The evidence on the role of C-reactive protein (-CRP) in predicting mortality among people with COPD remains mixed. Data from the National Health and Nutrition Examination Survey (1999–2010) were linked with mortality files up to December 31st, 2019, from the National Death Index. Adults ages 20 years or older were included in the analytic sample to assess the relationship between physician-diagnosed self-reported COPD and mortality based on blood CRP levels. Multivariate complex samples Cox regression analyses were conducted to determine the hazards ratios (with 95% CI). A total of 30,669 individuals comprised of the final sample with 2215 deaths observed upon follow-up for an average of 10.2 years. Compared to individuals without COPD or elevated CRP, individuals with COPD but without elevated CRP (HR = 1.25, 95% CI = 0.97–1.61) did not have a significantly higher risk of mortality. However, those with both COPD and elevated CRP had a significantly higher risk of mortality (HR = 2.70, 95% CI = 1.08–6.81). Age, health insurance status, and a history of comorbidities modified these relationships. Biomarkers such as CRP continue to show promise for long-term COPD-related outcomes. Additional prospective assessments of individuals with airway obstruction are warranted to understand if CRP levels predict the occurrence or worsening of COPD and if those relate to long-term health outcomes.","PeriodicalId":22426,"journal":{"name":"The Egyptian Journal of Bronchology","volume":"43 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141197741","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Still, reports about surgical treatments remain limited. To investigate the role of functional endoscopic sinus surgery (FESS) with posterior nasal nerve/vidian neurectomy, as a surgical protocol in the management of chronic rhinosinusitis with nasal polyposis (CRSwNP), and its effect on bronchial asthma (BA) patients’ quality of life and pulmonary function tests. This study was a prospective observational study that involved 25 patients with BA and CRSwNP who underwent full-house FESS with bilateral posterior nasal nerve or vidian neurectomy in the Otorhinolaryngology-Head and Neck Surgery Department and were evaluated in the Chest Department, Zagazig University, from May 2022 to December 2023. All included patients were subjected to pre- and post-operative respiratory assessments including spirometry, Asthma Control Questionnaire (ACQ), and Asthma Quality of Life Questionnaire (AQLQ). Also, all patients were subjected to pre- and post-operative nasal assessment including visual analog scale (VAS), nasal endoscopic evaluation, and routine CT paranasal sinus. This study included 25 patients (11 men and 14 women; age range 18–57 years; mean ± SD of age 33.24 ± 11.3 years). There were statistically significant increases as regards forced vital capacity and forced expiratory volume in 1 s according to preoperative, 3 months, and 6 months postoperative values. As regards ACQ, AQLQ, and VAS scores, there were highly statistically significant improvements according to preoperative, 3 months, and 6 months postoperative follow-up scores. Asthma medication step-down was successful in 52% of patients after 6 months of follow-up. The quality of life, pulmonary function, and nasal symptoms of people with bronchial asthma combined with CRSwNP can both be alleviated after posterior nasal nerve/vidian neurectomy beside FESS.","PeriodicalId":22426,"journal":{"name":"The Egyptian Journal of Bronchology","volume":"2012 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-05-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141197566","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

CT may be more sensitive than RT-PCR for diagnosing COVID-19-related pneumonia. This study assesses the accuracy of multi-slice computed tomography (MSCT) grading in predicting COVID-19 mortality. COVID-19 RT-PCR. For severity scores, all patients’ clinical examinations, history, and chest MSCT data were collected. According to the chest MSCT score, 102 (51.5%), 70 (35%), and 28 (14%) patients had mild, moderate, and severe illness. Out of the patients, 62 (31%) died, and 69% survived. Patients with severe MSCT scores showed a considerably greater mean age than other groups (P < 0.001). Moreover, this group had a considerably higher mean BMI (P < 0.001), and a majority (57.1%) were obese (P < 0.001). Compared to the mild group, the moderate and severe groups had significantly increased rates of diabetes, hypertension, and liver disease (P < 0.001). The moderate group had a greater rate of no comorbidities (P < 0.001). A severe MSCT score was linked to increased leucocytes, C-reactive protein, ESR, ferritin, d-dimer, HbA1c, and fasting blood sugar, as well as decreased mean lymphocytes (P < 0.001). Severe MSCT scores were linked to increased ICU admissions (P < 0.001) and increased demand for advanced mechanical ventilation and oxygen assistance (P < 0.001). A severe MSCT score was associated with the highest death rate, followed by a moderate MSCT score. Low mortality rates were observed in mild MSCT-scored patients (P < 0.001). It is an inflammation of the lung tissue, provoked by immune mechanisms, which happens to prone individuals as a reaction to a wide range of antigens. There are different degrees of fibrosis and inflammation. A group of extracellular mediators both proinflammatory and profibrotic claimed to be involved in the pathogenesis of HP. Among these mediators, a significant role is played by transforming growth factor-beta (TGF-β). Correlation between the severity of hypersensitivity pneumonitis and the serum level of TGF beta. Sixty subjects were included in the study who were classified into 30 patients newly diagnosed with hypersensitivity pneumonitis and 30 healthy subjects served as controls. All the participants were subjected to complete history taking, physical examination, spirometry, 6-min walk distance test, HRCT, and serum levels of TGF-β. The serum level of TGF beta is elevated in newly diagnosed HP cases (fibrotic and non-fibrotic) in relation to control participants showing statistical significance p value < 0.001, and the serum level of TGF beta in the fibrotic group of HP patients is more than that in non-fibrotic group with statistical significance p value 0.012. The serum level of transforming growth factor can be used in the assessment of the severity of hypersensitivity pneumonitis as regards the intensity of lung parenchymal changes.","PeriodicalId":22426,"journal":{"name":"The Egyptian Journal of Bronchology","volume":"106 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-05-11","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140932932","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The sonographic examination of diaphragmatic dynamics in ICU patients by measuring diaphragmatic thickness and excursion in connection to various modalities of mechanical ventilation (MV) and patient outcomes was the objective of this study. This prospective observational study was carried out on 50 patients in respiratory ICU in Kafr Elsheikh and Benha University Hospitals. Patients were classified into 2 equal groups: COVID-19 group and non-COVID group. All patients underwent ultrasound assessment included the diaphragm thickness fraction and excursion in ICU patients on admission and on weaning. Successful weaning (SW) was higher in group I compared to group II. In group I (COVID), diaphragm excursion, thickness at end inspiration and at end expiration in NIV at weaning were significantly higher in patients with SW but thickness at end expiration on admission was significantly lower. In group II (non-COVID) MV patients, excursion at weaning was significantly higher in patients with SW, also were thickness at end inspiration and end expiration on admission, thickness at end inspiration and end expiration at weaning in NIV patients but thickness at end inspiration in MV on admission was significantly lower. Weaning success and mortality were significantly predicted by excursion in NIV at weaning, thickness at end inspiration in MV at weaning, thickness at end inspiration in NIV at weaning, and thickness at end expiration in MV at weaning.","PeriodicalId":22426,"journal":{"name":"The Egyptian Journal of Bronchology","volume":"151 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-05-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140889173","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The persistence of manifestations had resulted in classifying COVID-19 infection into bipolar phase which is a post-COVID-19 subacute phase by which the ongoing symptoms extended for 4–12 weeks and chronic or long COVID-19 phase by which the associated symptoms and abnormalities were extending for more than 12 weeks even after the pandemic. Determining the percentage of patients who continued to suffer from different symptoms beyond 12 weeks of COVID-19 exposure and the use of different complementary modalities among patients with post-COVID. The present study is a cross-sectional analytical observational study which was conducted through an online questionnaire using Google Forms with a total sample of 123 Egyptian patients older than 30 years old diagnosed with symptomatic and/or laboratory-confirmed COVID-19 infection with persistent health problems over 12 weeks prior to December 2021. The findings revealed elevated percentages of persistent symptoms and unfavorable long-term consequences extending beyond 12 weeks post-infection, irrespective of vaccination status or hospitalization. These outcomes significantly affected individuals’ health, finances, family dynamics, and lifestyle. Additionally, a notable proportion of cases sought relief through complementary medicine, particularly probiotics and herbal remedies.
