Khare, Naresh Guguloth, Sankalp Jaiswal, Sarvesh Pandey, Ajay Shukla, Ajay Yadav, Devender Singh","doi":"10.4103/jbjd.jbjd_41_22","DOIUrl":"https://doi.org/10.4103/jbjd.jbjd_41_22","url":null,"abstract":"Background: Fragility fractures, which most typically afflict postmenopausal women’s wrists, hips, and spines, are frequently caused by osteoporosis. In comparison to other times and places, the incidence is approximately two times higher during perimenopause. The risk of future wrist fractures is markedly increased by a prior fracture. Many studies have emphasized the importance of screening as a preventative measure against future fractures in these people with osteoporosis. This study aimed to assess the utility of distal forearm dual-energy X-ray absorptiometry (DEXA) as a screening tool to assess the risk of development of primary osteoporotic fractures at the distal end radius (DER) in postmenopausal women. Objectives: The study’s objective was to determine whether DEXA at the distal end of the radius could be used as a screening technique to determine the fragility fractures. Materials and Methods: The method followed included a cross-sectional study conducted on 60 patients (30 in group A: postmenopausal women with DER fracture, and 30 in group B: postmenopausal women without fractures), who fulfilled the inclusion criteria. Results: All patients underwent DEXA scan at three sites (hip, spine, and distal radius). On comparison of T-score between Hip, Spine, DER in group A , T-score with value <−2.5 was 53.33% at Hip, 70% at Spine and 70 % at DER with Total of 64.44%. mean±S.D at Hip was −2.73±1.27, at Spine was −3.23±1.25 and at DER was −3.42±162. P-value 57 was 0.298, Hip vs Spine: 0.184 , Hip vs DER : 0.184, Spine vs DER : 1. On comparison of Tscore between Hip, Spine, DER in group B, T-score with value <−2.5 was 30% at Hip, 56.67% at Spine, 13.33% at DER with total of 33.33%. mean±SD was −2.09±0.9 at Hip, −2.54±1.32 at Spine and −1.64±0.78 at DER. P-value was 0.002,Hip vs Spine : 0.037, Hip vs DER: 0.209, spine vs DER: 0.0009. this supports above study and we can use DER DEXA Scan as a screening tool in postmenopausal women. There was a significant deterioration of T-score at DER and hip and spine in the fracture group compared with the control group (P < 0.001). Conclusion: Some postmenopausal women showed preferential bone loss at the distal region of the radius, which would make them more susceptible to fragility fractures there. In general, forearm DXA for the evaluation of local bone density may prove beneficial to screening people at risk for distal radial fractures and makes it easier to identify patients who need osteoporosis treatment early on.","PeriodicalId":232675,"journal":{"name":"Journal of Bone and Joint Diseases","volume":"50 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-05-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132142994","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

A 15-year-old patient presented with a complaint of habitual patellar dislocation. He was diagnosed with a case of medial patellofemoral ligament insufficiency; there was a history of direct trauma to the knee 6 years back. Clinical examination showed a critical angle of dislocation of the patella of 15 degrees of flexion. The patient was managed by modification in the standard Campbell approach with lateral slit grafting of vastus lateralis with medial patellofemoral ligament reconstruction fixed with an interference screw. The patient showed a significant improvement after surgery. Preoperatively stability was checked at an extreme range of motion the stability was satisfactory. In this case, there is no abnormality of knee extensor apparatus or predisposing bony factors; isolated medial patellofemoral ligament reconstruction seems effective in treating confirmed patellar instability with the standard Campbell approach (lateral slip grafting of vastus lateralis with medial patellofemoral ligament reconstruction fixed with interference screw) shows good result and good patient satisfaction.","PeriodicalId":232675,"journal":{"name":"Journal of Bone and Joint Diseases","volume":"33 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-05-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121743737","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In this case report, we have managed this deformity by mini-open approach and, thereby, compared it to other approaches. In this case, we have taken a patient who had chronic posterior heel pain for 1 year and was managed by conservative methods but was not satisfied; therefore, after thorough evaluation including radiographs and blood investigations, the decision of surgical intervention was taken to remove the posterosuperior bump on the calcaneum in by a mini-open lateral approach. He had a current American orthopedic foot and ankle score (AOFAS) score of 65 before the surgery. The AOFAS score improved from 65 points presurgery to 88 points at 1-week follow-up postsurgery and increased to 94 points at 2-week follow-up. The lateral approach to Haglund syndrome is better than other approaches whenever there is sparing of the Achilles tendon in terms of degeneration or split. As this approach is done by mini-open; there is a significant improvement in AOFAS points in a short duration; the method is cost-effective; and the patient is able to walk from the next day. Further, the Achilles tendon does not require splitting or postoperative immobilization.","PeriodicalId":232675,"journal":{"name":"Journal of Bone and Joint Diseases","volume":"86 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-05-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114376810","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Early surgery may include stabilization of fracture with pedicle screws and connecting rods, reduction of fracture- dislocations and decompression of spinal cord by doing laminectomy at appropriate site. The purpose of this study was to understand better the role of surgical decompression ie laminectomy in thoraco lumber traumatic paraplegia. Materials and Methods: A randomized study was conducted after approval by institutional ethics committee of the hospital. We included 40 patients with history of traumatic spinal cord injury attended emergency department of department of orthopaedic surgery of our institution. All patients were then divided in to two groups group A and group B. A standard surgical procedure of posterior pedicle screw fixation with connecting rod were done in all patients in both groups while group B patients also received surgical decompression by doing laminectomy. Results: 26 patients were included in group A and 14 patients in group B. At Post Op, ASIA A grade level was same while B grade was removed. No At 3 months, the slight shifting to improved grades was seen. No significant difference was found in proportion of various grades of ASIA between the groups (p=0.186). The same results were found at 6 month and 1 year as observed in 3 months. Conclusion: Early surgical decompression after a severe spinal cord injury increased neurological recovery. Their has been various controversies regarding the same . With this study we have tried to gather the results of operative and non- operative management. Method: We conducted a prospective study for a period of 1.5 years to compare operative and nonoperative techniques in the treatment of calcaneal fractures. Patients attending the casualty, outpatient department (OPD), and those who were admitted indoors of SVBP hospital associated with LLRM Medical College and were assigned into two groups based on the treatment given operative and nonoperative treatment from April 2021 to October 2022 and were regularly followed for a period of 1.5 years. Radiological evaluation and functional assessment were done at 6 months and 1 year. The Functional outcome measures were assessed using American Orthopaedic Foot and Ankle Society (AOFAS) scale and radiological parameters by means of Bohler’s and Gissane’s angles. Results: Forty patients with calcaneal fractures participated in our study. Twenty of them were operated, and 20 patients were treated conservatively with pop cast. Amongst operated cases, radiological Bohler’s angle was restored in 12 cases, and had good functional results at 1 year follow-up, and six cases showed fair results, whereas two patients had poor results (mean Bohler: 20.7; AOFAS: 70.8). Amongst the Twenty cases treated conservatively with pop cast had showed fair results (mean Bohler: 28.2; mean AOFAS: 69.3). Conclusion: The overall outcome of operated cases were better than non-operated cases. There is a proportionate relationship between the restoration of Bohler’s and Gissane’s angles and the functional outcome of the patients (SD operative:3.33; SD nonoperative: 1.69; t value: 8.453; P = 0.001). These findings have prompted a recent increase in surgical fixation of displaced midshaft clavicular fractures. Objective: In addition to open reduction and plate fixation, intramedullary pins, wires, or nails may also be used to treat clavicle fractures. In this study, we compare the clinical, radiological, and functional results of midshaft clavicular fractures managed with open reduction and plate fixation versus conservative therapy, as well as their overall complication rates. Materials and Methods: All skeletally mature patients presenting to outpatient department/inpatient department with clavicle fracture less than 3 weeks old and displacement/loss of cortical contact between fractured ends of more than 2 cm were included in the study. Patients were randomly allocated to either conservative or surgical management. For conservative management, strapping of the involved side and application of figure-of-eight bandage was done. The minimum follow-up requirement of all patients was 1 year. The clinical assessment involved the assessment of complications, grading of results according to Constant and Murley score as well as the Disabilities of the Arm, Shoulder, and Hand score (DASH score). A radiological assessment for the union of the fracture was also done. Results: In the conservative management group (n = 52), the mean age of the group was 36 years (range, 19–58 years), and the mean follow-up was 1.6 years (range, 1–2.2 years). At 1 year follow-up, the mean DASH score was 7.9 ± 2.1, and the mean Constant score was 90.2 ± 15. The overall complication rate was found to be 21/52 (40.3%) in the operative management group. In the operative management group (n = 50), the mean age of the group was 42 years (range, 24–57 years) and the mean follow-up was 1.3 years (range, 1.1–1.8 years). At 1-year follow-up, the mean DASH score was 10.8 ± 1.1, and the mean Constant score was 93.1 ± 13. The overall complication rate was found to be 14/50 (28%) in the operative management group. In comparison, the overall rate of complications was significantly lower in the operative management group compared with the conservative management group (P = 0.001). Conclusions: Conservative management of midshaft clavicular fractures was associated with similar clinical and radiological outcomes compared to operative management. However, the overall complication rate was lower for operatively managed patients.","PeriodicalId":232675,"journal":{"name":"Journal of Bone and Joint Diseases","volume":"34 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-05-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124716968","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
