{"title":"COVID-19 Severity Among Non-Vaccinated Pregnant Women During the Delta Wave in Tunisia: A Retrospective Monocentric Study","authors":"Manel Kammoun, A. The aim of our study is to describe the COVID-19 syndrome among non-vaccinated pregnant women during the delta wave and to investigate risk factors for severe forms. Methods: In this study, we included all non-vaccinated pregnant women who tested positive for COVID-19 and who required hospital admission at any stage of gestation during the Delta wave in the maternity of Sfax, Tunisia. Patients were divided into 2 groups according to the mode of delivery in case of completed pregnancies and according to the severity of the disease. Severe COVID-19 is considered when the pregnant woman requires advanced oxygen support or intensive care unit referral. We performed univariate and multivariate logistic regression models to investigate the predictors of severe maternal outcomes among infected pregnant women. The significance level was set top ≤ 0.05. Results: one hundred patients were included. Severe adverse outcomes were observed in 23 patients (group1). The mortality rate during the Delta wave was 6%. The mode of delivery had not influenced the maternal and perinatal outcomes. Age >35 years old [OR 3.16, 95% CI 1.13- 8.84], BMI>30 kg/m2 [OR 2.63, 95% CI 1.0 -6.95], preeclampsia [OR 4.0, CI 95% 1.04- 15.32], dyspnea [OR 7.55, 95% CI 2.62- 21.7], cytolysis [ OR 4.6, 95% CI 1.48- 14.2], and lung injury in CT Scan > 50% [OR9.6, 95%CI 1.48-62.1] were significantly associated with an increased risk of severe maternal outcomes. Conclusions: During the delta wave in Tunisia, non-vaccinated pregnant women seem to be at higher risk of severe maternal outcomes and maternal deaths. {"title":"Evaluation of the Anti-Inflammatory Activity of the Leaf Extract and Fractions of Ipomoea Quamoclit in Albino Wistar Rats","authors":"O. Ipomoea quamoclit is one of such ethnomedicinal plants of reported health benefits. Aim of Study: This study was aimed at evaluating the anti-inflammatory activity of ethanol extract and fractions of the leaves of Ipomoea quamoclit Mill. Materials and Methods: Ipomoea quamoclit leaves were harvested and extracted with 80% ethanol by cold maceration for 48 hours and the crude extract was fractionated to obtain n-hexane, n-butanol and ethyl acetate fraction, respectively. The crude extract and its fractions were screened to unveil the resident phytochemical constituents. The ethanol extract was tested for acute toxicity (LD50). The systemic acute inflammation of the crude extract and fractions of Ipomoea quamoclit was studied using the water (fluid) displacement method after inducing inflammation with egg albumin, using rats. The effect of the crude extract and fractions of Ipomoea quamoclit on arthritis was studied using the formaldehyde-induced arthritis model in rats. Results: The results of the qualitative phytochemical evaluation of the plant confirmed the presence of these phytoconstituents such as alkaloids, flavonoids, saponins, tannins, steroids, terpenoids, glycosides, carbohydrates, proteins, and coumarins. There were no deaths recorded for acute toxicity study for both phases of the test. In the acute anti-inflammation assay, significant (P<0.05) anti-inflammatory activities were observed from the 5th hour for the ethyl acetate fraction (150 and 400 mg/kg) and the butanol fraction (400 mg/kg). The anti-arthritic activity of the crude extract (150 mg/kg), ethyl acetate fraction (150 and 400 mg/kg), n-hexane fraction (150 mg/kg) and butanol fraction (150 and 400 mg/kg) was obvious from the second day after administration. By the 10th day the crude extract (50, 150, and 400 mg/kg) and ethyl acetate fraction (150 and 400 mg/kg) brought about a significant reduction of chronic inflammation (arthritis) in the test animals to normal basal levels. Conclusions: This study thus provides scientific information and validates the ethnobotanical use of Ipomoea quamoclit leaves in management of diseases and inflammation inclusive. {"title":"Intracranial Bleeding as Primary Manifestation of Antiphospholipid Syndrome: Case Report","authors":"Dominique Zöld, J. Various antibodies have been identified causing the activation and aggregation of thrombocytes and endothelial dysfunction resulting in a wide range of symptoms. Frequent manifestation may be arterial or venous clots in extremities or internal organs, neurological manifestation or pregnancy morbidities. Rarely, APS can manifest in the form of hemorrhage. We discuss the case of a 40-year-old male presenting with a severe headache, fatigue and fever. Head CT confirmed a subdural hematoma. Laboratory findings showed anemia, thrombocytopenia and elongated coagulation times. Immunoserology found the strong presence of lupus anticoagulant, anti-cardiolipin-, and anti-β2glycoprotein-I IgG antibodies confirming the diagnosis of primary antiphospholipid syndrome, specifically LA - hypoprothrombinemia syndrome. Following the administration of high dose glucocorticoids, we chose to give intravenous immunoglobulin followed by rituximab. Although his symptoms regressed and there was no further hemorrhaging, we chose to perform plasmapheresis due to continuously high concentration of anti- phospholipid antibodies to lower the antibody titer, which proved to be effective. Antithrombotic treatment can be challenging in such cases due to the co-occurrence of hypoprothrombinemia and prothrombotic antibodies. We decided to administer low-dose aspirin, with which the patient has no bleeding or blood clots for over 6 years. Methods: A retrospective assessment of electronic data and case notes of HAART naïve patients in our hospital between January 2016 and December 2018 was done. Data on age, sex, packed cell volume (PCV) and CD4 at diagnosis was collected. Continuous variables were compared with the students’ t-test while categorical parameters were compared with the chi-square. Spearman’s correlation was used to assess the association between anaemia and the CD 4 count. Risk factors for the outcome variable were determined with logistic regression and a p-value of ≤0.05 was considered statistically significant. Results: The records of three hundred and twenty-one (321) new registrants in the Antiretroviral Therapy (ART) Clinic in Rivers State University Teaching Hospital (RSUTH) were evaluated. Their mean age was 40.43 ±10.51 years (range 19 – 76 years). Anemia was prevalent in 280(87.2%) patients. The CD4 count was normal in 102 (32.6%) patients. Female gender (p=0.001) and WHO clinical stage (p<0.001) were found to be the strongest predictors of anemia in this study. Conclusion: The findings from this study shows that a significant proportion of people infected with HIV present with severe immunosuppression and low hemoglobin. Subsequently decreasing work accuracy or increasing the medical errors in the medical field. The increasing amount of medical errors that was suggestive of depression encouraged us to estimate the extensiveness of depression among health workers in the hospitals in Kuwait. Aim: To provide basic data that will contribute to the design of preventive measures that will reduce the prevalence of Depression among health workers. Study Design: Cross sectional study. Materials and Methods: The research was conducted in AlFarwaniya Hospital & Mubarak AlKabeer Hospital in 2016. Data was performed using IPad and analyzed using SPSS. Results: The highest percentage of depression is among the ones whose ages are between 30 to 39 which is 12% (37) out of 52.4 % (162). It shows that females had depressive symptoms more than males, 7.1% of males were depressed compared to the 12.2% who were females. It seems that Kuwaiti health workers show depressive symptoms more than the non-Kuwaiti. It shows that doctors have highest depression rate (28%) then the nurse and finally the other careers. Married individuals show 68% are depressed which is the highest, single individuals with (28%) and divorced with (1.7%). Smokers were with 25% and nonsmokers with (19%) of depression. Chronic diseases don’t show significant effect on depression. Conclusion: This study showed that there is a difference in the prevalence of depression between the genders as well as the social status. Frequently, there is a need to start therapy rapidly. Autoantibodies directed to epitopes on red blood cell (RBC) consisting in sugar and/or protein structures are crucial in the pathogenesis of AIHA. The isotype is important for the clinical significance of an autoantibody. Frequently, patients are icteric and suffer from clinical signs of anaemia, such as pallor, fatigue, shortness of breath and palpitations, on examination hepatosplenomegaly can be present. If there is no vital indication for a transfusion it is prudent to wait for the results of the immune-haematological tests and the ensuing transfusion advice based on this. Focus of treatment should be stoppage of hemolysis or at least be attenuated via an inhibition of autoantibody production and/or inhibition of premature RBC destruction. There are five types of body rhythms such as diurnal, circadian, ultradian, infradian, and circannual rhythms. The present review aims to discuss body rhythms, body rhythms disorders, and their energetics. Diurnal rhythms occur during the day and in resting, or inactivity at night, circadian rhythms complete one cycle in 24 hours and known as midday activity, ultradian are recurrent or repeated cycle in 24 hours, infradian rhythms complete one cycle more than one day, and circannual rhythms complete their cycle in 24 hours or one year. Body rhythms are following disorders like delayed sleep phase syndrome, altered Sleep, phase Shift Work Dysfunction, and circadian rhythm nap defects. Body rhythm control stress, lighten body load, exercise, avoid smoking, restrict sleeping, avoid napping during the day, provide energy for eating, and limit alcohol consumption.
