Only 2% of all reported GCTs are found in the hand, which shows a higher rate of recurrence as compared to those occurring at a more proximal location. Here, we report a case of recurrence of GCT of the proximal phalanx of the middle finger in a 52-year-old female, which was initially treated with extended curettage and bone grafting. Within 1 year, there was a recurrence of the tumor and a ray amputation had to be performed.","PeriodicalId":198403,"journal":{"name":"Journal of Orthopaedic Association of South Indian States","volume":"17 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130030703","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Direct force vector on the abducted arm is the mechanism of injury and is associated with soft-tissue, bony, and neurovascular injuries. An 83-year-old female with a history of unwitnessed falls, diagnosed with bilateral inferior shoulder dislocation clinically and radiologically. She underwent closed reduction, followed by immobilization of bilateral shoulder. There were no associated injuries bilaterally on further evaluation. Bilateral inferior shoulder dislocation is a case to be reported not only because of its rarity but also to address the gray area of management, and more research on the same is warranted for optimum clinical outcome.","PeriodicalId":198403,"journal":{"name":"Journal of Orthopaedic Association of South Indian States","volume":"4 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132160788","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Coracoid dimensions have been studied earlier, but its correlation with body anthropometry has not been studied till now. At our institute, during Latarjet procedure, we often see that length of coracoid is sometimes smaller, though the patient is of moderate to good build. This led to our interest in this study. Materials and Methods: One hundred computed tomography scans (male/female = 50/50) of Indian population were analyzed for coracoid dimensions namely total length, base length, tip height, base height, tip length, tip width, midpoint width. Body anthropometry like height, weight, body mass index of respective patients was also calculated. Our aim is to compare male to female differences in coracoid dimensions and to determine the effect of body anthropometry on coracoid dimensions. Results: The coracoid dimensions were total length 3.8 centimeter (cm) (standard deviation [SD] ± 0.47, range 3–5.1 cm), tip length 2 cm (SD ± 0.34, range 1.39–3.25 cm), tip width 1.2 cm (SD ± 0.2, range 0.67–1.7 cm), tip height 0.97 cm (SD ± 0.18, range 0.57–1.43 cm), base length 1.35 cm (SD ± 0.3, range 0.5–2.5 cm), base height 1.4 cm (SD ± 0.31, range 0.7–2.2 cm), and midpoint width 1.27 cm (SD ± 0.23, range 0.7–1.9 cm). The coracoid is larger in males than in females. We observed a negative correlation between height and all coracoid dimensions. Conclusion: Patient's height has a negative correlation with coracoid dimensions. A tall individual may not always have a large coracoid and surgeon must be prepared for alternatives in case of a shorter coracoid.","PeriodicalId":198403,"journal":{"name":"Journal of Orthopaedic Association of South Indian States","volume":"90 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125462381","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Musculoskeletal affection with diffuse viral myositis (VM) has been an uncommon occurrence. The progression of myositis to avascular necrosis (AVN) of bilateral femoral heads has been rarely reported. The case study in a 60-year-old male presented with painful bilateral hips with difficulty in ambulation simulating pseudoparalysis. The magnetic resonance imaging confirmed diffuse myositis along both the hips which needed close observation. The serial imaging at 3-month intervals showed resolution of VM, however, progression to AVN of both femoral heads. The unusual affection was initially managed medically, however, later it necessitated a bilateral total hip replacement. An astute clinician's perspective during the radiological evaluation allowed to identify the progression. The long COVID-19 may present a debilitating musculoskeletal affection of myositis around hips with progression to AVN of femoral heads. The VM of severe acute respiratory syndrome coronavirus 2 etiology may have an unrelenting long-duration affection. A multidisciplinary team approach may help in overcoming the crisis.","PeriodicalId":198403,"journal":{"name":"Journal of Orthopaedic Association of South Indian States","volume":"18 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127574428","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Magnetic resonance imaging (MRI) findings were typical of lipoma arborescens (LA). He underwent open synovectomy of the knee and a biopsy confirmed LA. 5 years later, he presented with increasing pain and swelling around the ankle joint. MRI was suggestive of LA involving tibialis posterior and flexor digitorum longus. He underwent open synovectomy with resolution of symptoms. In LA, total synovectomy helps resolve the lesion. Patients should be counseled regarding the possibility of the development of LA metachronously. To the best of our knowledge, there are no reported cases in the literature where metachronous LA presented as extraarticular in one and intraarticular in another joint.","PeriodicalId":198403,"journal":{"name":"Journal of Orthopaedic Association of South Indian States","volume":"86 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127772843","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Historically performed as a salvage surgery for cuff tear arthropathy, the indications for RTSA have now expanded over the years owing to an evolution in implant designs and the application of biomechanics in the shoulder joint. The indications for reverse polarity prosthesis have expanded from the earlier cuff tear arthropathy to included a wide range of noncuff related indications as well. The goal of this brief overview is to walk readers through the growing list of RTSA indications. The indications for RTSA are ever increasing due to its peculiar design, biomechanical advantages, and relatively good functional outcome. Off-label indications are expanding and the avenue is yet to be completely explored.","PeriodicalId":198403,"journal":{"name":"Journal of Orthopaedic Association of South Indian States","volume":"93 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115243874","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and Methods: A single-centered, prospective study on 21 patients with unstable dorsally displaced distal radius fractures between March 2019 and March 2022. We used low-profile dorsal locking plates (Arthrex) for all the cases. The functional outcome was measured using VAS score, disabilities of the arm, shoulder, and hand (DASH) questionnaire, and Gartland and Werley Score. The arc of motion and grip strength were measured on the operated wrist and compared with the contralateral wrist. We also recorded the associated postoperative complications. Results: The study group contains 11 men and 10 women with a mean age of 49 years. The mean duration of follow-up was 18 months (range 12–32). There were no cases of malunion, extensor tendon irritation, infection, neuropathy, or implant failure. The radiological union was seen in all cases. The range of motion and grip strength of the operated wrist was found to be more than 95% of that of the contralateral wrist. The mean VAS score was 0.85, DASH score 4.75 ± 0.97, and Gartland and Werley score showed excellent outcomes for all 21 cases. The experimental demonstration on bone models showed poor mechanical stability by volar plates on shear-type fractures. Conclusion: The newer low-profile dorsal locking plate provides mechanical and structural stability to the dorsally displaced fractures, even in those with poor bone stock, and is associated with negligible soft-tissue complications. We strongly recommend using dorsal plates for all dorsally displaced fractures which are distal to or at the level of the watershed line.","PeriodicalId":198403,"journal":{"name":"Journal of Orthopaedic Association of South Indian States","volume":"9 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126417871","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The current treatment guidelines suggest nonsteroidal anti-inflammatory drugs (NSAIDs) for the management of OA based on the patient's risk profile. Etodolac, a preferential cyclooxygenase 2-selective, chiral NSAID, has a long and well-established record of being an effective therapy for acute musculoskeletal pain, OA, and rheumatoid arthritis. Studies have shown that the pharmacological properties of R and S-enantiomers of etodolac differ profoundly; S-Etodolac is responsible for the majority of the anti-inflammatory activity, whereas the R-form is almost inactive. The objective was to develop evidence-based practical consensus recommendations for the management of OA with chirally pure S-Etodolac in the Indian settings. Literature review was carried out from the PubMed Database to identify relevant articles between January 1980 and May 2022 using keywords such as “osteoarthritis,” “NSAIDs,” “Etodolac,” “chirality,” “S-Etodolac,” “consensus,” and “management.” Extensive literature review was done. Further, a committee of 13 orthopedic specialists from India with significant experience in managing patients with OA was constituted. The key areas of discussion were as follows: (i) Selection of medications; (ii) role and risk/benefit profile of etodolac versus other NSAIDs; (iii) patient subgroups who would benefit from S-Etodolac oral therapy; and (iv) S-Etodolac gel in the management of OA. Experts strongly recommended S-Etodolac therapy in OA patients at increased risk of NSAID-related gastrointestinal complications, cardiovascular risk, and renal impairment. The patient pool that would derive maximum benefit from a combination of S-Etodolac and paracetamol therapy includes: (i) OA patients with the acute phase of the disease with reactive synovial effusion or acute painful inflammatory arthritis; (ii) moderate pain; and (iii) acute exacerbation of OA. This article can guide medical practitioners in clinical decision-making while choosing an appropriate NSAID therapy for the management of OA.","PeriodicalId":198403,"journal":{"name":"Journal of Orthopaedic Association of South Indian States","volume":"94 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124764162","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The incidence of Tendo achilles (TA) rupture is increasing while there exist controversies on the optimal clinical outcome. Along with either conservative or surgical management protocols, early functional rehabilitation plays a pivotal role. Technique: The patient with tissue defect 2 cm was selected. Prone position, midline incision, and suture anchor were inserted over the anatomical footprint of TA. Semitendinosus autograft was harvested and sutured with remnant TA to cover the defect using fibers from the suture anchor. Conclusion: An acute rupture of a healthy tendon can be successfully treated either conservatively or operatively. The technique of Tendoachilles repair with suture anchor and semitendinosus autograft augmentation, introduced in this literature, is a novel approach that requires further research to validate its superiority over conventional surgical techniques.","PeriodicalId":198403,"journal":{"name":"Journal of Orthopaedic Association of South Indian States","volume":"43 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126947102","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The abnormal bone morphology makes total hip arthroplasty (THA) in these patients challenging with high complication rates. This study aims to report the radiological and functional outcomes of THA in DDH patients in the South Indian population. Methodology: All DDH patients who underwent THA for secondary OA of the hip in our hospital between 2008 and 2021 were included in the study. Functional outcomes were measured using preoperative and postoperative Oxford hip score (OHS) and short form-12 (SF12) form. Radiological imaging was used to assess Crowe's classification, radiolucency, acetabular inclination angle, and offsets. Results: Mean follow-up duration was 5.5 years (2–12 years). A total of 16 DDH patients were included in the study. OHS and SF-12 scores remarkably improved compared to preoperative scores. Bony ingrowth with no evidence of loosening was seen consistently on both femoral and acetabular side in all patients on X-rays. Our complications included one posterior dislocation, one acetabular screw cutout in the immediate postoperative period, and one patient with subtrochanteric nonunion at the osteotomy site. Conclusions: THA in arthritic dysplastic hips had consistent improvement in the hip scores and quality of life. The use of the excised shortened vascularized graft technique described below-hastened osteotomy healing to 6 weeks as compared to the earlier technique. Deferring immediate partial weight-bearing in Crowe IV hip may minimize the risk of cup failure and screw cutoff.","PeriodicalId":198403,"journal":{"name":"Journal of Orthopaedic Association of South Indian States","volume":"54 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114976053","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
