The PCNS lymphoma makes up 1-7% of primary brain tumors. PCNS T-cell lymphoma accounts for about 2% of all PCNS lymphoma and almost 99% are seen in immunocompromised patients (Latta et al 2010, Batchelor et al 2003). Here, we report a case of PCNS T-cell lymphoma in an elderly patient who is immunocompetent. The radiological findings, histopathological studies, and reviews of the current literature of this rare condition are discussed.

MRI, on the other hand, is more sensitive in detecting infarct core. We present our early experience adopting MRI-first policy, using Putra Acute Stroke Protocol with 8-minute first 3 sequences, for hyperacute stroke.\u0000Methods: We retrospectively reviewed our early 6 months experience between June until Dec 2020. All hyperacute stroke cases who had MRI first and the door to needle (DTN) were evaluated.\u0000Results: There were total of 124 cases with 11 haemorrhagic stroke (8.9%) and 6 cases stroke mimics (4%). There were total of 105 cases of acute stroke, who had MRI first, where, 18 were thrombolysed (17.1%) while 8 cases had thrombectomy (7.6%). Fourteen were thrombolysed within 60 minutes (77.8%). DTN time range between 6 to 78 minutes with total accumulated time of 716 minutes, giving an average DTN of 42 minutes.\u0000Conclusion: Our experience shows there is no significant overall delay in DTN with higher percentage of thrombolysis rate. MRI-first policy, adopting Putra Acute Stroke Protocol, helps to achieve early and confident decision after the 8-minute scan. The stroke mimics and haemorrhagic stroke were excluded effectively. Most patients presented with acute upper gastrointestinal bleeding. CTA abdomen is usually performed to attain the diagnosis. However, AEF has invariable appearances on CT where mostly are non-specific and similar to perigraft infection (PGI). A case of secondary AEF following multiple series of aortic surgery, with CT manifestation of recurrent pseudo-aneurysm of aortic stump, is presented.\u0000Case Presentation: 65 years old man who had undergone series of aortic reconstructive surgery for infra-renal abdominal aortic aneurysm presented acutely for hematemesis and per rectal bleeding. Initial CTA abdomen revealed pseudoaneurysm of the distal aortic stump, which was successfully embolized with coils. However, the symptoms recurred and repeated CTA showed new pseudo-aneurysm but no typical findings of AEF. Emergency laparotomy confirmed AEF at duodenojejunal junction that was repaired with primary closure. Unfortunately, patient died 3 days later due to multiple post-operative complications.\u0000Conclusion: Presence of pseudo-aneurysm with background of perigraft infection should raise the suspicion of AEF, especially in patients with history of aortic intervention that presented with gastro-intestinal bleeding. We report such a case and discuss the etiology, diagnostic, and treatment challenges of such patients.\u0000Case Presentation: This is a 37-year-old gentleman with a history of myelomeningocele (MMC) repair at the age of 2 years who presented to us with worsening bilateral lower limb weakness and numbness. Clinical examination demonstrated affected motor and sensory functions from the level of L2. An MRI Lumbosacral revealed S3/S4 dysraphism with a tethered low-lying conus. A CT angiogram of the spine followed by a spinal angiogram confirmed the diagnosis of a SDAVF with feeders from the median sacral artery and other branches of the left internal iliac artery. He underwent angioembolization, following which we noted a residual SDAVF and hence underwent a difficult second-stage angioembolization to the right lateral sacral artery branch. The patient remained status quo on follow-up.\u0000Conclusion: SDAVFs occurring in spinal dysraphism requires a high index of suspicion to ensure the diagnosis is not missed or masked by co-existing tethered cord and dysraphism. The symptoms of BA occlusion can range from temporary weakness or paraesthesia to near-complete paralysis, leading to a high risk of morbidity and mortality. CT and MRI are the imaging modalities of choice for identifying and locating the infarct area, ruling out any other lesions, and detecting vascular abnormalities. The combination of thrombectomy and stenting has been shown to increase the safety and durability of angioplasty without causing noticeably negative side effects, and it may be utilized as the first line of treatment for BA stenosis or occlusion in acute stroke.

This study aims to develop an automated detection algorithm with higher sensitivity in ICH evaluation in comparison to the conventional method. This indirectly influences the patient’s prognosis by reducing the risk of delay or misdiagnosis.\u0000Methods: Selected 50 CT brain images with primary ICH were used for three different measurement approaches including the conventional Kothari method (Conventional), AI-based method (A.I.), and manually marking by the radiologist, which is the ground truth (G.T.). In the automated system, a convolutional neural network (CNN) is used to localize the ICH, followed by a thresholding technique to segment the ICH, and finally, the measurements are computed. The segmentation performance is measured using Dice similarity coefficient. The automated ICH measurements are compared against the ground truth (A.I. vs G.T.). Concurrently, the ICH measurements calculated using the conventional method are also compared against the ground truth (Conventional vs G.T). The t-test analysis is performed between the sum squared error (SSE) of ICH measurements from the automated-ground truth and the conventional-ground truth.\u0000Results: The mean volumetric Dice similarity coefficient for the automated segmentation algorithm when tested against the ground truth, is 0.859±0.135. The t-test analysis of the SSE between conventional-ground truth (median=5.45, SD=3.96) and automated-ground truth (median=0.73, SD=0.78) achieved p-value < 0.001 (p=5.10E-9).\u0000Conclusion: The automated AI-based algorithm significantly improved the ICH surface area measurement from the CT brain with higher accuracy and efficiency in comparison to the conventional method.
