It is commonly seen in poor-controlled diabetic patients. Therapeutic options are limited. Psoralen and ultraviolet A therapy (PUVA), methotrexate, and topical steroids have been used with some success. Pentoxifylline is a xanthine derivative used mainly in peripheral vascular diseases. It also has antifibrotic effects due to its collagen synthesis inhibition action. Case Presentation: In this paper, a trial of intralesional pentoxifylline treatment and magnetic resonance images of a male patient with scleredema diabeticorum are presented. Some subjective symptoms of the patient improved after this treatment, but, the thickness of the lesion did not change. In this study, we aimed to assess treatment responses to narrow-band ultraviolet B (NB-UVB) and psoralen plus ultraviolet A (PUVA) therapies in patients with MF and parapsoriasis in a comparative manner. Materials and Methods: We retrospectively reviewed patients with early-stage MF and parapsoriasis who underwent phototherapy in Phototherapy Unit of our clinic between 2008 and 2018. In all groups, data regarding phototherapy were recorded and evaluated statistically. Results: In this study, 127 patients including 46 patients with early-stage MF, 48 patients with small plaque psoriasis and 33 patients with large plaque psoriasis received phototherapy. PUVA therapy session rate was significantly higher in patients with MF while NB-UVB therapy rate was significantly higher in patients with SPP. Number and duration of sessions to achieve complete remission were significantly higher in patients with MF when compared to those with SPP while no significant correlation was found between MF and LPP. In addition, the number and duration of sessions to achieve complete remission were significantly higher in patients with LPP than those with SPP. {"title":"The Effect of Delivery Mode on the Female Sexual Function Index","authors":"S. We aimed to examine the effect of delivery mode on female sexual functions. Materials and methods: Female sexual function index scoring questionnaire (FSFI) was translated to Turkish and conducted on 184 volunteer sexually active women aged 18 - 49 years between June 2017 and January 2018 and their results were analyzed. Women included in the study were divided into three groups as only the vaginal delivery group, only cesarean delivery group, and both vaginal and cesarean delivery group. Women included in the study were divided into three groups 1 as only the vaginal delivery group, group 2 only cesarean delivery group, and group 3 both vaginal and cesarean delivery group. Results: Total FSFI scores were significantly higher in group 3. There was no significant difference between the vaginal delivery group and cesarean delivery group. Lubrication (5±0, p=0.005), orgasm (4.71±0.48, p=0.002), satisfaction (9.42±0.97, p=0.025) were significantly higher in group 3. There was no significant difference between total scores the group 1, group 2, and group 3. Conclusion: Sexual functions are affected by factors such as hormones, socioeconomic status, and psychological factors. Though SCT is largely a benign disorder, it can rarely present with complications such as splenic infarction. Case Presentation: Here, we report the case of a 27-year-old Muslim male developed acute left upper quadrant abdominal pain and constipation while visiting a hill station, Ooty (Tamil Nadu, India). The patient had a history of similar complaint while visiting the same hill station while he was a student and was hospitalized and treated as acute gastritis. He also gave a history of abdominal pain while traveling in the airplane to Qatar where he works. Conclusion: Our patient had multiple episodes of abdominal pain while flying and visiting hill station, but the diagnosis of SCT was not suspected probably due to the above reasons. {"title":"Comparison of Partial Internal Sphincterotomy and Partial Internal Sphincterotomy with Additional Advancement Flap Anoplasty in the Treatment of Chronic Anal Fissure","authors":"Y. The success rate and complication rate of both methods were compared concerning the literature. Materials and methods: We have retrospectively evaluated charts of patients who have undergone the operation for chronic anal fissure between January 2015 and July 2018. A total of 123 patients were separated into two groups as partial Lis+anoplasty (group-I, n:76) and only partial Lis (group-II, n:47) groups. Age, sex, follow up time, anal examination findings, pain, bleeding, constipation score, patient satisfaction score, VAS pain score during rest or stooling, and recurrence and continence rates of patients were recorded. Results: 61 male (49,59%) and 62 female (50,41%) total 123 patients were included. The mean age of the patients was 42,87±11,69 years. The duration of the operation was 28,50±11,61 min. The hospital stay was 1,50±0,70 days. The mean follow-up time was 24±10 months. In group-II patients (15.22%), the recurrence was higher than group I. Similarly, bleeding (10.87%), pain (13.04%), and discomfort (15.22%) were higher in group-II compared to group-I. Bleeding, constipation score, VAS stooling and resting scores were higher in group-II, while the satisfaction score was found higher in group-I. There was no difference between the two groups in terms of incontinence rate. The purpose of this study is to share our experiences with patients who underwent hypospadias repair surgery in a university hospital. Materials and Methods: After obtaining approval from the hospitals ethical review board, the files of 151 patients with hypospadias who underwent surgery between March 2013 and October 2018, were examined retrospectively. One hundred thirty-five patients (89%) had anterior, and 16 patients (10.6%) had posterior hypospadias. Results: The average age of patients was 4,2 years (1-14). While 30% of the patients with anterior hypospadias had cordee below 30 degrees, 100% of patients with posterior hypospadias had chordee. In terms of complications, 15(9,9%) patients had urethro-cutaneous fistula, 20 (13,2%) patients had meatal stenosis, 3 patients (1.9%) glans dehiscence, and 1 patient (0.6%) had ventral skin necrosis. Conclusion: Hypospadias surgery is still challenging due to being associated with certain complications. Statistical analyses were carried out using SPSS version 20.0. Screening for syphilis was performed by reverse sequence algorithm using chemiluminescent microparticle immunoassay (CMIA), Rapid Plasma Reagin and T.pallidum hemagglutination tests. Results: A total of 91 HIV-positive patients were included in the study. Of these patients, 15 (16.48%) had never been tested for syphilis, and the rate of the patients who have a proper follow-up for syphilis was only 36.17%. Nineteen patients were diagnosed with syphilis; the overall syphilis rate was 20.88%. The mean time to the diagnosis of syphilis was 9.89±13.5 months, ranging from 0 to 48 months. Of the 76 patients screened for syphilis, 19 (25%) had CMIA positivity with concurrent RPR positivity. Concurrent TP-HA testing was performed in 54 (71.05%) of the 76 patients and 17 (89.4%) of 19 patients with CMIA positivity, and all of these patients with CMIA positivity revealed positive TP-HA results. Conclusions: Syphilis co-infection rate in HIV-positive patients in our hospital was high. However, there are still deficiencies in the screening of syphilis, and it has been shown that a specific screening algorithm is not adopted by the clinicians who follow-up HIV-positive patients. Case Presentation: Herein, we aimed to report the Color Doppler Ultrasonography (CDUS) and Computerized Tomography (CT) findings of MALS in a 27-year-old man presenting with complaints of abdominal pain and slimming. Conclusion: We think that MALS should be considered in young patients with persistent abdominal pain who are admitted with chronic epigastric pain. {"title":"Impact of Refresher Training on Knowledge Regarding Tuberculosis among Health Workers of West Bengal","authors":"I. Peripheral health workers in rural areas come in contact with TB suspects regularly during their home visits. The success of the Revised National Tuberculosis Control Programme (RNTCP) depends largely upon the involvement of these workers particularly Auxiliary Nurse Midwives (ANMs). Materials and Methods: All the ANMs and Health Assistants attached to a block primary health center (BPHC) of West Bengal were called for a refresher training on tuberculosis and RNTCP. Medical Officer Tuberculosis Unit of the area impacted the training along with Senior Treatment Supervisor (STS) and Senior Tuberculosis Laboratory Supervisor (STLS) of the concerned Tuberculosis Unit (TU). The training was of one full day duration and conducted at the BPHC. The resource persons told them about the diagnosis and management of TB cases according to RNTCP guidelines. A pre-test questionnaire having 20 multiple choice and true/ false type questions were given to the participants. The same questionnaire was given to them after the training. The score of post-tests was compared with the pre-test questionnaire. Results: In total 34 health workers participated in the training. 44.1% of the participants were ANM and 38.2% were Health Assistants (Female). Rest were Health Assistants (Male). The mean (SD) duration of their service was 17.79 (1.8) years. 35.3% of them had a graduate degree. The mean (SD) post-test score was significantly better than the mean (SD) pre-test score, 16.53 (1.73) vs 9.76 (1.71). There was a significant improvement in score for 12 questions. Conclusion: With time the knowledge regarding TB and RNTCP is fading in health workers. Thus, the preoperative diagnosis of pelvic actinomycosis is quietly difficult to reach. However, the clinician should keep alert this clinical entity in the presence of pseudotumoral mesentery infiltration on CT scan, especially for women with a longstanding IUD.
