinoma metastasizing to the head and neck has been reported to range from 15-30%. Intranasal mass, or occasionally orbital mass maybe the presenting symptom of metastatic renal cell carcinoma to the nose and sinuses.\u0000Case presentation: We report a case of left RCC with large metastases to the frontonasal region producing head and neck symptoms before the primary lesion could be detected. Clinical presentations of metastatic RCC to the nasal and paranasal regions varies from recurrent epistaxis, nasal obstruction, facial pain, induration or even an orbital mass. In our case, although the patient had typical presentation of metastatic nasal tumour, the diagnosis of metastatic disease was not made.\u0000Conclusion: Patient presented with nasal and paranasal region tumour with no other systemic symptoms, presence of metastatic disease particularly from renal cell carcinoma should be included in diagnosis, as it is a slow growing tumour and the fact that nasal and paranasal areas are the most commonly affected site of metastatic RCC in the head and neck region.

Anticoagulant treatment has been shown to reduce mortality in COVID-19 cases and is routinely used. Spontaneous extraperitoneal hemorrhage (SEH), which includes retroperitoneal, iliopsoas or rectus sheath hematomas, is a known complication of anticoagulant use. Trans arterial embolization (TAE) is a safe and minimally invasive management option to control bleeding in SEH. We report 7 cases of SEH in COVID-19 patients admitted to Hospital Sungai Buloh. This case series highlights the occurrence of SEH in COVID-19 patients, its clinical and radiological manifestations and management pathways.\u0000Case presentation: All patients were on anticoagulants and presented with abdominal pain and/or swelling with sudden drop in hemoglobin. Computed tomography angiography (CTA) showed contrast extravasation indicative of active bleed. All patients proceeded with conventional transfemoral angiography with option of TAE. TAE was utilized in 6 out of 7 cases and was successful in achieving hemostasis with no procedure related complication.\u0000Conclusion: SEH should be suspected in COVID-19 patients on anticoagulants presenting with abdominal pain or drop in hemoglobin. Age at first diagnosis, presenting features, prior therapies, endovascular treatment, surgical treatment, and therapeutic outcomes were all recorded. A comparison of pre-and post-procedure pictures was made to assess the clinical outcomes for all patients.\u0000Results: Seven patients with complex high flow head and neck AVMs were jointly managed by performing plastic surgery and interventional radiology. There were male and female, with an average age of 24 years. Each patient underwent embolization followed by surgical resection within a 24-hour window of up to 72 hours. In six out of seven patients, the lesion was removed therapeutically, whereas the seventh required palliative resection. Only one patient, there was a minor wound healing difficulty able to be treated with dressings.\u0000Conclusion: The results revealed that embolization alone is not effective to reduce the adverse effects. Incomplete removal of AVM showed the same effect as incomplete removal of a tumour. It may not improve symptoms and may worsen the recurrence, thus multi-modality approach is beneficial. A multidisciplinary approach, aided by the interventional radiologist and enable plastic surgeons to remove the high-flow AVMs effectively.

More studies are needed to provide insight of MI care in rural settings in Malaysia.OBJECTIVE: This study aims to define characteristics of patients who experienced MI in rural setting and their outcomes. METHODS: We conducted an observational study involving secondary data collection through review of medical records of patients who presented to rural primary healthcare facilities before being referred to regional secondary and/or tertiary centres. We explored their outcomes including inpatient and 30-day cardiac-related mortality.RESULTS: Nine patients were included. Eight (88.9%) experienced STEMI. Eight (88.9%) were male. Median age was 53 (Q1:40.5–Q3:64.5) years. Median duration from symptoms onset to presentation was 2 (1.125 – 35.250) hours. Median time from presentation to aspirin delivery was 45 (12.5 – 86.0) minutes. The median door-to-needle time was 190 (163.0 – 212.5) minutes. Five (55.6%) received medical thrombolysis in secondary hospital prior to elective percutaneous coronary intervention (PCI). Inpatient mortality was 11.1% corresponding to 30-day mortality.CONCLUSIONS: MI is potentially under-diagnosed and under-treated in resource-limited rural settings where community awareness is low complicated by logistic challenges. Better resources with better equipped healthcare facilities are essential for prompt diagnosis and interventions for MI patients.

Neurological examination revealed reduced power of both lower limbs (4/5). The patient’s condition deteriorated, and he subsequently passed away. “Ivy sign” was first described in 1995, so called because of its resemblance to ‘ivy creeping on stone’(1) noted in post contrast MR images of approximately 70% of Moyamoya patients (2). It appears as continuous or discontinuous linear leptomeningeal enhancement due to accumulation of gadolinium in engorged pial vascular network formation that causes leptomeningeal collateral development (1, 2). It can also be depicted on FLAIR image since fluid signal is effectively suppressed in this sequence; appears as high signal intensities along the cortical sulci and subarachnoid spaces (3). Recognizing Ivy sign as part of Moyamoya disease will avoid misdiagnosis, confusion with other differential diagnosis and eliminate potential unnecessary investigations.
