In hyperthyroidism we observe hair loss, lower leg myxedema and onycholysis or, in the case of hormone deficiency, generalized swelling of the skin, which becomes cold and pale. Primary hyperparathyroidism is revealed by pruritus, presence of chronic urticaria or deposition of amorphous calcium salts. In hypoparathyroidism, the skin is dry while the nails become very brittle. Skin lesions in diabetes include necrobiosis lipoidica, granuloma annulare, scleroderma-like diabetic edema and acanthosis nigricans. Overactive pituitary gland is often manifested as acromegaly with hypertrophy of soft tissue thickening and hypertrichosis. The skin in the early stages of hypopituitarism feels swollen, is pale yellow and oily, and finally becomes alabaster and dry. The characteristic features of Cushing syndrome are central obesity, lunar face, buffalo hump, and striae. In Addison’s disease we observe hyperpigmentation. Lichen sclerosus (LS) is a chronic, inflammatory dermatosis of unknown etiology affecting skin and mucous membranes. It was first described in 1887 by Darier. Lichen sclerosus usually begins as a single or multiple plaques (rarely as lichenoid papules) gradually transforming into porcelain-white atrophic lesions. Depending on localization, it may manifest with itching, pain or a burning sensation and often may cause sexual dysfunction. Objective. Presentation of an LS case that posed diagnostic difficulties. Case report. We present a patient with clinical diagnosis of lichen planus hypertrophicus on the skin of forearms and hands which demonstrated histopathological features of LS. Additionally, the presence of LS lesions was found on the skin of the eyelid and penis. Conclusions. Our case seems to be interesting because of the differences between clinical and histopathological findings and multiple localization of lesions. Wieloogniskowa postać liszaja twardzinowego. Zapalenie opryszczkowate skóry (choroba Duhringa) jest pęcherzową podnaskórkową chorobą autoimmunologiczną. Charakteryzuje się występowaniem swędzących polimorficznych wykwitów skórnych, którym towarzyszy enteropatia glutenozależna, na ogół klinicznie bezobjawowa. Etiopatogeneza schorzenia wiąże się z obecnością przeciwciał przeciwko transglutaminazie tkankowej i naskórkowej. Podstawą rozpoznania jest badanie immunopatologiczne bezpośrednie. Przewlekłe stosowanie sulfonów skutecznie łagodzi objawy skórne. Cel pracy. Przedstawienie 3 przypadków choroby Duhringa oraz zwrócenie uwagi na trudności spotykane przy ocenie obrazu klinicznego, diagnostykę i leczenie, a także monitorowanie bezpieczeństwa terapii. Opis przypadków. Przypadek 1.: 37-letni pacjent. W wywiadzie pierwsze zmiany skórne od roku. Chory dotychczas leczony ogólnie preparatami przeciwhistaminowymi oraz miejscowymi glikokortykosteroidami, jednak bez poprawy. Rozpoznanie dermatitis herpetiformis ustalono na podstawie badania immunopatologicznego. Pacjent był leczony dietą bezglutenową i dapsonem w dawce 100 mg/dobę; uzyskano poprawę. Przypadek 2.: 62-letni pacjent, u którego pierwsze swędzące grudki pojawiły się w październiku 2012 roku. Chory był leczony lekami przeciwhistaminowymi oraz miejscowo glikokortykosteroidami, jednak bez poprawy. W badaniach laboratoryjnych z odchyleń stwierdzono zwiększone stężenie IgE i trójglicerydów. Wyniki badań histopatologicznego i immunologicznego potwierdziły rozpoznanie choroby Duhringa. Pacjenta leczono dietą bezglutenową i dapsonem w dawce 150 mg/dobę i uzyskano poprawę. Przypadek 3.: 58-letni pacjent. Pierwsze zmiany o typowej morfologii pojawiły się około 2 lat temu. Chory był leczony ambulatoryjnie lekami przeciwhistaminowymi oraz miejscowymi glikokortykosteroidami, jednak bez poprawy. W badaniach dodatkowych potwierdzono rozpoznanie dermatitis herpetiformis. Włączenie terapii dietą bezglutenową i dapsonem w dawce 100 mg/dobę spowodowało ustąpienie zmian. Wnioski. Przedstawione przypadki ilustrują trudności diagnostyczne w chorobie Duhringa, rozpoznawanej przez długi czas, pomimo braku poprawy po leczeniu lekami przeciwhistaminowymi i miejscowymi glikokortykosteroidami, jako zmiany wypryskowe. Wielopostaciowość zmian nie zawsze jest widoczna na wszystkich etapach choroby, co może powodować trudności diagnostyczne. Statistically significant rise of the mean value of total IgE was noted in both groups of patients. {"title":"[Use of Nd:YAG laser for treatment of basal-cell carcinomas and some premalignant conditions].","authors":"J Rubisz-Brzezińska,&nbsp;W Bendkowski","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In 51 patients, 29 female and 22 male of average 61 years old, 94 lesions were treated with Nd: YAG laser; 70 basal-cell carcinomas, 9 superficial basal-cell carcinomas, 12 actinic keratosis and 3 cutaneous horns. Size of changes varied from 7 to 24 mm. Quantity of energy used in laser-therapy was dependent on diagnosis, focus size, location and amounted from 50 to 240 J/cm2. Authors are of the opinion that Nd:YAG laser-therapy is a very good method for treatment of skin tumors. The lamps produced by Heraeus were used, in 48 patients--Psorilux 3060 and in 65--Psorilux 5050. The first symptoms of improvement were visible after 7-10 days. The mean duration of the treatment was 4 weeks. In 46 per cent of the patients psoriatic eruptions disappeared completely and in the further 44 per cent ones a considerable improvement was observed. The better results were obtained in patients irradiated with lamp of greater power and in those, who received a greater number of irradiations. Severe side effects were observed only in 2 cases.
