{"title":"A man in his fifties with lymphoma, vomiting and diarrhoea.","authors":"Jakob Lillemoen Drivenes, Anders Lillemoen Drivenes, Randi Haukaas Bjerkreim, Istvan Pregun, Tor Jacob Eide, Otto Emil Nyquist","doi":"10.4045/tidsskr.24.0605","DOIUrl":"https://doi.org/10.4045/tidsskr.24.0605","url":null,"abstract":"<p><strong>Background: </strong>Diagnosing immunosuppressed patients who present with fever and gastrointestinal symptoms can be particularly challenging for clinicians.</p><p><strong>Case presentation: </strong>We report the case of a man in his fifties who developed severe enterocolitis, immunosuppression and protein loss following treatment with rituximab and bendamustine. His medical history included recurrent iridocyclitis and small lymphocytic lymphoma, with subsequent identification of necrobiotic xanthogranuloma, prompting the initiation of chemoimmunotherapy. The patient initially presented with fever, vomiting and diarrhoea. Despite receiving appropriate medical care, he experienced recurrent infections and significant gastrointestinal distress, necessitating an extended hospital stay that involved interdisciplinary collaboration. Initial investigations indicated cytomegalovirus (CMV) reactivation, which was initially misinterpreted as CMV colitis. However, further biopsies revealed no classic CMV lesions, and comprehensive gastroenterological and microbiological evaluations ruled out infectious causes. This led to a diagnosis of rituximab-induced enterocolitis, a rare adverse effect of the treatment, confirmed by re-examination of the biopsies. Despite attempts to manage the condition with corticosteroids and TNF inhibitors, the patient ultimately succumbed to complications, including severe infections and renal failure.</p><p><strong>Interpretation: </strong>This case underscores the critical need to recognise rituximab-induced enterocolitis, especially in patients presenting with unexplained gastrointestinal symptoms during therapy. The figures can help form a basis for evaluating rational use of medications in hospitals.</p><p><strong>Material and method: </strong>Purchasing data for anxiolytics, antidepressants, mood stabilisers and antipsychotics from the Hospital Pharmacies Drug Statistics for the period 2012-23 were used. The data are sourced from sales to specialist mental health clinics and for SUD treatment. Defined daily doses and the number of defined daily doses per 100 bed-days were used as units of measurement.</p><p><strong>Results: </strong>Use of anxiolytics increased by 127 % from 2012 to 2023, while decreasing by 24 % for antidepressants. The use of mood stabilisers and lithium decreased by 26 % and 16 %, respectively. Total use of antipsychotics increased by 29 %, while rising by 79 % for depot injections.</p><p><strong>Interpretation: </strong>Significant changes in medication use were observed during the reference period. Due to the potential for serious side effects, treatment must be closely monitored by measuring the International Normalised Ratio (INR). In some health trusts, Noklus (Norwegian Organization for Quality Improvement of Laboratory Examinations) offers training for patients in self-management of INR. The aim of this study was to explore Norwegian patients' experiences with self-management of INR and their motivations.</p><p><strong>Material and method: </strong>In 2022, three focus group interviews were conducted at Nordland Hospital in Bodø, with a total of 16 individuals who performed self-management of INR. The interviews were audio-recorded, transcribed and analysed using systematic text condensation.</p><p><strong>Results: </strong>Participants reported improved self-esteem and a sense of freedom after starting self-management of INR. Some expressed initial concerns about the increased responsibility for their own health, but they gradually developed a more relaxed attitude towards self-management. They also experienced reduced stress levels and gained increased knowledge about their own bodies. The condition appears to be more prevalent than previously estimated, when mild to moderate cases are taken into account. Most patients respond well to dietary adjustments, while a small proportion may require assistance or experience seizures or syncope. Hypoglycaemia is associated with both weight-dependent and weight-independent mechanisms. Key factors include improved insulin sensitivity and increased secretion of incretin hormones such as glucagon-like peptide 1 (GLP-1). The primary goal of treatment is to minimise the postprandial fluctuations in blood glucose and insulin levels. Although certain medications may help to alleviate symptoms, they are rarely used.
