Residual risks mean that the risk could not be reduced by design effort or protection measures, which should be prevented from happening by the corporations between users and designers. In patient safety management fields, safety information is not sufficiently taught to medical workers due to lack of “triage system” which can rank the extent of residual risks in safety information in order to select appropriate proactive solutions, i.e., safety lecture for workers or information sending etc. Authors proposed risk triage sheet for medical equipments for the purpose of pre-service safety assessment. The developed sheet contains hazard check, risk evaluations and selecting safety measures. Application cases for 11 types of equipments revealed the validity of the developed triage sheet because the rank is distributed uniformly according to the types of equipment; rank I 2 types, rank II 2types, rank III 5 types and rank IV 3types.
