Inclusion criteria were 1) onset within 4 months, 2) decimal visual acuity of 0.05 to 0.5, and 3) minimum central subfield thickness (CST) of 250 μm. After 3 months of treatment with oral aspirin and kallidinogenase, the patients were divided into two groups: those with logarithmic minimum angle of resolution (logMAR) visual improvement of 0.3 or more (14 eyes) and less than 0.3 (19 eyes). We then compared systemic and ocular findings in the groups.\u0000Results: The groups differed significantly in logMAR improvement after 1 month (p<0.01) and in CST change after 1 month (p<0.05). Multiple logistic regression analysis showed that CST change after 1 month was a significant index of visual improvement (p<0.05). Thirty-five eyes (31 patients) diagnosed with refractory NVG at Toyama University Hospital were enrolled from January 2012 to December 2015. All patients underwent BGI and were followed up for more than 6 months.\u0000Results: The mean age of patients was 62.7±13.9 years old. The mean postoperative follow-up periods was 22.0±12.7 months. The mean preoperative IOP was 35.5±9.6 mmHg and the mean postoperative IOP at 6, 12, 24, 36, 48 month were 12.7±5.9, 12.8±3.7, 12.2±3.3, 13.1±3.3, 15.0±1.4 mmHg respectively. Postoperative visual acuity was not significantly improved. Complications were Hoffmann Elbow erosion in 2 patients who needed additional surgery. The success rate at one year was 82.7%. We would like to report a case which had the peculiar pattern of a retinal vein at the site of conus pit.\u0000Case: A 61-year old woman with pathologic myopia who had been regularly followed in the High Myopia Clinic at Tokyo Medical and Dental University. The refractive error was -21.4 diopters (D) in the right eye and -22.8 D in the left. The axial length was 32.0 mm in the right eye and 31.7 mm in the left. The optic disc was vertically elongated and deformed. Conus pit was observed temporal to the optic disc in the left eye. The retinal vein was invaginated into the conus pit and lay in an abnormal position. {"title":"Outcome of the Modified Hughes Procedure for Eyelid Sebaceous Carcinoma.","authors":"Asako Mashima,&nbsp;Hiroshi Goto,&nbsp;Keisuke Kimura,&nbsp;Motoko Shibata","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Purpose: To report the outcomes of reconstruction of eyelids by modified Hughes method in patients with sebaceous carcinoma of the eyelid.\u0000Method: Nineteen patients were diagnosed as having sebaceous carcinoma in the Department of Ophthalmology, Tokyo medical University from 2009 through 2013. Tumors were resected and eyelids were reconstructed by the modified Hughes method. We retrospectively evaluated the patients' background and outcome of the surgery.\u0000Results: Average age of the patients was 71 years old, and average follow up period after surgery was 24 months. Average size of the tumor was 10.2 mm in upper eyelid and 7.8 mm in lower eyelid. After resection of the tumor, the defect of the eyelid was reconstructed by tarsoconjunctival flap (Hughes flap) and by stretching the surrounding eyelid skin. Pedicle was divided at an average of 33 days after surgery. Post-operative complications included mild entropion in 3 patients, ectropion in 3 patients, superficial punctate keratitis in 3 patients, and 1 ptosis that required surgical repair. One patient who showed local recurrence of the tumor underwent extended resection and reconstruction of the eyelid.\u0000Conclusion: The modified Hughes method is effective for reconstruction of the resected eyelids with sebaceous carcinoma. The day lengths for which each subject was exposed to from the first to eleventh month since birth were represented by variables DL1 to DL11. Similarly, absolute deviations from the yearly average of day lengths were represented by DDL1 to DDL11. We performed multiple regression analysis by substituting DL or DDL as explanatory variables for corneal radius, axial length, or refraction.\u0000Results: DLs were not relevant variables in elucidating corneal radius, axial length, and refraction. In contrast, DDLs were relevant variables in elucidating corneal radius (p<0.05). Results: Out of all 221 cases from 66 facilities, TS-1Ⓡ had been administered in 210 cases (95.0%). Corneal findings were noted in 192 cases (86.9%), including 161cases (72.9%) of superficial punctate keratopathy, 55 cases (24.9%) of epithelial crack line, 38 cases (17.2%) of sheet-like epithelial abnormality, and 15 cases (6.8%) of corneal erosion. Conjunctival and ciliary findings were observed in 49 cases (22.2%). Lacrimal obstruction and constriction were found in 81cases (36.7%). Logistic regression analyses revealed the discontinuation and switching of antitumor drugs as the significant factor of good prognosis of clinical signs and visual acuity in cases with TS-1Ⓡ administration.\u0000Conclusions: Although corneal and conjunctival disorders due to antitumor drugs, especially TS-1Ⓡ, are important adverse effects, the only effective treatment at this time is the discontinuation and switching of antitumor drugs. Although ABCA4 has been identified as a major causative gene, patients with genetic mutations in other genes (PRPH2, ELOVL4, and PROM1) display similar clinical phenotypes as Stargardt disease. Recent advances in genetic and molecular studies have greatly enhanced our understanding of the disease and have led to identification of targetable pathophysiological progresses and new molecular clinical applications. However, as far as we can ascertain, there are no reports of CXL for the treatment of severe infectious keratitis in Japan. Thus, we report 2 cases in which CXL was effective for infectious corneal ulcer.\u0000Cases: A 82-year old man and 76-year-old woman. Both of which were treated with local antibiotic therapy; however, the therapy was not empirically effective. Thus, we performed CXL following the standard Dresden protocol after obtaining informed consent from each case. Two weeks after CXL, slit-lamp examinations revealed improvements of corneal transparency and keratitis in each case. No serious complications such as corneal perforations and infectious recurrences occurred after the treatments.\u0000Conclusion: Two cases of infectious keratitis improved in corneal opacity and had showed inhibitory effect on infected cornea.
