The objective of the study was to evaluate the success and complications of microsurgical over-top decompression for single-level lumbar spine stenosis.\u0000Material and Methods:  This observational study was conducted at the Neurosurgery unit at Lady Reading Hospital Peshawar from Jan 2018 to December 2021. All those patients who had undergone over-top decompression for single-level degenerative lumbar spine stenosis irrespective of age and gender were included in our study.\u0000Results:  We had a total of 187 patients who underwent microscopic over-top decompression for lumbar spine stenosis. Four patients lost their final follow-up. The mean age of the patients was 46 years (ranging from 18 – 68 years). Spinal stenosis was more common in men (58.3%) and at L4/5 (51.87%) level. The mean duration of surgery was 56 minutes (range 35-86 minutes). Leg pain improved in 83.1% of the cases with overall patient satisfaction after surgery in 82.5% of the cases. The most common (6.95%) complication of the procedure was iatrogenic durotomy. Most of the complications were minor and treated conservatively.\u0000Conclusions:  Overtop decompression is a safe and effective minimally invasive procedure for lumbar spine stenosis. It has good results in symptomatic improvement. There is minimal soft tissue and bony dissection. Therefore, mobility and stability of the spine are preserved. This study involved 80 patients aged between 20-55 years of both genders diagnosed of CSF rhinorrhea presented with trauma. \u0000Results: In the department of Neurosurgery BVH, Bahawalpur 2000 patients were admitted with head injury in two years from April 2021 to February 2022. A total of 80 patients of these had cerebrospinal fluid (CSF) rhinorrhea. This study was conducted to evaluate the management options of CSF rhinorrhea in the patients with head injury. CSF rhinorrhea was stopped in 70 patients with conservative management. In 10 patients cerebrospinal fluid rhinorrhea continued after the conservative management of 2-6 weeks and surgical repair of Dural tear at the base of skull was done in these patients. Site of CSF rhinorrhea leakage was located in these patients with the help of CT Scan and MRI skull.In all these patients Dural tears was repaired with a graft of fascia Lata or periosteum, pasted on the site of defect with fibrin glue. \u0000CONCLUSION:We concluded that all post traumatic C.S.F rhinorrhea cases should be managed conservatively because most of the C.S.F leaks stop spontaneously with proper conservative management. The complication rate after cranioplasties seems to be higher than elective craniotomies so this study was conducted to determine the outcome of cranioplasty after craniectomy. \u0000Materials & Methods:  The patients included in this study had craniectomy and cranioplasty for any indication. Titanium mesh was placed on the skull defect 3 – 6 months later in the private hospitals of Quetta. The resulting complications were reported. \u0000Results:  There were 50 male and 10 female patients. The majority (66.66%) of patients were from the age group 15 – 40 years. The majority of patients (83.33%) did not develop any complications. 8.3% of patients had wound infections, 3.3%had mesh exposures, 1.6% developed loosening screws and 1.6% had mesh indentation due to external pressure. \u0000Conclusion:  This study concludes that there are few patients (< 20%) who develop complications after cranioplasty. Non-probability convenience sampling technique was used. Children with reported sexual abuse who fulfilled the inclusion criteria were included in this study. All children included in the study were screened for psychological disturbances through the Urdu version of SDQ. The severity of symptoms was assessed based on ICD-10 criteria. \u0000Results:  There was a significant relationship between the identity of the perpetrator with the stability of the family. In victims belonging to the intact family system, the perpetrator was mostly an outsider while, in cases of broken families, oftentimes, the abuser was a relative or known to the victim. 44.44% of individuals belonging to broken families were abused within the confinements of the home. 17.8% of children from unstable families had mixed anxiety and depression disorder, while the rate was lower among children who had intact families. Surprisingly, only five children altogether had post-traumatic stress disorder. Hyperkinetic disorder and conduct disorder was significantly more prevalent in victims belonging to a broken family system. \u0000Conclusion:  Childhood sexual abuse and its resultant psychiatric morbidity are equally common in children from intact and broken families. A total of 36 patients with dorsal and lumbar spine tuberculosis were operated on for transpedicular fixation. Assessment scores were used pre-operative and post-operative and patients were kept on follow-up till six months after the surgical procedure. Variables like age, gender, spinal level, preoperative, and follow-up clinical status were calculated.\u0000Results:  There were 19 (53%) male and 17 (47%) female patients with a mean age of 27 ± 8. Thoracolumbar was the commonest segment involved in 17 (47%) patients, followed by lower thoracic in 8 (22%) and lumbar in 7 (19%). There were 7 (19%) patients on the preoperative American Spinal Injury Association (ASIA) impairment scale in grade B, 12 (33%) in grade C, 15 (42%) in grade D, and 2 (5%) in grade E. The follow-up assessment at 6 months showed that ASIA grade B was seen in 3 (8%), grade C in 4 (11%), grade D in 16 (44%), and Grade E in 13 (36%) patients. Preoperative and follow-up scores on the ASIA impairment scale, COBS ANGLE, and ESR showed a significant difference (p-value < 0.05).\u0000Conclusion:  We concluded that transpedicular fixation can restore the stability of the spine in thoracic and lumbar tuberculosis. We evaluated the average time required to return to work following percutaneous transpedicular fixation versus open pedicle screw fixation in traumatic lumbar spine injury. \u0000Material and Methods:  A randomized controlled trial included 60 patients. At study entry baseline demographics (age, gender, & duration of injury) were recorded. 30 patients were in the percutaneous transpedicular fixation group (A), while 30 patients were in the open pedicle screw fixation group (B). All the patients were followed every month time taken to return to work (TTRW) was noted on a proforma. \u0000Results:  Mean time taken by patients to return to work after surgery in Group A was 2.9 days, while in group B it was 5.1 days in group B. The difference between the two groups was significant (p-value 0.001). Within Group A, male and female genders showed a significant difference (p-value 0.032) in the TTRW after surgery. However, Group B did not show a similar difference between male and female patients. Duration of procedure had a significant effect on the TTRW (p-value 0.001). \u0000Conclusion:  We found ‘ time is taken to return to work’ was 2.93 ± 0.82 in group A and 5.10 ± 0.71 in group B (P-value 0.001). There was a significant difference in both groups. Anwar, Muhammad Akmal Hussain","doi":"10.36552/pjns.v26i1.622","DOIUrl":"https://doi.org/10.36552/pjns.v26i1.622","url":null,"abstract":"Background/Objective:  We compared the efficacy of multi-dural stab craniectomy with open dural flap craniectomy in patients having acute subdural hematoma.\u0000Materials and Methods:  A randomized controlled trial was conducted in the Department of Neurosurgery, Allied hospital, Faisalabad. 70 patients having acute subdural hematoma were included. All of the patients were randomized in two groups: group A received a multi-dural stab craniectomy, while group B received an open dural flap craniectomy. Efficacy was assessed after 3 months of treatment in terms of a good recovery.\u0000Results:  Out of 70 patients, the mean age was 34.51 ± 8.712 years. Because of similar age and presenting GCS, the data show a non-significant difference in clinical outcomes between patients undergoing multi dual stab and open dural flap operations. However, a significant difference between the clinical results occurred at 3 months after the surgery (p-value 0.004). The efficacy of the multi-dural stab procedure was significantly higher from the open dural flap surgery (p-Value 0.006). The efficacy of the multi-dural stab surgery was significantly higher in the under 35 years age group of patients compared to open dural flap surgery. We evaluated the outcome of manual traction in patients presenting with dizziness and neck pain. \u0000Materials and Methods:   50 patients included who had dizziness and neck pain with a reduced range of cervical motion. Patients had sessions of manual traction after pre-treatment evaluation. Patients included in the study after history and physical examination were sent to the otorhinology department to rule out the vestibular cause of dizziness.  Patients were sent to the Physiotherapy department for manual traction. Pain and dizziness were assessed from VAS and DHI scores. \u0000Results:  There were 28 male and 22 female patients. The mean age of the patients was 23.92 ± 11.39 years. The mean VAS scores (dizziness) were 46, 31, and 14 before treatment, at one week, and at one month, respectively. The mean VAS scores (pain) were 40, 21, and 8 before treatment, at one week, and at one month, respectively. The mean DHI scores (for dizziness disability) were 47.5, 34, and 21, at one week and at one month, respectively. Magnetic resonance imaging and the computed tomographic scan were done.4 to 6 ml of ozone were inserted in disc space. Periodic follow-up was done for up to 6 months of therapy. A single session of therapy was required in 55%, twice in 22.5%, and multiple in 13% of patients. \u0000Results:  Out of 310 patients, n = 220 were male and 90 females. Cervical herniated discs were 30 and lumbar 280, single cervical disc herniation was found in n = 26 and multiple in n = 4 while the single-level herniated lumbar disc was n = 220 and multiple in n = 60. Self-analysis of pain before intervention was assessed by visual analog, scored was VAS 4 in n = 80, VAS 5 – 6 in n = 120, VAS 7 – 9 in n = 70, and VAS 10 in n = 40 patients.
