Although DNA is not the only biomolecule that suffers damage, its importance lies in the fact that it is the only biomolecule that cannot be replaced by the cell, so when it suffers damage, the cell must repair it, tolerate or, in a extreme case, activate pathways that will lead to death, since the objective is to maintain cell integrity and the homeostasis of the organism.There are thousands of agents that can damage DNA, some are produced by the cell and are called 'endogenous, while others are external agents and are known as 'exogenous. The cell cannot avoid the damage caused by endogenous agents, since they are products of its metabolic activity, for example, so when they occur, cellular mechanisms are immediately activated to mitigate them. The same happens with the damage caused by exogenous agents, since the cell will do everything possible to diminish the adverse effects they can cause. The problem becomes apparent when the cell is unable to repair the damage or poorly repairs it, or repairs so much that the mechanisms are overwhelmed, when the damage remains in the DNA and a state of chromosomal instability is generated that can lead the cell to dysfunction and malignization. This state of chromosomal instability can be reflected in increased DNA breaks or micronuclei in exposed cells, which can be quantified by special methods such as the 'Comet Assay' and the 'Micronucleus Assay'. Since identifying DNA damage is a way of evaluating the toxic potential of the agents to which populations are exposed, it allows us to know their mechanisms of action and helps to understand the factors that influence the detriment in population's health. Where it has been seen that there is a prothrombotic state with involvement of the Central Nervous, with involvement of large vessels such as the middle cerebral artery, is due to mechanisms induced by the infection itself, hypercoagulable state and endothelial damage. Neurological manifestations in COVID-19 are found in 36% of patients. Case description: This is a 36-year-old male patient with fever, cough and general malaise. A COVID test was performed, which came out positive. His condition was getting worse adding photophobia, right hemiparesis and deviation of the corner of the mouth to the left, which is why he went to the doctor, arriving 8 hours after the onset of the clinical picture, where a simple skull tomography was performed, showing extensive infarction of the left middle cerebral artery with malignant cerebral edema. He was transferred to emergency surgical management where a left decompressive craniectomy was performed. After this, mechanical respiratory assistance with intubation and anti-cerebral edema measures were maintained, achieving ventilatory progression; however, a tracheostomy and gastrostomy were performed due to poor predictors of extubation. He was kept under post-surgical observation, leaving him with 3/5 right hemiparesis, without any other deficit, therefore, he was discharged home. Discussion: The case presented was managed with decompressive craniectomy, resulting in an improvement in survival, as reported in the literature where it is recommended that such management should be performed early. Conclusions: This report reveals that patients with COVID-19 present in young men in the fourth and fifth decade of life, without comorbidities, that recieved early treatment with decompressive hemicraniectomy, improved their life prognosis, consistent with the cases presented in the literature. Avoidance of exposure to oxidizing agents is important for patients with congenital methemoglobinemia because of their deficient enzymatic pathways and decreased oxygen-carrying capacity. Here, we present a pediatric patient with preoperatively undiagnosed congenital methemoglobinemia who underwent catheterization with probably diagnosis of pulmonary hypertension under general anesthesia. The pediatric patient was a 10-year-old who displayed a low pulse oximetry reading of 92% prior to induction of anesthesia. Methemoglobinemia was first suspected intraoperatively because of a mismatch of SpO 2 of finger pulse oximetry and SaO 2 of arterial blood and was later confirmed by multiple wavelength. CO-oximetry. The presentation of this case not only provides detailed information about the clinical evolution of the patient, but also serves as a basis to illustrate Key aspects of histopathological is and immunohistochemical implications in Castleman disease. Lo mismo hace que la obtención de edad ósea sea poco práctica. Programas y aplicaciones de inteligencia artificial han sido desarrollados con el objetivo de optimizar la lectura de edad ósea, uno de ellos es el software automatizado de BoneXpert (BE). Estudios científicos han determinado la eficacia de dicho método al ser comparado con el método convencional de Greulich y Pyle, lo que favorece su uso en la práctica clínica cotidiana. Objetivo: Correlacionar resultados de edad ósea obtenida mediante método de Greulich y Pyle versus software automatizado BoneXpert. Métodos: Estudio descriptivo, retrospectivo, realizado con radiografías de mano disponibles en la base de datos del Hospital Herrera Llerandi. Se utilizaron radiografías con informes de edad ósea disponibles de 2021 a 2022. Resultados: Se incluyeron 20 casos, 9 de sexo femenino (45%) y 11 de sexo masculino (55%), la media de edad cronológica fue de 8.98 años. La correlación entre la edad ósea por método convencional (GP) y la edad ósea mediante software automatizado (BE) fue de 0.978 sugiriendo una correlación alta. Se concluye que del software automatizado de BoneXpert posee una alta precisión diagnóstica al ser comparada con el método convencional de Greulich y Pyle, otorgándole la virtud de poder ser utilizada como herramienta diagnóstica.
