The infestation, whose incidence is often underestimated, is widespread with a higher prevalence within the paediatric age, particularly in children under the age of 2. In young children diagnosis can be challenging because of the differences in appearance and location of skin lesions, especially in the neonatal forms. Neonatal scabies has no treatment approved by European guidelines, and the most widely used drug in infants such as permethrin is off-label under 2 months of age. This article describes a case of neonatal scabies. The patient presented with vesicle-crusted erythematous elongated lesions on the trunk and papular lesions in the axillary region, with minor involvement of the upper limbs. Clinical suspect was then confirmed through dermatoscopy. 5% permethrin cream was applied for three consecutive days with a 3-hour exposure time. {"title":"Una sindrome infiammatoria multisistemica fredda complicata da shock","authors":"Ian Valencic, Mattia Moratti, Francesca Lombardi, Rosa Francavilla, Chiara Ghizzi","doi":"10.53126/mebxxvio151","DOIUrl":"https://doi.org/10.53126/mebxxvio151","url":null,"abstract":"<b>Rationale -</b> In clinical practice there are numerous factors influencing the correct process of diagnosis and treatment, especially in the context of complex clinical cases such as severe and atypical forms of \"multisystem inflammatory syndrome\" (MIS-C): several studies have observed that in the Emergency Department fewer than half of cases of hemodynamic shock are treated according to Paediatric Advanced Life Support (PALS) guidelines.<br> <b>Objectives -</b> To favour the recognition of MIS-C in its atypical form and to provide indications on the correct treatment in the course of its acute complications such as mixed shock (distributive-cardiogenic). <br> <b>Materials and methods -</b> Starting from the analysis of a clinical case, the literature about MIS-C and the management of mixed shock in the Emergency Department was reviewed. Artificial nutritional support, often indispensable in the most critical phases of the course, requires careful evaluation of the clinical conditions at the time of hospitalization and equally punctual monitoring of blood chemistry variables during re-feeding. Prolonged reduced caloric intake and malnutrition, especially in its most extreme forms, can cause damage to various organs and systems and the refeeding syndrome must be kept in mind as a further comorbidity and aggravating factor of the patient's conditions. The paper presents the case of an 18-month-old girl taken by her mother to the paediatrics department of St. Luke’s Hospital in Wolisso, Ethiopia for general malaise, fever, asthenia and pallor for several days. On suspicion of anaemia, a hemochromocytometric examination was carried out, which showed severe microcytic anaemia, so it was decided to proceed with a whole blood transfusion, after a smear test for malaria parasites. Following a negative test for malaria, it was decided to proceed with a fresh microscopic examination of the faeces, which proved positive for hookworm eggs. Therapy with albendazole was then prescribed, at a dose of 200 mg per os as a single dose, with improvement in the clinical picture. This case underlines the importance of considering hookworm infection as a possible cause of severe anaemia, especially in areas where malaria is not endemic. It is essential to engage in hookworm infection control through preventive measures, such as hygiene, anti-parasite therapy and health education, to reduce the burden of disease in populations at risk.
