Cantisano-Zilkha","doi":"10.1076/OTOO.4.2.74.16587","DOIUrl":"https://doi.org/10.1076/OTOO.4.2.74.16587","url":null,"abstract":"An ophthalmic plastic clinical experience of 22 years of Botox ® use is reviewed, beginning with the clinical trials at Manhattan Eye, Ear and Throat Hospital in 1980 for benign, essential blepharospasm control, and continuing with the current ever-expanding facial cosmetic applications in private practice today. We have reviewed the advantages and potential complications of its use as a primary rhytid ablator for crow’s feet, glabellar furrows, transverse forehead creases, lower eyelid creases and hypertrophic orbicularis muscle folds, upper lip furrows and “smokers” lines, and neck ring creases. We discuss its use in relaxation of platysmal bands and persistent frowning. Patients who are not good candidates for these indications are discussed. Chemodenervation with Botox ® has become an integral step in the preparation of our patients undergoing non-ablative laser facial rejuvenation as well as those undergoing laser resurfacing and Intense Pulsed Light photorejuvenation. It is in every organ of the body. As ophthalmic surgeons know, it has been well tolerated inside the human eye for the last 30 years. When we used it to reconstruct monkey eyelids that had been deformed by trauma, it maintained volume for more than 2 years. Because it is not a foreign protein, and because it is derived from a non-animal source, the potential for inflammatory reaction and allergy is remote. This article will review our 6-year experience utilizing Restylane and Perlane in 2241 patients. Restylane and Perlane have been effective agents used to fill naso-labial folds, marionette lines, glabellar furrows, depressed scars, upper lip lines and to augment lips. We have observed only mild, transient erythema and edema at the injection sites lasting one to two days. Rarely punctate ecchymoses followed the injections and lasted one to five days. The underlying supportive structures of this area may need dental rejuvenation. Once this is established, the upper lip skin can be managed with non-ablative or resurfacing lasers, chemical peels, filling agents, and Botox. The body and contours of the lips may need volume supplied by filling agents, but also pigment, which will require artful micropigmentation, and Botox applied to the orbicularis oris for lip eversion. They are often even willing to achieve a less dramatic result and to return for multiple treatment sessions in exchange for a no downtime procedure. Thus, new and effective non-invasive techniques have become eminently important. The course of twenty consecutive patients were reviewed retrospectively. By minimizing lateral heat spread, fine wire electrodes of varying shapes have been accurately employed to shave benign facial lesions and ablate facial telangiectasias with minimal trauma, recovery time, or cutaneous demarcation.
