Procurement and implementation of process improving information and communication technology (ICT) is traditionally sought to be based on financial estimates. These are often difficult to produce and may be misleading. There is a need for a more pragmatic managerial solution. This paper suggests an alternative approach, based on the theory of constraints (TOC), arguing that the primary focus of technology assessment should be on technologies’ ability to break or alleviate organizational constraints, in order to increase throughput rather than reduce cost. Ultimately the latter will become as a consequence of the former. This paper presents a conceptual contribution. Countries of all over the world are implementing new strategies, methodologies and technologies to better serve the millions of patients, who demand better medical attention. The current system, which is archaic and not very systematic, widens the gap even more than the quality of medical services that should be provided to the millions of diabetic patients. Diabetic patients go through several difficulties with their medical treatment such as lack of motivation, difficulty of understanding, real or functional analphabetism, serious economic problems, high levels of depression, and self-care of diabetes. In the same manner, the medical relationship among patient, sanitary personnel, relatives and society is extremely affected by the little or no intention to establish follow-up parameters, prevention and self-care among the aforementioned actors. The Holistic, Interactive and Persuasive model to facilitate self-care of patients with diabetes (hiPAPD) is a model of Information and Communications Technologies centered on the diabetic patient. The development of online health applications is moving quickly, and positive results have been shown. Yet solid research on use and acceptance of different information, communication and participation tools by patients is still lacking. In this qualitative study, we examined the health-related internet use of patients with rheumatic diseases, their motives for using or not using certain applications, and their needs and preferences with regard to a rheumatology IHCA. Method Semi-structured individual interviews were conducted with eighteen patients, who were selected from a hospital’s patient panel. Participants were diagnosed with eight different forms of rheumatism and their mean age was 50.7 years. The interviews were coded by two independent researchers. Results The applications most preferred by participants were: information provision on both medical and support topics, online communication with their doctor and insight in their personal health records. Patient support groups were less valued as were participation tools such as symptom monitoring and online exercise programs. Furthermore, a large discrepancy was shown between patients’ current use and their future preferences with respect to information about care and support, access to personal health records and having online contact with their doctor. Conclusion Patients see great value in an IHCA provided by their own hospital, since it could increase reliability, and would give them the confidence to use the application. The current study shows a significant discrepancy between current use and future preferences rheumatism patients have regarding online communication with their doctor, online symptom monitoring and insight in their personal health record. Overall a rheumatology IHCA should contain communication and participation tools, both linked to the hospital, and information about disease, care and practical support. Among these, we find the health communities for people who suffer from lifestyle problems. These communities aim at helping people change their habits, and thereby preventing them from getting severe illnesses later. The communities are used for conversations about everyday struggles for a healthier lifestyle, and they contain issues concerning treatments, exercises, motivation, setbacks, etc. The question is in what ways and to what extent these communities support self-management among the members. This paper presents the results from a survey to members of a web community on overweight and another one on smoking. The results showed the members’ appreciation for existing conversational topics on facts and motivation. However, it is unclear whether they are effective in preventing health problems like overweight. The aim of this study was to investigate usage and effectiveness of a web-based application to increase healthy behaviour in adults with a healthy weight or slight overweight (BMI 18-28 kg/m2) by means of a Randomized Controlled Trial. A total of 297 respondents were randomly assigned to the intervention (n=147) or the waiting list group (n=150). The intervention group received access to the intervention for 12 weeks. At pre- and post-test we measured dietary and physical activity behaviour (primary outcomes) and BMI, knowledge, attitude, self-efficacy, subjective behaviour and insight in behaviour (secondary outcomes). All participants, regardless of randomized group, show improvement in healthy behaviour and subjective assessment of healthy behaviour. People who are older, score higher on dietary behaviour and under-estimate their dietary behaviour are more likely to use the Healthy Weight Assistant (HWA). Using the HWA leads to improvement in physical activity behaviour and insight in this behaviour. The aim of the pilot project was to investigate the weight loss efficacy and the cost of individual dietetic internet-based consultations in a Danish medical centre in combination with an internet community. A total of 46 obese patients in general practice were offered participation in a cohort study during May 15th to December 1st 2008. Patients from three different health centers were included. 32 patients gave informed consent to participate and were given access to weekly e-mail consultations with a dietician and access to an advanced internet community. The weight was objectively decided by inclusion and conclusion of the study. 22 (17 women and 5 men) of the 32 included patients completed the study. The average age was 41 years, the weight was 103 kg and the BMI was 36.7 kg/m2.
