Objective: This study compares the knowledge, attitude and practices of referral among nurses and nurse assistants at the PHC centres in Enugu State, Nigeria Methodology: This was a descriptive cross sectional study. A multistage sampling method was used in selecting the study population. Survey instrument was a pre-tested, self-administered questionnaire. Results: A total of 65 PHC workers were studied. Eighteen (27.7%) and 47(72.2%) respectively were nurses and nurse assistants. There was no statistically significant difference in the mean ages of the nurses and nurse assistants. All the nurses, 18(100%) understood the meaning of referral, its advantages and methods of referral as compared to 38(80.9%) and 42(89.4%) of the nurse assistants respectively. All the nurses knew complicated pregnancy as a referable case compared to 28(59.6%) of the nurse assistants. The attitude of both the nurses and nurse assistants to referral practices were largely positive. Conclusion: The nurses and nurse assistants had good knowledge and positive attitude to referral practices. Their practices were hampered by lack of financial support, referral forms and poor feedback mechanism. Few studies which have investigated both risk factors and outcomes associated with these conditions contain conflicting reports. Aims and Objectives: This study aims at documenting the length of the umbilical cord in babies born in Ebonyi State University Teaching Hospital (EBSUTH), as well as any association between umbilical cord length and fetal sex, number of fetuses, presenting part, cord accidents and certain intrapartum complications. Methodology: Prospective study of umbilical cords length of deliveries conducted in the department of Obstetrics and Gynaecology of EBSUTH, Abakaliki, from 1st October, 2008 over a six months period was done. Analyses were done using EPI info. Results: A total of 588 consecutive deliveries met the inclusion criteria. The mean cord length was 57.87 +12.6 cm and ranged between 22cm and 124cm.The mean cord length for male babies were significantly more than the females (58.16cm vs 57.59 cm). Significant difference also existed in cord lengths for multiple deliveries and singletons. The mean umbilical cord length associated with loops was significantly higher than that of the study population 63.58cm vs 57.28cm. Shoulder dystocia, hand prolapse in a transverse lying fetus, abruptio placentae and prolonged second stage of labour had shorter cords while fetal distress, cord prolapse and mecunium stained liquor had longer cords when compared with the mean umbilical cord length of the study population. Conclusion: Umbilical cord length was positively associated with number of fetuses and sex of the babies, but not with FETAL presentations. Extremes of cord length could predispose to certain intrapartum complications. Ibekwe","doi":"10.4314/EBOMED.V9I2.71688","DOIUrl":"https://doi.org/10.4314/EBOMED.V9I2.71688","url":null,"abstract":"Context: As the glaring inequity in maternal death ratios becomes more apparent between countries in the Northern and Southern Hemispheres, health experts try to achieve a reduction in their own nations' pregnancy-related deaths. Perhaps no research holds as much potential to make a difference in the developing world as research conducted on the gynaecological indications of misoprostol. The media tend to portray that misoprostol is widely available among private sector pharmaceutical distributors in Nigeria, yet evidence is lacking to that effect. Objective: To examine the availability of misoprostol in two major Nigerian cities, in order to explore the supply side of misoprostol six months after the drug's registration for management of postpartum hemorrhage in Nigeria. Methods: This was a cross sectional descriptive study involving proprietors, pharmacists or vendors of pharmaceutical and patent medicine outlets in two Nigerian cosmopolitan cities, Lagos and Abuja. They were interviewed using a semi-structured questionnaire in May and June 2006 in Abuja and Lagos respectively. Result: Misoprostol was not well-known or widely available in either city. Out of nearly six hundred shops surveyed, only 18 (4%) reported misoprostol cost information, indicating they had stocked the drug in the past, and only 15 of these (3%) currently had misoprostol on their shelves. When pharmacists and drug sellers who stocked misoprostol were asked what it was recommended for, treatment or prevention of postpartum hemorrhage was not mentioned. Conclusion: Concerted efforts to disseminate scientific information and educate all levels of health care providers should accompany new policy and protocol changes to achieve maximum national impact. Objectives: To evaluate the success rate of IOL and its determinants at the Ebonyi State University Teaching Hospital (EBSUTH), Abakaliki. Methods: A retrospective study involving 251 parturients who had induction of labour over a five-year period. Results: IOL accounted for 2.9% of the 8,676 deliveries during the study period. The overall success rate for labour induction in EBSUTH was 83.1%. Multiparous women achieved a success rate of 91.4%, grandmultipara 95.7%, and primigravida 58.2%. IOL using Misoprostol had a success rate of 83.1% and Oxytocin titration with synchronous fore water amniotomy 83.1%. Common indications for IOL included prolonged pregnancy (69.0%), and hypertensive disorders in pregnancy (15.7%). Factors influencing outcome included parity, booking status, gestational age, pre-induction cervical status and latent period interval. Conclusion: Induction of labour EBSUTH had a high success rate of 83.1%. Booking status, pre induction cervical Bishop's score ans parity amongst others significantly influenced the success rate in this study. The socio-demographic characteristics of patients seeking IVF-ET treatment in Nigeria have not been sufficiently documented. Objective: To describe the socio-demographic characteristics of patients seeking IVF-ET treatment in a public hospital in Nigeria. Methodology: A descriptive study of 187 partners who attended UBTH between April 2007 and October 2008. Outcome measure: Mean age, parity, duration of infertility, social class, indications for infertility. Results: The mean ages of the women and their husbands or male partners were 36.7 and 43.7 years respectively. 82.4% of the women were nullipara while 17.6 were multipara. Twelve point two percent had at least a living child. All the couples were in social class I-III. Tubal blockage was the commonest single indication for IVF-ET while ovulation disorder and male factor as single indications accounted for 12.2 and 8.1% respectively. Almost 17% of the women had multiple female factors while 39.9% had both male and female factors responsible for their infertility. Conclusion: Majority of patients seeking IVF-ET are advanced in age with longstanding infertility and have no children. The poor are not accessing IVF-ET yet. Aspects of the complications of mandibular fracture investigated are those emanating from the methods of treatment employed in its management. Such methods of treatment include conservative, closed reduction with intermaxillary fixation and open reduction with inter-maxillary fixation or rigid internal fixation. It is expected that with improvement in the knowledge of this condition and its management protocols complications will be reduced to the barest minimum. But, this is not so as there are many confounding variables that influence treatment outcome. This paper examined those complications associated with the various treatment methods and recommends that the older methods of treatment like closed reduction technique and transosseous wire osteosynthesis are still relevant in the current day practice.
