The patient had undergone maxillary resection and postoperative recovery with an obturator; however, slurred speech, water leakage from the nose, and aesthetic issues persisted. The all-in-one-type prosthesis was designed to improve speech and aesthetics by connecting the obturator and facial prosthesis with a magnet and connecting the epithese to the glasses. The prosthesis was fabricated by a dental technician, who created a wax pattern of the epithese directly on the patient's face and used it to fabricate a custom-made epithelial prototype. The final prosthesis was fitted and adjusted for optimal position and comfort. The improvement in speech function was evaluated via formant analysis and nasalance evaluation and found to be satisfactory. In this case report, we employed the psychoacoustic parameter – sharpness - to compare the quality of consonants in a cleft palate patient while wearing the old prosthesis, the new prosthesis, and the new prosthesis following palatogram guided modification. The result suggests that sharpness can be successfully analyzed as a criterion to monitor consonants quality during prosthesis fabrication for cleft palate patients.
Feasibility and accuracy of using intraoral scanners to digitize maxillectomy\u0000 defects for prosthetic rehabilitation: A systematic review
M. Elbashti, P. Molinero-Mourelle, H. Aggarwal, A. Aswehlee, M. Schimmel, S. Abou-Ayash, T. Yamamori, K. Baba, Y. Sumita
Purpose: Few studies have focused on the feasibility and accuracy of intraoral\u0000 digital impressions for maxillectomy defects, especially for extensive soft tissue\u0000 defects. Using intraoral scanners alone might be feasible for producing maxillary\u0000 obturator prostheses, albeit with some limitations. It seems logical to investigate this\u0000 ambiguity. Therefore, this systematic review aimed to assessing the feasibility and\u0000 accuracy of using intraoral scanners (IOSs) to digitize maxillectomy defects. Materials\u0000 and Methods: PubMed, the Cochrane Oral Health Group Trials Register, and the Cochrane\u0000 Central Register of Controlled Trials were electronically searched, and five\u0000 prosthodontics journals were manually searched for English-language articles published\u0000 as of December 2020 that assessed the feasibility and accuracy of using intraoral\u0000 scanners to acquire digital impressions for maxillectomy defects. Results: Two in vitro\u0000 studies, three clinical studies, six clinical reports, and three techniques were\u0000 included (N=14). Aramany’s and Brown’s classifications were used to classify defects in\u0000 twelve and one articles, respectively; the remaining article did not specify defect\u0000 class. The 3M True definition IOS with Cone-beam computed tomography (CBCT), Computed\u0000 tomography (CT), and/or optical scanners were used in both in vitro studies, mainly to\u0000 evaluate accuracy. The Trios 3 scanner was used in nine studies as the main resource for\u0000 data acquisition (75.0%), whereas the Trophy 3DI, Lava, and Cerec Omnicam scanners were\u0000 used in three articles (25.0%). Four degrees of feasibility were identified: feasible\u0000 (14.3%), feasible with limitations (28.6%), feasible with CBCT or CT (35.7%), and\u0000 feasible with conventional impressions (21.4%). Accuracy was evaluated in four studies\u0000 but was not mentioned in ten studies. Conclusion: The results revealed a low level of\u0000 evidence for the feasibility and accuracy of using intraoral scanners to digitize\u0000 maxillectomy defects. Additional multicenter clinical studies are needed to evaluate the\u0000 feasibility and accuracy of digital workflow compared with the conventional\u0000 approach.
