The incidence of Tendo achilles (TA) rupture is increasing while there exist controversies on the optimal clinical outcome. Along with either conservative or surgical management protocols, early functional rehabilitation plays a pivotal role. Technique: The patient with tissue defect 2 cm was selected. Prone position, midline incision, and suture anchor were inserted over the anatomical footprint of TA. Semitendinosus autograft was harvested and sutured with remnant TA to cover the defect using fibers from the suture anchor. Conclusion: An acute rupture of a healthy tendon can be successfully treated either conservatively or operatively.
