Hastanın üç yıldır distal tutulumlu ülseratif kolit hastalığı vardı ve bu yüzden mesalazin tablet ve mesalazin lavman alıyordu. İshal ve kanama tariflemeyen hastanın rektosigmoidoskopisinde hastalık remisyonda izlendi. Laboratuvar tetkiklerinde hemoglobin: 15,2 g/dL (13,3-17,2), lökosit: 9000, ALT: 14 U/L (<50), AST: 20 U/L(<50), GGT: 18 U/L (<55), total bilirubin: 1,79 mg/dL (0,3-1,2) ve direk bilirubin: 0,32 mg/dL (0-0,2) olarak saptandı. Olası karaciğer hastalığı açısından değerlendirilen hastanın tomografisinde koledokta 30 mm uzunluğunda ve 20 mm çapında kistik lezyon izlendi. Manyetik rezonans kolanjiyopankreatografide lezyonu, koledok orta kısmından köken alan Tip-1B koledok kisti olarak tanımlandı (Resim 1). Patients are usually asymptomatic and the diagnosis is commonly made incidentally, during an abdominal surgery session or at autopsy; resulting in the patient undergoing unnecessary operative intervention. The risk of intraoperative iatrogenic injury is higher, and the associated morbidity of the procedure is greater. Here we reported the case of a gallbladder agenesis, who was being investigated for dyspeptic complaints and incidentally diagnosed as having gallbladder agenesis; her complaints owing to a gastric ulcer. In his story transcatheter arterial chemoembolization (TACE) was performed twice with the diagnosis of hepatitis B induced liver cirrhosis and HCC. After successfull TACE procedure patient was discharged. Four days later patient admitted with vesiculobullous lesions at procedure place. The patient was started antiviral therapy with the diagnosis of herpes zoster and lesions ameliorated. Diyet gibi çevresel etmenler semptomların gelişiminde etkili olabilmektedir. Barsak sağlığı ile ilgili çalışmalarda farklı diyet önerileri bulunmaktadır. Bunlardan biri de kısa zincirli, emilimi sınırlı/düşük karbonhidratlardan olan fermente oligo-, dive mono-sakkaritler ve polyollerin (FODMAP) sınırlandırıldığı diyettir. FODMAP içeren karbonhidratların sindirimi ile fermente substratların, suyun ince barsak ve kolondaki miktarı artmaktadır. Bu durum luminal distansiyon ve gaz oluşumuna neden olmaktadır. Literatürde diyette fermente karbonhidratların sınırlandırılmasının (düşük FODMAP içerikli diyet) gastrointestinal sistem (GİS) semptomları üzerine olumlu etkiler gösterebileceğini savunan çalışmalar bulunmaktadır. Son dönemlerde İBH ve İBS için güncel tedavi yaklaşımı olan FODMAP içeren karbonhidratlardan sınırlı tıbbi beslenme tedavisinin sağlık üzerine etkinliği konusunda çalışmalar yapılmaktadır. Literatüre her yeni eklenen tıbbi beslenme tedavisi için bireyin beslenme ve genel sağlık durumu üzerine olumlu/olumsuz etkilerinin değerlendirilmesi önemlidir. Ancak, literatürde yer alan randomize kontrol çalışmalarının sınırlı sayıda olduğu dikkat çekmektedir. İBH ve İBS tanısı alan bireylerde fermente karbonhidratların sınırlandırılmasının gastrointestinal sağlık üzerine etkilerinin incelendiği uzun dönemli yapılan daha çok sayıda çalışmaya gereksinimi olduğu düşünülmektedir. The liver grafts from anti-HBc positive donors are currently the main sources of de novo HBV infection after LT, which is usually defined by the development of positive HBsAg and/or detectable serum or liver HBV DNA in previously HBV naïve recipients. Here we reported the case of a liver transplant recipient with de novo HBV infection who had a favorable outcome after entecavir therapy. The patient received orthotopic liver transplantation because of end-stage cryptogenic cirrhosis and was found to have de novo HBV infection 6 months later.
