{"title":"Negative Pressure Therapy: Theory and Practice","authors":"Ling Guo-ru","doi":"10.7748/phc.22.9.14.s14","DOIUrl":"https://doi.org/10.7748/phc.22.9.14.s14","url":null,"abstract":"NPT(negative pressure therapy)which also can be called VAC(vacuum-assisted closure)is a treatment which can promote wound healing.It can not only be used to one stage treatment of chronic and complex wounds but also can assist to temporary close the wound and to the preparatory work of pre-operation wound area such as dermatoplasty. Although the physiological basis of VAC has been not fully expounded, the clinical effects have been proved.We analysed the relevant researches that based on randomized clinical trial, VAC and topical negative pressure which are chosen from Pubmed-medline and assessed the scientific data which derived from animal experiment, randomized clinical trial and observing in clinical application. According to the data, NPT has remarkable effects in inducing wound healing especially in early stage of wound healing. Besides, NPT also has positive effect in anti-infective therapy. Experimental data have presented that VAC has positive effect in promoting wound healing. But the functional mechanism of it has not been clarified because of evidence-based medicine data could not explain the positive effects of clinical treatment. Methods: 60 patients with gastric cancer scheduled were randomly divided into preoperative oral carbohydrate or placebo drink. Preoperative general well-being, 2-3 hours before the induction of anesthesia all patients received either a placebo drink or carbohydrate drink, blood glucose, insulin were measured before and immediately after surgery, Insulin resistance index was calculated using Homeostasis Model Assessment (HOMA), and postoperative complications and length of hospital stay were respectively recorded. Results: No complications associated with anesthesia were recorded as a result of preoperative fluid consumption. In the two groups, blood glucose, insulin and HOMA-IR immediately postoperative in the preoperative oral carbohydrate group were significantly lower than those in the preoperative oral placebo group. Insulin sensitivity index (ISI) was reduced in both groups; the oral carbohydrate group was higher than oral placebo group. There were no differences between two groups on length of hospital stay. Conclusion: Preoperative consumption of carbohydrate-containing fluid is safe.
