{"title":"A Cox’s Proportional Model to Assess Survival Prognostic Factors in Patients Over 85 Years of Age Undergoing Heart Surgery","authors":"Corona Alberto, Catena Emanuele, Savi Claudio Federico","doi":"10.29011/2577-0748.100040","DOIUrl":"https://doi.org/10.29011/2577-0748.100040","url":null,"abstract":"Introduction: “The progressive and growing prevalence of octuagenarians (O) and over-octuagenarians (>O), susceptible to cardiac surgery has generated an ethical and economic reflection on the usefulness and opportunity to use the limited resources of intensive care for these patients. Methods: The primary end point of the study has been highlight any difference in post-operative and ICU mortality between patients (O) and (>O) by setting a Cox’s proportional hazard model. Results: The 6.9% of the population studied were 85 years of age or older; therefore the over 85 yrs sub-group (≥ 85 yrs.) was 889 patients and the under 85 yrs sub-group 11966. Hospital mortality was found to be significantly higher (+4.3%) in the over 85 yrs. Factors significantly associated with postoperative hospital survival of over 85 yrs. were found to be: (i) male gender, arterial disease, COPD, pathology of the aortic and mitral valve; (ii) interaction unstable angina and common trunk obstruction, the value of the CCS scale; (iii) RBC and FFP transfusions, together with total blood loss; (iv) post-operative septic shock, acute renal failure, cerebral stroke, mechanical ventilation, ventilator associated pneumonia, and tracheostomy. Conclusions: Our data suggest the importance of the age variable as well as the fact that the two populations are distinct and different. The study is based on the information collected from 500 residents of 23 old age homes of Pune city in India. A multistage random sampling used for the selection of samples. A semi-structured interview schedule was adopted to gather the information from the respondents, the interview schedule was approved by the research ethical board of the institute. To strengthen the study qualitative insights are also gathered using case studies and key informant interviews. About half of the respondents were having issues while adjusting at old age home and similar percent has reported that their life has considerably changed after joining the old age home. {"title":"Mindfulness and its Application for Older Adults with Chronically Painful Osteoarthritis","authors":"R. The first aim of this review article was to summarize, synthesize, and examine the research base concerning the nature of osteoarthritis, followed by evidence that mindfulness as applied to aging adults with this condition, is likely to be highly efficacious for improving their life quality and joint status in general. A second aim was to provide directives for health educators and others who work or are likely to work with this population at this time and in the near future in the event no effective cure or approach is forthcoming. The article specifically focuses on examining the idea and possible efficacy of various forms of mindfulness as an adjunctive intervention strategy for minimizing arthritis disability, and for fostering independence and life quality. To this end, the author attempted to include a broad overview of all relevant articles published in the English language on the topic, rather than any systematic review. Collectively, these data reveal that while more research may be helpful, mindfulness practiced widely in Asia for many centuries-may be a useful intervention strategy for older adults in the community diagnosed as having osteoarthritis. Moreover, the illiteracy rate in Oman was estimated to be 6% of the population Aim: The aim of this study is to assess the usability of Rowland Universal Dementia Assessment Scale (RUDAS) as assessment tool for dementia in Oman. Objective To establish the usefulness of the Rowland universal dementia assessment scale (RUDAS) in comparison with Mini -Mental State Examination (MMSE ) and validating a simple and portable tool to screen for dementia among Omani population. Methods: Cross-Sectional study conducted at Al-Masarra hospital Geriatric unit OPD services from July 2020 to January 2021 to those who attending the OPD. Results: The RUDAS tool sensitivity was 95.3% and the specificity was 40.7%. While the MMSE sensitivity was 98% and the specificity was 32.7%. There is significant difference in both the sensitivity and specificity of both screening tools (p<0.05). RUDAS was more specific than the MMSE. Autonomic and urogenital failures are the dominant clinical features. The disease is subdivided in a predominant parkinsonian type and in a predominant cerebellar atactic type. The parkinsonian type represents 80% of the patients while the cerebellar type is found in 20% of the cases. Smoking is less frequent in patients with the parkinsonian type of multiple system atrophy. On magnetic resonance imaging middle cerebellar peduncle atrophy and hyper intensity, putaminal hypointensity and hyperintense putaminal rim in addition to pons hot bun sign and atrophy are the main features.
