{"title":"TempTest®: un instrumento de precisión en las urticarias físicas.","authors":"Solange Rodrigues-Valle, Guilherme Azizi, Sérgio Duarte-Dortas","doi":"10.29262/ram.v68i1.827","DOIUrl":"https://doi.org/10.29262/ram.v68i1.827","url":null,"abstract":"<p><strong>Background: </strong>TempTest® is a new method based on the Peltier effect, which is validated for the diagnosis of patients with cold urticaria or heat urticaria, and it is able to measure the temperature variations and activity of these pathologies.</p><p><strong>Objective: </strong>To analyze the results of provocation tests by using this new method in patients with UFr and UCal, who were followed in a center of reference and excellence for urticaria (GA2LEN UCARE) in Rio de Janeiro.</p><p><strong>Methods: </strong>The medical records of 12 patients who had a history of UFr or UCal and were tested with TempTest® during December 2017 and February 2020 were analyzed. For temperature provocation tests, the patients were requested to place their forearm on the TempTest® thermal element for five minutes. A positive response was defined by the appearance of a 2-mm-papular lesion 10 minutes after the provocation test was started, as recommended.</p><p><strong>Results: </strong>10 of the patients were women and 2 of them were men, from 13 to 77 years of age (mean = 50.2 years). Among the twelve patients, three of them had heat urticaria and nine of them had cold urticaria. To this day, there are diagnostic algorithms and mobile applications that improve the clinical approach, as well as laboratory tests and more specialized procedures, such as skin tests and controlled exposure tests; which are useful for identifying the drug involved and for selecting safe and effective therapeutic alternatives. The first line of treatment are systemic corticosteroids; however, there are patients who are refractory to them, as well as to other immunosuppressants. Rituximab has been used as a successful alternative since 2000 with good results but without information on its behavior in the Mexican population.</p><p><strong>Objective: </strong>To assess the clinical response to treatment with rituximab in Mexican patients with pemphigus.</p><p><strong>Methods: </strong>This was a cross-sectional, retrospective study in a tertiary hospital, which included patients with moderate-severe pemphigus who had been treated with rituximab from 2007 to 2020.</p><p><strong>Results: </strong>Six medical records of patients diagnosed with pemphigus were obtained; four of them with Pm vulgaris, and two of them with pemphigus foliaceus; all patients had received systemic immunosuppressive therapy prior to rituximab. New methods for diagnosis and treatment have been described; accordingly, the international guidelines were renewed.</p><p><strong>Objective: </strong>To create a national platform for the development of updated guidelines, solidly based on evidence: Comprehensive Asthma Management (Spanish acronym: MIA).</p><p><strong>Methods: </strong>MIA uses the ADAPTE method. The MIA development group consists of experts in pulmonology-allergology-methodology and representatives of 13 institutions and societies of specialties that manage asthma. The international reference guidelines (selected with AGREE-II): GINA 2020, GEMA 5.0, BTS/SIGN 2019 and ATS/ERS consensus document 2014-2019 on severe asthma. MIA covers suspected asthma, diagnosis, treatment, and special groups. Key clinical questions were formulated on treatment steps 1-3, biomarkers and severe asthma.</p><p><strong>Results: </strong>Based on evidence, safety, cost and local reality, the core group developed responses. Through a Delphi process the broad MIA development group suggested adjustments until consensus was reached.</p><p><strong>Conclusion: </strong>A document was generated with multiple figures and algorithms, solidly based on evidence about asthma management, adjusted for Mexico with a broad base among different societies that participated in its development. A case report].","authors":"Fernando Steven Fuentes-Abreu, Juan Camilo Ardila-Herrera, María Raigosa","doi":"10.29262/ram.v68i1.855","DOIUrl":"https://doi.org/10.29262/ram.v68i1.855","url":null,"abstract":"<p><strong>Background: </strong>Warfarin is an oral anticoagulant which is widely used in the prevention and treatment of thromboembolic events, to which multiple adverse effects are attributed; nevertheless, descriptions of hypersensitivity reactions to this medication are rare.</p><p><strong>Case report: </strong>A 59-year-old patient with a history of mesenteric thrombosis, who had previously received long-term treatment with warfarin. Upon discontinuation of the medication, the patient experienced a recurrence of thrombosis at a hepatosplenic level, with a subsequent hypersensitivity reaction upon administration of the coumarin. In addition, to statistical data, there are estimators that can help clinicians transfer research findings to routine clinical practice. These estimators are measures of risk, association, and impact. Risk measures report current uncertainty or probability (prevalence of a disease, sensitivity, specificity) or for future events (cumulative incidence, incidence density). Measures of association are related to the identification of the risk in order to determine whether certain factors increase or decrease the probability of development of a disease (relative risk, odds ratio, hazard ratio). While measures of impact allow, among other things, to estimate the effect of a treatment (relative risk reduction, absolute risk reduction, number needed to treat). The patients were categorized and compared based on the presence or absence of obesity. In order to establish an association between variables, logistic regression analyses were performed.</p><p><strong>Results: </strong>In total, 111/188 of the patients had obesity; these patients were older than those without obesity (35.9 ± 15.3 years vs. 44.4 ± 13.8 years, p <0.001). No significant difference was observed in the groups regarding sex, total IgE serum concentration, peripheral blood eosinophil count, personal history of atopic diseases, frequency of allergic sensitization, hospitalization for asthma, and asthma control. Demographic data, severity of the AD (according to the Eczema Area Severity Index), clinical phenotype, total serum IgE, the presence of allergic and non-allergic comorbidities, as well as of anxiety and depression, were recorded by means of targeted questionnaire and Hamilton Scale. Descriptive and inferential statistics were obtained, considering a statistical significance associated with a value of p <0.05.\u0000\u0000\u0000RESULTS\u0000187 patients were included; the median age was 12 years with a range of 1-87 years. Differences were found regarding sex and severity (p < 0.05). Mild forms of AD were presented in 57.8% of the patients, moderate forms in 20.9%, and severe forms in 21.4%. The severity was associated with allergic comorbidities such as asthma (p = 0.001) and allergic conjunctivitis (p <0.001). During the last years, many studies and advances have been developed with the aim of better understanding the pathophysiology, as well as optimizing patient management. Several international working groups have attempted to clarify and standardize the care of HAE communicated as guidelines and consensus recommendations. We considered necessary to provide recommendations for the diagnosis and treatment of patients with HAE in Argentina.</p><p><strong>Methods: </strong>A group of specialists of allergy and immunology from Argentina by using the online surveys methodology as well as face to face meetings developed the intended consensus.</p><p><strong>Results: </strong>Recommendations were established based on published evidence and the expert opinion.
