Hyun, Young-Jae Kim, Jung‐Wook Kim, Ki-Taeg Jang, Ji-Soo Song","doi":"10.5933/jkapd.2023.50.4.373","DOIUrl":"https://doi.org/10.5933/jkapd.2023.50.4.373","url":null,"abstract":"This study aims to confirm the average chronologic age according to the developmental stages of the mandibular canine (L3), first and second premolars (L4, L5), and second and third molars (L7, L8) in children and adolescents, and to confirm the developmental stage of L3, L4, L5, and L7, which can estimate the development of L8. A total of 1,956 digital panoramic radiographs of healthy individuals aged between 6 and 15 years who visited Seoul National University Dental Hospital from January 2019 to December 2020 were selected. The developmental stages of L3, L4, L5, L7, and L8 on both sides were evaluated using the dental maturity scoring system proposed by Demirjian and Goldstein. The average age at which the follicle of L8 was first observed was around 9.34 ± 1.35 years and varied from 6 to 12 years. The possibility of agenesis of L8 was high when no traces of L8 were observed after the following stages: L3, L4, and L5 at the developmental stage F and L7 at the developmental stage E; the age was about 10 years. In estimating the development of L8, when only one tooth was considered, estimation accuracy with L5 was the highest, and there was no significant difference when all four teeth were included. Risk factors were assessed for 125 children with radiographic data through a retrospective analysis of medical records and panoramic images. 68.0% of childhood cancer survivors exhibited at least one dental abnormality. The types of abnormalities varied depending on the age at cancer diagnosis and treatment intensity, ranging from microdontia (43.2%), to abnormal root development (39.2%) and tooth agenesis (33.6%). Logistic regression analysis demonstrated that a young age at diagnosis (under 3 years), the use of heavy metal agents, a history of hematopoietic stem cell transplantation (HSCT), and combination treatment of chemotherapy, radiation therapy, and HSCT were associated with a significantly higher risk for overall dental abnormalities. The increased risk ratios were 6.00, 3.06, 3.22, and 7.87, respectively (p < 0.05). This was performed by quantifying the durability of adhesion between the material and composite resin, measured by the shear bond strength (SBS). Five calcium silicate-based materials, TheraCal LC® (TLC), TheraCal PT® (TPT), TheraBase® (TB), Well-Root™ PT (WPT), and Endocem® MTA (EMTA), as well as two glass ionomer-based materials, Fuji II and Fuji II LC, were included. Specimens containing these materials were manufactured and bonded to composite resin with a universal adhesive applied in self-etch mode. The SBS values and failure modes were recorded, and the mean SBSs of the materials were compared. Both TPT and TB exhibited SBS values that were similar to TLC, while both WPT and EMTA appeared to have statistically lower SBS values. Mixed failure was commonly observed in TLC and TPT, while all WPT and EMTA samples showed cohesive failure. In comparison with TLC and TPT, more samples with cohesive failure were observed in TB, implying that this material forms a stronger bond with composite resin. Data for this study were obtained from the annual health screening statistical yearbook of the National Health Insurance Service. The investigation focused on the screening rate according to year, age, and region, as well as examination results from 2008 to 2021. The study revealed a general increase in screening rates over the years, with a simultaneous decrease observed as the age of the subjects increased. Regional variations in screening rates were observed, with the highest and lowest rates in Sejong and Jeonbuk, respectively. Analysis of examination results revealed an increasing proportion of subjects requiring treatment as they aged. From 2008 to 2013, less than 15% of subjects aged 18 - 24 and 18 - 29 months required treatment, whereas over 30% required treatment in the remaining age groups. Between 2014 and 2021, the proportion of subjects in the 18 - 29 month age group requiring treatment remained under 15%, but it exceeded 40% in the other age groups. Four types of artificial teeth were restored with composite resin using sectional matrix systems-Palodent V3 Sectional Matrix System and myJunior Kitand a circumferential matrix system-Tofflemire Matrix System-and modeled threedimensionally for analysis. When sectional matrix systems were used, there was a higher probability of concave proximal surface contour and simultaneously greater contact area and volume. This is attributed to the dead soft properties of the matrix band used in sectional matrix systems, which can lead to deformation of the band and hence an excessive amount of resin applied around the contact point. Additionally, the rubber wedge in the sectional matrix system may not help the matrix band fit into the cavity. Both patients presented with multiple missing teeth, conical canines, alveolar bone atrophy, and a skeletal Class III tendency. In the first case, a modified form of conical-crown-retained denture was used to cover the canines. This approach was carried out without tooth extraction or coping. In the second case of severe alveolar bone resorption and a distally tilted lower left canine, the tooth was restored with a hybrid ceramic crown. It was subsequently converted into a clasp-retained removable partial denture, utilizing a suction mechanism. Both patients are currently receiving regular check-ups for the maintenance of their prosthetic appliances and the evaluation of their growth patterns. The prevalence of delayed mandibular premolar eruption in this retrospective analysis, comprising 254 patients (aged 9 - 15 years), was 5.19%, with no significant difference based on gender. The mandibular second premolars were most affected (4.39%) compared to the first premolars (0.76%). No significant difference in prevalence was observed between the left and right sides. Among the treated mandibular premolars, primary molar-related lesions were identified as the leading cause (7.85%) of delayed tooth eruption. The treatment duration varied based on the Nolla stage, eruption stage, and treatment method. Teeth with Nolla stage 7 or lower had a treatment duration of 22.89 ± 11.96 months, whereas those with stage 8 or higher had a 15.02 ± 6.34 month duration. The deeper the tooth was located in the bone, the longer the treatment period became. The treatment duration varied depending on the treatment method, and statistically, there was no significant difference. The treatment durations for affected mandibular premolars increased with the depth of impaction angle of inclination. In this study, the treatment duration for delayed eruptions varied depending on the Nolla stage, eruption stage, and treatment method. The analysis was performed by collecting information about the number of visits, age, gender, diagnoses, and treatments for patients at the Pediatric Dentistry Department of Seoul National University Dental Hospital from January 2017 to December 2022. Compared with the period from 2017 to 2019, the total number of visits decreased during the COVID-19 pandemic, with a substantial decline in 2020. Patients in all age groups, except those aged 10 - 14, experienced decreased visits, and there was no significant gender difference. Z29.8 (need for other specified prophylactic measures) declined in all age groups, especially among those aged 0 - 4, except for those 20 years and older. K02.1 (caries of dentine) was sought by the age groups in the following order: 10 - 14, 15 - 19, 5 - 9, and 0 - 4 years, with the most significant decrease in the 0 - 4 years age group compared to others. K07.4 (malocclusion, unspecified) decreased in younger groups aged 9 and below but increased in older groups aged 10 and above. During the period from 2020 to 2022, preventive, restorative, pulp, and trauma treatments decreased, but surgical and orthodontic treatments increased compared with the previous three years. The use of sedation decreased and general anesthesia increased during COVID-19 compared to pre-pandemic. The analysis was conducted using treatment records from a random sample of approximately 1 million pediatric patients from a population that included all children and adolescents for each year from 2011 to 2020. In this study, the number of children diagnosed with K02 dental caries and the number of children receiving dental caries treatment across all ages have increased. However, the number of children aged 10 to 14 who received pulp treatment or extraction has decreased. In the National Survey of Children’s Oral Health, the decay-missing-filled teeth index for 5- and 12-year-olds has stagnated or increased slightly, but the percentage of the population with active dental caries has decreased. Accessibility and local environments for dental caries treatment have generally improved compared to the past, but preventive dental care has stagnated over the past decade. Third grade dental students at Jeonbuk National University (JBNU) participated for 2 years. They had clinical training for 3 weeks in pediatric dentistry at JBNU Dental Hospital. Then, 20 items of the modified form of Jefferson Scale of Empathy-Health Profession Students Version were done twice, before and after clinical training. The students who had lectures and role-playing were in the experimental group, while those who had no additional trainings were in the control group. Out of 69 students, 35 were in the experimental group and 34 were in the control group. Empathy level has increased after clinical training in both groups, but only the experimental group showed a significant increase (p = 0.0052). It also showed a significant increase in specific items (No. 6, 9, 10, 15, 17, 18, 19, 20) compared to the control group (p < 0.05). Clinical trainings such as lectures and role playing might have increased empathy levels related to standing in patient’s position.
