However, there is still no surety as to when we will be finally free from the pandemic and mortality will always remain the most important factor of concern. Materials and methods: One hundred patients who died due to COVID-19 were sampled using systematic random sampling from our hospital, and demographic, clinical, blood, and radiological investigations data were collected retrospectively. An analysis of these parameters was then done using descriptive statistical methods to point out potential risk factors for mortality, which may help in formulating future hypotheses. Results: The mean age was 68.3 years, and 69% were more than 65 years of age. Sixty-one percent were males, suggesting a male preponderance. Diabetes and hypertension were the most common comorbidities. Oxygen saturation < 90%, respiratory rate > 24 / minute, heart rate > 90 / minute, impaired level of consciousness, National Early Warning Score (NEWS) > 12 seem to be important mortality predictors. Neutrophil/lymphocyte ratio of more than 5.2, lymphopenia, low platelet counts, low serum albumin, electrolyte disturbances, high creatinine levels, and high serum inflammatory biomarker levels all appear to contribute to mortality in COVID-19 patients. Conclusions: More studies need to be performed to test for the variety of hypotheses proposed in this study. Gupta","doi":"10.5005/jp-journals-10070-7053","DOIUrl":"https://doi.org/10.5005/jp-journals-10070-7053","url":null,"abstract":"Background: Individuals suffering from diabetes mellitus are at higher risk of all forms of sexual dysfunctions, but there is hardly any study in India that discussed all aspects of sexual activity in diabetic patients. Hence, the current study was conducted to estimate the prevalence of the sexual disorder in diabetic patients. Materials and methods: One hundred nine diabetic men were selected, who fulfilled the inclusion criteria and assessed by the International Index of Erectile Function-5 (IIEF-5) questionnaire. Result: The prevalence of erectile dysfunction (ED) was 89.9%, out of which 58.16% reported spontaneously with maximum (62.4%) having severe ED, andthe IIEF-5 score provided by patients themself and their spouses was statistically significant ( p -value = 0.000). The hypoactive sexual desire and decreased libido were 25.7% and 42.2%, respectively. A total of 68.8% of participants failed to attain orgasm, 56.9% of participants had premature ejaculation but 20.2% had delayed ejaculation, and 61.47% had all three of erection, ejaculation, and orgasm problems. Conclusion: This study concluded that nine out of ten diabetic men were suffering from ED and only interviewing the male sexual partner is sufficient for the evaluation of ED and also concluded that ED is not so much social stigma nowadays. Examination revealed an upper motor neuron (UMN) pattern of weakness in all four limbs, accompanied by a UMN type of bladder involvement in a shock stage. Two significant cervical lymph nodes were noted in the right submandibular region. MRI of the cervical spine revealed an extradural mass, compressing the cord at the C2 to C4 region. This case report is about a young girl who presented with insidious onset, a gradually progressive neurological illness characterized by bilateral sensorineural hearing loss, wasting, and weakness of all four limbs along with bulbar paralysis. On examination, she had involvement of lower cranial nerves, tongue fasciculation, generalized amyotrophy, and bipyramidal features. Electrophysiological studies showed features of chronic denervation. A provisional diagnosis of MMND was made. MMND resembles Brown–Vialetto–Van Laere (BVVL) syndrome and some of the other complex childhood motor neuron disease syndromes, like Boltshauser syndrome, Nathalie syndrome, and Fazio–Londe syndrome. Early diagnosis of BVVL, which is a riboflavin transporter deficiency, is essential, as it is fully responsive to high-dose riboflavin supplementation. In BVVL syndrome, a female predominance has been documented (1:5), while in MMND, an equal distribution is noted. In BVVL, at least half of the reported cases are familial whereas in MMND most of the cases are sporadic. In BVVL, lower motor neuron signs in the limbs are infrequently present and pyramidal signs are rare, whereas in MMND lower and upper motor neuron signs are seen in the majority of the patients. Third, fifth, or sixth cranial nerves are never noted to be affected in MMND, but they may rarely be involved in BVVL. Mukit, Faysal Ahmed","doi":"10.5005/jp-journals-10070-7049","DOIUrl":"https://doi.org/10.5005/jp-journals-10070-7049","url":null,"abstract":"Primary sclerosing cholangitis (PSC), a disease of unknown etiology, is diagnosed on the clinical and biochemical findings of cholestasis combined with the characteristic findings seen on cholangiography. Small duct cholangitis is a variant of the PSC spectrum with distinct clinical course and outcome. We present two cases of small duct PSC, which are presented with the clinical and biochemical features of cholestasis but without typical positive findings on cholangiography. A liver biopsy was attempted, which confirmed sclerosing cholangitis. Upon diagnosis, both the patients were prescribed ursodeoxycholic acid at a dose of 15 mg/kg body weight/day and liver biochemistry was followed up after 3 months, which revealed biochemical improvement. Being an uncommon variant of this “not so common” disease spectrum, small duct PSC often poses a diagnostic dilemma with its presentation due to the clinical and cholangiographic mismatch. Addressing this issue, the present study conducted in athletes vis-à-vis nonathletes assessed their anthropometric measures and pulmonary function parameters and explored for association, if any. Materials and methods: A cross-sectional study was undertaken in athletes vis-à-vis nonathletes. Respondents were screened for risk factors and were then assessed for their anthropometric measures including height, weight, body circumferences, fat level, skeletal muscle, and skinfold thickness. Pulmonary function test was then conducted in all respondents. Results: Mean anthropometric measures including body mass index, body fat percentage, waist circumference, waist–hip ratio, and skinfold thickness were on a slightly higher mark for nonathletes. Spirometric indices were on a bit higher grades for athletes’ group. However, consistent negative association was noted between increased anthropometric measures and pulmonary functions.
