{"title":"Comparison of Outcomes of Early and Late Surgical Interventions in Lipomeningomyelocele (LMMC) and Lipomeningocele (LMC)","authors":"Shahzeb Ahmad, Shakil Mashori, Syeda Kiran Zahra1, Rana Muhammad Usama, Abdul Hanan, Shmama Tu Zahra","doi":"10.36552/pjns.v26i4.821","DOIUrl":"https://doi.org/10.36552/pjns.v26i4.821","url":null,"abstract":"Objectives:  The study compared the signs and symptoms of post-operative complications in early vs. late intervention lipomeningomyelocele (LMMC) and lipomeningocele (LMC). \u0000Materials and Methods:  We compared the clinical and surgical data between two groups i.e., lipomeningomyelocele (n = 189) and lipomeningocele (n = 64), and their early vs. late surgical interventions for 3 years from January 2018 to July 2021. We included patients of both genders (n = 253) with lipomeningomyelocele or lipomeningocele aged up to 7 years. A detailed neurological exam i.e., sensory, motor, and cerebellar signs was performed to evaluate the patients. \u0000Results:  The presentation of LMMC (74.7%) was very high compared to LMC (25.3%). 74.7% underwent detethering of the spinal cord, as they had cord tissue coming out of the defect. 25.2% had only meninges coming out of the bony deficiency and performed dural repairs. 47 patients had incontinence which was improved postoperatively. Sixty-nine patients had hydrocephalus which was treated with VP shunt or ETV. 23 patients had diastematomyelia which is a bony spur duly repaired intra-operatively. 50 presented with paraplegia and 19 cases with club feet. The majority of patients in both groups, reported for Power would fall between 3/5-4/5. For patients who underwent late intervention, 7 presented with post-operative incontinence, 12 with hydrocephalus, 12 with CSF leakage, and 13 with paraplegia. \u0000Conclusion:  If performed on time, surgical intervention in lipomeningocele and lipomeningomyelocele yields good results. Regarding pain, neurological deficit, and spinal instability, the study sought to determine the surgical outcome of spinal tuberculosis. \u0000Material and Methods:  This study was conducted in the Department of Neurosurgery, Ayub Teaching Hospital. A total of 42 patients with ages ranging from 24 – 66 years were included in the study. Information about the patient was gathered to determine indications (neurological impairments) about the ASIA Grading Scale and frequently affected areas. \u0000Results:  Commonest location was a thoracolumbar junction in 42.8%, followed by the lumbar spine in 23.8%. The pain was found in all subjects. The main indication of surgery was neurological deficit which is seen in 57% of patients followed by spinal instability or deformity in 16.6% of patients and patients with failed chemotherapy in 4.7% of patients. The preoperative KA was between 30 – 60º with a mean of 43.06 ± 4.11 degrees and 9.45 ± 3.99 degrees for postoperative KA (p-value < 0.0001). Most fixations of the cord were done through a posterior approach that was in 83.3% of patients. The neurological improvement according to ASIA grading was seen in 95.2% and no improvement was seen in 4.7% of patients upon 3 months follow-up. \u0000Conclusion:  Surgical treatment of the caries spine improves pain, lowers neurological compression, and successfully reverses deformities like kyphosis, resulting in better clinical results. One hundred and twenty patients with Carpal Tunnel Syndrome (CTS) who attended the study site were enrolled. Baseline data, including age, gender, residential area, Duration of CTS, smoking, hypertension, diabetes mellitus, and obesity, were obtained. The blood samples were obtained for screening for hypothyroidism.\u0000Results:  It was observed that most of the CTS patients were males (63.3%). The observed mean age was 44.3 ± 12.1 years, and 65.8% of cases belonged to urban areas. It primarily affects the immunocompromised but is rarely seen in immunocompetent individuals. The pitfalls in the healthcare system of lower-middle-income countries produce many obstacles, impeding timely diagnosis, timely treatment, and optimal quality of care. A 57-year-old immunocompetent, diabetic female presented with progressive generalised fatiguability, forgetfulness, and subtle behavioural changes for the last one and a half years. MRI revealed an area of gyri from dense post-contrast enhancement in the right posterior parietal region. Her symptoms kept aggravating with periods of partial remission as steroids were being administered. Reasons for delays in diagnosis and hence timely treatment include lack of primary care referral, conflicting neurosurgical opinion, and absence of multidisciplinary team management. A right posterior parietal parasagittal craniotomy was performed to excise the lesion. The patient was subsequently referred for adjuvant therapy. We use our patient's clinical journey to exemplify the structural barriers to providing optimal and timely care for brain tumour patients in a developing country. Primary CNS Lymphomas in immunocompetent patients are rare. Poor infrastructure and referral pathways contribute to delayed diagnosis. Lack of multi-disciplinary care owing to organisational issues is a major problem faced by brain tumour patients in a lower-middle-income country like ours. The present study was designed to analyze the clinical outcome of patients operated in our department for spontaneous cerebellar hematoma evacuation. \u0000Method: The medical data of all the patients in whom spontaneous cerebellar hematoma had been surgically evacuated were retrospectively analyzed. The parameters of age, preoperative Glasgow coma scales (GCS), and the volume of the hematoma was studied and their relationship with clinical outcome based on the Glasgow outcome scale (GOS) was analyzed \u0000Results:  The study included twenty-four patients. Fourteen (58.3%) patients survived and ten (41.7%) patients expired. The mean age of the surviving patients was 57 years and the mean age of the expired patients was 57.2 years. The difference wasn’t statistically significant (p = 0.9612). The mean hematoma volume in the surviving patients was 23 cm3 and in the expired patients was 26.2 cm3. The difference didn’t reach statistical significance (p = 0.1614). The mean preoperative GCS of the surviving patients was 12.4 and of the expired patients were 8.3. This difference was statistically significant (p = 0.004). All the surviving patients had a favorable outcome based on the GOS score \u0000Conclusion:  Surgical evacuation of cerebellar hematomas leads to a good clinical outcome. Preoperative GCS is significantly associated with the clinical outcome. {"title":"Role of Anterior Cervical Decompression Fusion (ACDF) and Plating in Patients with Lower Cervical Caries Spine","authors":"Shahzaib Tasdique, Zainab Sarwar, Sania Javed, Samra Majeed, Waqas Mehdi, Azam Niaz","doi":"10.36552/pjns.v26i4.802","DOIUrl":"https://doi.org/10.36552/pjns.v26i4.802","url":null,"abstract":"Objective:  We assessed the neurologic status of patients with cervical spine caries after anterior cervical decompression fusion and plating in terms of muscle power measured by MRC (medical research council) grade and functional status measured by Nurick grade. \u0000Materials and Methods:  A descriptive case series study was conducted and 36 subjects with lower cervical spine (C3 – C7) were included with cervical spine caries who needed anterior cervical decompression and plating operated by the department of Neurosurgery, KEMU/Mayo Hospital. The cervical spine TB was confirmed using an MRI plain and contrasted cervical spine. Post-surgical neurological examinations were performed on the 1st, 7th, 21st, and 42nd days and the 3rd and 6th months after surgery. The neurological state was measured in terms of muscle power measured by MRC grades and functional outcome was assessed from Nurick grades. \u0000Results:  The mean age of the patients was 37.0 ± 14.4 years. The majority of patients had C5 caries spine (28%) and the least had C7 caries spine (14%). Most of the patients had pre-operative MRC grade 3 (41.7%). Intradural extramedullary tumors include neurofibromas and meningiomas while intramedullary tumors are mostly astrocytoma and ependymoma. The purpose of this study was to share the experience of managing these patients as a multidisciplinary team (neurosurgeons, general surgeons, thoracic surgeons, ENT surgeons, plastic surgeons, and oncology) in selected cases where the tumors grow into the surrounding structures. \u0000Material and Methods:  This study was conducted at the Neurosurgery Department at Jinnah hospital Lahore from January 2021 to September 2022. 50 patients admitted to the Neurosurgery Department from OPD/emergency with the diagnosis of a spinal tumor confirmed on MRI were included in the study. Those patients who were unfit for surgery were not included in the study. Data was analyzed for gender, region, and location of the tumor, histopathological diagnosis, and multidisciplinary team involvement. \u0000Results:  Out of 50 patients, 28 (56%) patients were male and 22 (44%) patients were female while ages ranged from 14 to 63 years. Extradural location was the most frequently encountered location, i.e., 50%. Metastatic tumors were the commonest. It will give insight and drive further development in the care of myelomeningoceles as well as the unique problems that emerge in these patients post-operatively.\u0000Materials and Methods:  At the Jinnah Postgraduate Medical Center in Karachi, the Department of Neurosurgery, this prospective, observational study was carried out over 12 months. Patients of both genders from 1 month to 2 years of age, diagnosed with myelomeningocele, and admitted to the ward for surgical repair were included in the study.\u0000Results:  Out of 94, there were 58.5% female patients, with the highest percentage of age group i.e. 48.9% in > 3 months to 9 months. Medium size of MMC (71.3%) at the lumbar side location (46.8%) was observed in the highest frequency among the study sample. The overall postoperative complication rate was 19.1%, with CSF leak being the most common (9.6%) followed by wound infection (5.3%). There was no association of the complications with the age, size, and location of MMC.\u0000Conclusion:  Myelomeningocele treatment may be delayed as a result of distant homebirths. Arslan Ahmad, Khawar Anwar","doi":"10.36552/pjns.v26i4.766","DOIUrl":"https://doi.org/10.36552/pjns.v26i4.766","url":null,"abstract":"Objectives:  An observational study to evaluate the outcomes of Acromegaly patients treated in the department of Neurosurgery III, Punjab Institute of Neurosciences (PINS), Lahore, Pakistan.\u0000Materials and Methods:  This study was the analysis of a case series of 20 patients (mean age: 30 years) of both genders with acromegaly due to macroadenoma who had endonasal excision and postoperative treatment with Sandostatin LAR.\u0000Results:  This study consisted of a total of 20 patients out of which 11 (55%) were male and 9 (45%) were female. All 20 Patients (100%) had a headache as their main symptom and 13 patients (65%) had visual field deterioration. 19 patients (95%) had complete resection of the tumor whereas 1 (5%) had a subtotal resection. After surgery and treatment growth hormone levels improved in 19 patients (95%) and remained elevated in 1 patient (5%), IGF-1 (Insulin-like growth factor – 1) levels improved in all 20 (100%) patients. Following consecutive octreotide injections vision of 11 patients (84%) out of 13 improved and in 2 (16%) patients, it remained the same as of Preoperative status whereas Headache improved in all 20 (100%) of the patients. None of the patients had any complications related to the procedure or the injection of octreotide LAR.\u0000Conclusion:  Acromegaly due to macroadenoma responds well to treatment with octreotide LAR after endonasal resection in terms of hormonal levels and symptoms of the patients. We included 148 patients. After fulfilling the inclusion criteria, the patient’s baseline data, including the patient's age, gender, and CT scan findings with the degree of midline shift, was noted. The patients were monitored for three months to evaluate the outcome. The collected data was analyzed using SPSS version 22.0. \u0000Results:  Our study showed that 105 (70.9%) patients showed satisfactory outcomes while 43 (29.1%) showed unsatisfactory outcomes. Patients with no midline shift were 70, out of which 55 (78.6%) showed satisfactory outcomes. Similarly, patients with 1-2 mm midline shifts showed satisfactory outcomes in 39 (69.6%) while 3-5 mm midline shifts showed 11 (50%) satisfactory outcomes.
